C Notes was created to ameliorate the pain of note writing. Most medical notes have a lot of the same boilerplate information with minor variations. We sought to capture the 60% of dental notes that contain a lot of the same information with minor variations. At the same time we created a method by which the remaining 40% of optional/additional, truly unique, entry components for the complex parts of the note could quickly be entered also. Thus, allowing the practitioner to write complete detailed notes efficiently. For a single dentist practice (90% of US practices are solo practitioner) the struggle is to be able to maximize the number of patients that can be seen in a day. Waiting until the end of the day to write notes can lead to errors in memory and errors/omssions due to end of day mental fatigue. Detailed clinical notes are a necessity writing notes is not a billable part of the job, but is nonetheless important to document what was done in a procedure for future visits or in the event of a malpractice lawsuit. Dr. Allen Honigman, originally from Ottawa, Ontario in Canada, received his B.Sc in Biochemistry from the University of Ottawa. He then pursued an honors degree in Genetics at the University of Western Ontario. He then attended Idaho State University, and received his Masters degree in Microbiology and Immunology. After working towards a Ph.D. in Microbiology at the University of Texas Health Science Center at San Antonio, he decided on a dental career and went on to complete his Doctorate of Dental Surgery in 1991 at the University. From there, he went directly to UCLA, where he finished his periodontic specialty training residency in 1993. Dr. Honigman practiced in Southern California until he moved to Guam, in 1995, to accept a position as a periodontist and dental director for Pacificare. While there, he had the opportunity to lecture at local dental society meetings. In 1999 Dr. Honigman was offered a faculty position as the Director for Undergraduate Periodontic Clinical Education at Case Western Reserve University in Cleveland, Ohio where he was also awarded "Clinical Faculty of the Year" by the dental students. While there, he also gave a continuing education course on Periodontal Esthetic Surgery for the Cleveland dental community. In 2001, he moved to Arizona to practice and in 2003, opened his periodontics and implant practice in Chandler. He began utilizing CT guided implant treatment planning and placement in early 2004, and is one of the most prolific users of the technology in Arizona. Andrew Johnston, RDH: I am a Registered Dental Hygiene graduate from the Yakima Valley Community College Dental Hygiene Program in 2009. I hold three degrees including a Bachelors Degree in Business Management and currently working towards my MBA. First and foremost, I am a husband and a father to my wife and three children. When I have time to get away, I regularly lecturer at dental hygiene programs throughout the USA. I also speak at dental hygiene societies and associations. In 2013, I was asked to be a faculty member for DDS - Dental Development Seminars. My role includes mentoring and instructing dental professionals in technique driven delivery of local anesthetic. As the only hygienist on the faculty, I focus on being a liason between the dentists and hygienists to build trust and understanding between the two groups by helping ensure competency, accuracy, and confidence in the hygienist attendees. I also write, blog, and am a regular contributor on Hygienetown.com. I write articles for a number of publishings including my own blog http://andrewjohnstonrdh.blogspot.com. I am a regular panel member for Mark Frias' roundtable discussions and have been interviewed by Dr. Howard Farran for his Dentaltown podcast. In the future, I hope to be lecturing at regional and national conferences on my local anesthesia review course. My "Charitable Dentistry" lecture is a compilation of many years of mission trips and charity work throughout the world. I have volunteered my time and efforts with local programs such as the Union Gospel Mission's dental office, YVCC sealant days, the Boy Scouts of America, and as a coach for the local parks and rec teams. Nationally, I have consulted for dental teams and offices interested in learning how to participate in charitable ventures. I provide research that is specific to the desires of the team at no charge to them. Internationally, I have been on mission trips in the Philippines, Guatemala, and the Dominican Republic. Linda Douglas, RDH: Linda M Douglas graduated as a dental hygienist from the Royal Dental Hospital in London, England in 1982. After graduation she worked in periodontology before moving to Toronto, Canada where she has worked in private practice since 1990. Her desire to support patients with eating disorders has instigated in-depth study of eating disorders, and its effect on oral health. Daniel Lopez, RDH: Daniel Lopez is a dental hygienist currently working at the Callen-Lorde Community Health Center in New York. Currently the only two cites in Japan that use CWF are Niigata and Kyoto. Dr. Miho Imamura teaches orthodontics in Tokyo 10 times per year to both orthodontist and general dentists. She says the orthodontists don't like it when she teaches general dentists orthodontics but she says general dentists need to know and understand orthodontics. Dr. Miho Imamura teaches myofunctional therapy four times per year in Tokyo to both orthodontists and general dentists. Her mentor in myofunctional was Bill Zickefoose in Iowa who passed away in 2015. Her other mentor is Dr Toshihide Ohno in Yokohama, Japan which is a suburb of Tokyo. Dr. Shinya Minami met his wife in dental school and they were the only ones in their class that got married. His wife after dental school went seven more years of college to become an orthodontist. Their dental office hours are 9:00am to 7:00pm Monday, Wednesday, Thursday, Friday, Saturday, and closed Tuesday and Sunday. The dentist will see about ten patients per day for about one hour per patient. The hygienist appointments are one-hour long. When a patient is missing a molar about 40% will get a one-tooth removable partial, 40% will get a fixed 3-unit bridge, and 20% will get an implant crown. About 10% of Japanese dentist can read English.
