Ep 109Executive Session: Improving telemedicine by training effective "medical virtualists"

As concerns over COVID-19 increased, many physicians have opted to see fewer patients in their offices and expanded their use of telemedicine. However, as practices increase their use of telemedicine, many doctors and patients have expressed concern that the level of care may not be equal to the traditional office visit and that telemedicine may compromise the physician-patient relationship. At the same time, some healthcare leaders describe the problems with telemedicine as growing pains that can be overcome with training and increased familiarity with the technology. In this episode, Dr. Michael Nochomovitz, Chief Clinical Partnerships Officer, Devoted Health, and the former Senior Vice President and Chief Clinical Integration & Network Development Officer, New York Presbyterian; and Dr. Rahul Sharma, Professor and Chairman, Department of Emergency Medicine, Weill Cornell Medicine / New York-Presbyterian, New York, discuss their work in advancing the idea of a "medical virtualist" and the skillset needed to effectively deliver telemedicine. Ep 95‘The Biggest Challenge of My Career’: Keeping Up with Telehealth Coding Changes During COVID-19

In this episode of the MGMA Insights podcast, we’re joined by Nancy Enos, FACMPE, CPMA, CPC-I, CEMC, independent consultant, MGMA, principal, Enos Medical Coding. In her 40 years in practice management, Enos has never seen as many major – and frequent – coding and billing changes as she has since the start of the COVID-19 crisis. Many have stemmed from the rapid expansion of telehealth services, which, while challenging for billing staffs, have proven rather convenient for patients. “I’m hoping that after this is over, there’ll be a lot of consideration before things go back to the previous stringent rules about originating site and telehealth platforms,” Enos said. “Now that patients have been introduced to this service delivery, once you give something, it’s very difficult to take it away.” Enos and other industry experts will be presenting an MGMA online seminar May 21 on “Coding Essentials for the Non-Coder,” with a spotlight on telehealth and COVID-19. Ep 82COVID-19: How One of Northern California’s Largest Health Systems is Responding to the Crisis

In this episode of the MGMA Insights podcast, we continue a series focused on COVID-19 and how it’s impacting healthcare professionals and their patients. Throughout this series, we’ll share conversations with experts regarding challenges such as staffing, communication, emergency preparedness, patient education, advocacy and more. You can keep up with the latest by visiting the MGMA COVID-19 Action Center at MGMA.com/covid. Our guest is Peter Valenzuela, MD, MBA, chief medical officer, Sutter Medical Group of the Redwoods. Here are a handful of highlights from his interview: - (6:54) “We actually have calls every morning at 7 a.m. with the local team, and it includes our managers and our physician leaders. We talk about whether there's any new cases, whether we have any PPE equipment or use that we've had, and then whether there's anybody out on furloughs or out on leave secondary to this COVID-19 situation. I think it's keeping everybody on the same level as far as what we're doing and what our plans are.” - (9:29) “We've actually been really good about being efficient with our equipment because we set up an actual COVID clinic, where we screen out patients who answer positively to the screening questions. … If they answer positively to those, we redirect them to a separate clinic that we have where we do nothing but COVID-19 screening. That has kind of helped shift those patients from our main care center sites to one area where we can control the use of the testing kits, as well as the PPE.” - (21:24) “I think that the benefit with the fires for us was that we knew that [it] eventually was going to come to an end and then we'd be able to start up again, and we were able to plan around the fire containment. With the COVID virus, it's different because it's so novel and we don't know when we're going to reach a peak and when it's going to come down.” MGMA Insights is presented by Decklan McGee, Rob Ketcham and Daniel Williams. In this special episode of the MGMA Insights podcast, we continue a series focused on COVID-19 and how it’s impacting healthcare professionals and their patients. Throughout this series, we’ll share conversations with experts regarding challenges such as staffing, communication, patient education, advocacy and more. You can keep up with the latest by visiting the MGMA COVID-19 Action Center at MGMA.com/covid. Joining us here is Kelly Ladd, CMPE, CEO, Practice Administrator, Piedmont Internal Medicine. Here are highlights from her interview: * “I just don’t feel like some of these folks understand what’s happening on the frontline of the physician office. They don’t understand that we are not able to get basic protection like surgical masks, gloves are on backorder, and that we cannot do these tests.” * “We don’t have the test, and if we had the test, we can’t perform the test because we don’t have the proper safety equipment to do so. Thus far, I don’t know if I have any huge momentum in getting the attention of some of our officials to help us out – but I sure am trying.” * “To me, it seems that if people are going to put the doctors and the doctor’s offices on the frontline in testing the virus, that people need to make sure we have the resources to do so. I kind of equate it to, if we’re going to war and the Marines land, would you send the Marines in without a gun? Ep 78Take it to Heart: A Cardiologist’s Guide to Leveraging Clinical Analytics

In this episode of the MGMA Insights podcast, we’re joined by Joe Ebinger, MD, clinical cardiologist and director of clinical analytics for the Smidt Heart Institute at Cedars-Sinai Medical Center. Ebinger is passionate about using data-driven solutions to improve healthcare quality, decrease costs and increase value at a population level. The following are highlights from Ebinger’s interview: • “Data for data’s sake isn’t worth anything. What you want to be able to do is take that and turn it into something that is actionable.” • “The hard part within healthcare systems is there’s no single platform that can handle all aspects of patient care. The EHR doesn’t measure the hemodynamics in our cath lab or run the CT scanners. The EKG machines don’t talk to finance. Registry data is extracted from notes and submitted to external systems. It’s really pulling this all together, which I think is one of the most integral parts of what we do.” • “There’s so much clinically available data that you can look at. Don’t just let it float out into the ether. Harness it. Validate it and integrate it with the disparate data sources across your institution and partner with those caring for patients. If you can do that, you’re going to be able to really improve the lives of your patients every day.” Here are some links and references related to this week’s show: • Data Mining Techniques Could Improve Cancer Gene Therapies (bit.ly/38DRm1p) • One-Third of Young Providers Overwhelmed by Patient Data, Analytics (bit.ly/2IG95KZ) • The data-driven transformation in healthcare: Cedars-Sinai CIO outlines his strategy (bit.ly/2xst3qv) If you like the show, please rate and review it wherever you get your podcasts. Ep 77The 4th Industrial Revolution – Shaping the Future of Work and Education

In this episode of the MGMA Insights podcast, we’re joined by Heather McGowan, a keynote speaker, author and future-of-work strategist. McGowan is set to close MGMA20 | The Financial Conference with a general session presentation titled “The 4th Industrial Revolution: A New Leadership Imperative.” To see McGowan’s list of recommended reads or to browse any of her articles, visit linkedin.com/in/heathermcgowan. Highlights from McGowan’s interview: • “Only about 27% of people ever work in the field of their undergraduate major, yet we myopically focus on it. And those 27%, I would argue a good percentage of them are faculty. So, most of the people who are telling you about your future are telling you about their own experience, which probably won’t be yours.” • “If you think of all the things you do in a day, how many of them can you point back to because you were good at that in high school? Very small percentage. What we should be focusing on instead is, what are you interested in? What are you curious about? Because you’re going to have to learn and adapt for life.” • “If you think about that person you had on your team or the person you interviewed who kind of irritated you because they asked you a question you hadn’t thought about, hug that person. We’ve compiled a handful of highlights from Dookeeram’s and Aponte’s interview for your convenience: • (6:17) “Core to what we do and what you see in a lot of in-home care models is really focusing on our senior population. We certainly have an aging population. We know that nine million seniors are going to be homebound in the next 10 years, and that certainly warrants new solutions that can meet them where they are.” – Anne Marie Aponte • (7:05) “The consumer demand and the rapid growth of models like Uber and Netflix and things that put products or services at consumers’ fingertips really have changed the framework of how consumers think about healthcare and why can’t they get the same thing from their healthcare providers.” – Dave Dookeeram • (14:52) “At the end of the day, sickness doesn’t wait for a brick and mortar facility to be open. And our firm belief is in meeting those consumers where they are, and meeting them where they are might mean 9pm on Christmas Day.” – Dave Dookeeram • (33:38) “There is something so special about being in someone’s home. … The amount of information you learn about a patient and their individual barriers to care just by being under that roof with them or wherever they are – if it’s the senior living community or elsewhere – that is stuff that you just can’t see on a claim, you can’t see traditionally in a medical chart.” – Anne Marie Aponte Here are some links and references related to this week’s show: • Redefining On-Demand Care in Senior Populations (bit.ly/3c8IQuo) • On-demand Health Care Is as Easy as Ordering a Pizza (bit.ly/2SVJoMA) • An Uber for Health Care? Ep 74Insights: Welcome to the Energy Revolution – Prioritizing Recovery for Medical Professionals

In this episode of the MGMA Insights podcast, we’re joined by Sara Ross (bit.ly/38CtUmc), a keynote speaker, a leadership strategist and the Chief Vitality Officer at BrainAMPED (bit.ly/2P81UiC). Ross is set to open MGMA20 | The Financial Conference with a general session presentation titled “Welcome to the Energy Revolution.” If you're interested in attending The Financial Conference, we’ve got an exclusive $200 discount for podcast listeners looking to join us March 5-7 in Nashville. For more information or to register, visit mgma.com/TFC20 and use the promo code POD20 to receive your $200 discount. We’ve compiled a handful of highlights from Ross’s interview for your convenience: • (15:37) “I’ve seen leaders create environments where they’ve inadvertently made themselves indispensable. And that I think is a very, very dangerous place for people to be, because that means that they have to be a part of every decision. They have to attend every single meeting. They have to respond to every single email. ” • (16:57) “The word ‘self-care’ comes up, and we have a very narrow view of what that is. Sometimes we think that vegging out in front of Netflix is going to be the way to do it and not working is going to be helpful, but we just sit there and we scroll through our emails. Actually teaching people strategies to disconnect from the work that they’re doing and be present where they are is the second side of things that we don’t learn about because we’re so busy trying to work out of all the overwork that we’re experiencing.” Here are some links and reference related to this week’s show: • Hope Won’t Change Things, but this Simple Exercise Will… (bit.ly/3bNk4jk) • The ROI Of Leadership Development In The Rapidly Changing Healthcare Industry (bit.ly/2vGgGX0) • Helping Medical Professionals Avoid Burnout and Suicide (bit.ly/37AUDhp) If you like the show, please rate and review it wherever you get your podcasts. Ep 72Insights: 2020 Vision - MGMA’s Healthcare Predictions for the Decade Ahead

In this episode of the MGMA Insights podcast, we’re joined by Anders Gilberg, Senior Vice President of Government Affairs at MGMA. Headquartered in Washington, D.C., Gilberg and his team recently wrote and released an article (bit.ly/2Up1hVj) on healthcare's outlook for the 2020s, which he discusses in-depth in this week’s show. We’ve compiled an assortment of highlights from Gilberg’s interview for your convenience: • (18:06) “While I personally believe in these predictions … these are not from my personal experience. These are from the experience of the 50,000 MGMA members that can help inform what we do here in Washington.” • (30:34) “It almost sounds overly simplistic. It’s like flying a plane without any data or instruments. But the fact is that that’s what the government has asked our members to do in many respects with these value-based care programs. … A lot of the programs lack data.” • (33:27) “I think the true definition of disruption is that you’re disrupting something that exists today, and I’m not sure we know exactly how healthcare will be disrupted.” • (42:54) “I think working together that we can have a really positive impact on our healthcare system and what we do here in Washington. That’s what a collective voice of an organization like MGMA can do.” If you like the show, please rate and review it wherever you get your podcasts. Our patients are more savvy than ever before. They're consumers who are more involved and question more than ever before. That, I think, has been the biggest impediment is that we haven't kept up well with an age of consumerism.” • (37:50) “When you look at patient satisfaction data, that is people talking about their experience when they've been a patient with you. Nobody's been measuring the ones that got away. So, unless you're not concerned with building value or building your business, phone calls are really important.” Here are some links and reference related to this week’s show: • The Power of the First Phone Call: Factors Influencing Patients’ Likeliness to Return to a Medical Practice (bit.ly/2RknL7W) • A Healthcare Wake-Up Call: Prepare For The Patient Experience Of The Future (bit.ly/2NUNKk4) • Your patient has something to say: Taking customer service insights from the patient’s perspective (bit.ly/3azpXA9) • The role of professional intimacy in patient experience (bit.ly/36kW6Ih) • 3 Surefire Ways to Improve Your Patient Experience in 2020 (bit.ly/36kIm0e) If you like the show, please rate and review it wherever you get your podcasts. You have to keep checking your priorities to make sure they’re in line.” • (6:28) “I think there’s so much peace, clarity and confidence that comes when you say, ‘I’m doing what only I can do.’ I’m a physician, and at work, I don’t do a lot of administrative tasks that would be better suited to somebody else, and I’m able to have the confidence and knowledge and people around me that I can really focus on what only I can do. And at home, I’m able to focus on my family and my spouse and my marriage and my health and my well-being and all those things because I’ve got good systems in place that keep me out of the weeds of life, that allow me to focus and do what only I can do.” • (25:10) “You can’t just go and go and go, and you don’t want to be a productivity RVU machine, because you will burn out. There’s no way to prevent that if that’s the bottom line of why we’re doing this. It really has to be about, I can be efficient at work, and I can be efficient in my home life, and if I can be more efficient at work and see patients in a shorter period of time, that gives me more time to be at home.” Here are some links and reference related to this week’s show: • Physician burnout costs industry $4.6B annually (bit.ly/36adnnl) • Don’t Do Anything Someone Else Can Do 80% As Well (bit.ly/3asNSAW) • Where Does the Time Go?
