Dr. Jensen received his bachelor's degree from the University of Utah. He completed his DDS Degree at the Northwestern University School of Dentistry, and his Anesthesiology Residency at the Northwestern University School of Medicine. He completed his Oral and Maxillofacial Surgery Residency at the University of Michigan. Dr. Jensen is a Diplomate of the American Board of Oral and Maxillofacial Surgery and a Fellow of the American Dental Society of Anesthesiology. He is interested in bone grafting, dental implants, and distraction osteogenesis. He is currently Clinical Assistant Professor at New York University, University of Michigan, and University of Colorado at Denver. Dr. Jensen has held a variety of teaching positions since the early 1980s, and has also served as a clinical instructor in the St. Joseph General Practice Residency Program. Dr. Jensen has been a guest lecturer at scores of seminars and symposia, with a strong focus on implant dentistry, bone grafting, and maxillofacial surgery throughout North America, as well as in Europe and Japan. Howard sat down in Kofu, Yamanashi, Japan with orthodontist Dr. Miho Imamura DDS, PhD who learned to speak English in Iowa where she traveled to and from Des Moines from 1985 to 2010 to study American dentistry and CWF - Community Water Fluoridation. Currently the only two cites in Japan that use CWF are Niigata and Kyoto. Dr. Miho Imamura teaches orthodontics in Tokyo 10 times per year to both orthodontist and general dentists. She says the orthodontists don't like it when she teaches general dentists orthodontics but she says general dentists need to know and understand orthodontics. Dr. Miho Imamura teaches myofunctional therapy four times per year in Tokyo to both orthodontists and general dentists. Her mentor in myofunctional was Bill Zickefoose in Iowa who passed away in 2015. Her other mentor is Dr Toshihide Ohno in Yokohama, Japan which is a suburb of Tokyo. Dr. Shinya Minami met his wife in dental school and they were the only ones in their class that got married. His wife after dental school went seven more years of college to become an orthodontist. Their dental office hours are 9:00am to 7:00pm Monday, Wednesday, Thursday, Friday, Saturday, and closed Tuesday and Sunday. The dentist will see about ten patients per day for about one hour per patient. The hygienist appointments are one-hour long. When a patient is missing a molar about 40% will get a one-tooth removable partial, 40% will get a fixed 3-unit bridge, and 20% will get an implant crown. About 10% of Japanese dentist can read English.

Ken graduated with PHD in chemistry in 1970. He has worked as a research scientist ever since, mainly on soils and agricultural issues. His early research investigated the reaction of phosphate and fluoride with minerals and soils. Later work involved investigation of soil life and nutrient transfers. He also has done research on apatites and fluorosilicic acid, which are relevant to the controversy around community water fluoridation (CWF). In 2013 he became active on online debates about CWF. His local city council had become influenced by anti-fluoridation campaigners and stopped fluoridation, despite public support from it as evidence by a previous referendum and council polling. After protests the council held another referendum, fluoridation received 70% voter support and CWF was restarted. He writes a blog, Open Parachute, where he discusses scientific and related issues - particularly science denialism. P.I.E. offers a claims-made insurance policy which is the current industry standard. Coverage is available for dentists who practice hospital dentistry or perform I.V. sedation, either themselves or with the help of a qualified anesthesiologist or nurse anesthetist, in their own office.
