S7 Ep 406I'm 44 with a history of miscarriage. I just discovered I'm pregnant. What can I do to monitor this pregnancy? (Ask the Egg Whisperer)
In this episode, a listener sends in a question to Dr. Aimee. Here's what she asks, "My partner and I have been trying to have a child I'm 44 years of age, and my partner is 36. I've had two successful prior pregnancies, and we also lost two pregnancies after conceiving naturally. The losses occurred around weeks 10 and weeks 12. But now I found out that I'm pregnant and after having tested positive for HCG and missed my period by only a few days. Here are my questions. I know that my eggs are old and that this embryo may not hold. Is there a way that a specialist can monitor the early stages of pregnancy to see if this egg is a good or bad one? It's troubling to think that I have to wait for my 10 week mark and possibly miscarry. My partner and I don't want to go through another miscarriage, understanding that I'm in the 8 to 10% chance of having a good egg. His sperm meet the criteria for IVF, and here is what she asks, "We were given surprisingly good odds for success and I'm skeptical.I've had one successful pregnancy 15 years ago. Our doctor gave us 60-70% chance of pregnancy using ivf, icsi and genetic testing of embryos. Do you feel this is an accurate assessment of our odds of success? She asked if I'd talk about when a natural FET is indicated, as she has had three failed euploid FETs, and would like to increase her chances of pregnancy with her remaining 6 embryos. Here's her question, "Hi Dr. Aimee, thanks for all you do! Can you talk about when a natural FET is indicated? I've had three failed euploid FETs with an ERA and increasing medication intervention each cycle. My RE has suggested trying a natural FET since I was able to get pregnant on a letrozole IUI in 2019 (but miscarried just before 10 weeks). What are the pros/cons of natural, and would you agree with this approach next? I have 6 embryos left and if I cannot get pregnant, my wife would go next, but she really doesn't want to carry and I desperately do. Thanks for your thoughts!" S2021 Ep 392I have DOR and have done 5 rounds of IVF. What can I do to increase my chances of pregnancy? (Ask the Egg Whisperer)
Jennifer sent in a great question to "Ask the Egg Whisperer." She is 38, and from Seattle. Here's what she wrote," Hi Fertility Queen, you are the BEST! I'm 38 years old & diagnosed with diminished ovarian reserve. I've been through 5 rounds of IVF and have 2 5AA frozen embryos right now. My husband and I want 2 kids therefore tried to bank another embryo but the 5th round failed therefore we are moving on to a FET. I did a mock transfer and the BCL6 from the Receptiva came back positive therefore we are doing 2 Depot Lupron shots a month apart prior to the real transfer. My environment is good (healthy eating, lots of exercise, limited toxins in our beauty and cleaning products, great therapist). I'm taking a Prenatal with folate and DHA, Vitamin D, & Vitamin E. Are there any other supplements I should take? I've heard NAC may be good for endometriosis. Is there anything else I can do to prepare for this transfer? I am willing to do whatever it takes to help my pregnancy chances. Thank you so much!" The brief answer for Jennifer is this: 1. Have you done the ERA test on the two embryos you have? 2. Do you have symptoms of endometriosis? Consider the ReceptivaDX test, as it can determine if you might have silent endometriosis. 3. I do not think I saw it mentioned on your blog along with the other supplements and I haven't heard you mention it to anyone during your podcasts." My short answer? I definitely recommend fish oil and it's in most of the prenatals out there. And the prenatals that I recommend as well to my patients have fish oil in them too. S6 Ep 386I'm 37. Should I try IUI or do IVF? (Ask the Egg Whisperer)
In a recent Ask The Egg Whisperer, Barbara from Cyprus sent in a question as she weights the options of IVF. Here's what she asked, "I am 37 years old and I have a 5 year old boy. For the last 1 year we have been trying to get pregnant for a second child. I tried to follow your TUSHY Method and I think I got most of it done. I had polyps removed, AMH is 1.92, FSH 10.2, thyroid is normal, sperm is very good, tubes are open. My doctor is suggesting to do IVF without trying IUI first because of my low AMH and my age. However, I am very reluctant and scared to try IVF as I feel that maybe there is something I can "fix" and conceive naturally. Am I being unreasonable? Should I just bite the bullet and do it or do you suggest anything else I could do or test before IVF?" This is such a great question, and I totally get it. If I could help every single person get pregnant without IVF, I would do it. My guess is that your doctors don't want you to waste your precious time. They don't want to take your fertility for granted and they want to move forward with the healthy exit you have at the age of 37, it's harder to get pregnant with a healthy egg. However, given your history, you probably still have a really good chance of having a healthy pregnancy and could IUI work with you. But, it might be more efficient to go right to IVF first. And that's something that I recommend to everyone who is 37 and above. However, if your gut is telling you to consider IUI first, just do one or two rounds of it. If it doesn't work, then you can go to IVF. Make sure that you share your priorities with your doctor and that they also understand those priorities. Here's what she want's to know, "What is your advice for women over 40 who are undergoing hormone induction and probably will do IUI/IVF vs COVID vaccine? Is it advisable to get the vaccine if a woman has future plans to have kids? Thanks, and God bless." My recommendation is get the vaccine. I want all my patients to be fully vaccinated. Fully vaccinated means is that they have had their second shot. If they're doing the two shot Pfizer or Moderna vaccine, you will want to have that second shot two weeks before their embryo transfer, and it's the same thing with doing an IUI. Recently, Carrie wrote in to Ask The Egg Whisperer with a question. Here's what she she said, "Hi Dr. Aimee, Thanks for the incredible support you bring our community. I'm 39 and just had my first IVF cycle. AMH is 1.67. On day 9, they counted 25 follicles, but only 8 follicles measured above 11 mm. My protocol was "spontaneous antagonist." I'm feeling really discouraged that my follicles didn't grow in a more synchronized fashion. Is this common and is there anything I can do next time? I can't help but feel the remaining follicles that measured too small were just a waste." This can happen, and eight follicles is still pretty great. Heidi recently wrote in to Ask The Egg Whisperer, and here is her question, "I had healthy baby boy end of February this year, no issues during pregnancy. He passed away two days after birth (birth asphyxia, umbilical cord around neck, too long labor, infection, placental abruption and C-section was done too late). I got pregnant naturally after TTC 10 months. I have also 19 year old son. I have regular cycle 28days, I'm healthy, no PCOS, miscarriages or other issues. I'm using prenatal vitamins, omega3, coQ10, Maca and Vitex, d-vitamin, iron, folic acid. Physically I've been healing from surgery really good, mentally it's been hard. I want to test my estrogen, progesterone, FSH, LH and iron levels but is it too early to test these because I was just pregnant? How fast hormone levels are getting back to normal after labor? S6 Ep 375The Most Important Thing to Know Before IVF with guest Dr. Max Ezzati
I talk about the IVF Pyramid with all of my patients. It helps us visualize and prepare for what happens during IVF and what to expect. Today on the Egg Whisperer Show, I'm delighted to have the inventor of the IVF Pyramid joining me! So often in IVF, we talk about things in terms of depletion. What are you losing? For example, you start off with a certain number of eggs at the beginning of the cycle. That's called the antral follicle count. As the cycle progresses sometimes not all of those eggs grow. In the end, for example, you may gain two healthy embryos from twenty follicles and so it is a natural and normal human response to feel like something is lost and that something was taken away from you. I want to flip that on its head. Instead, you've started with twenty eggs from which you've now gained two embryos. So it took twenty follicles to get the two eggs from that cycle. Dr. Ezzati's pyramid makes this concept much easier to understand. I'm excited to have this conversation during National Infertility Awareness Week! Kim recently wrote in to ask the Egg Whisperer this question, "Hi Dr. Aimee. I am 34 years old. Recently I was diagnosed as PCOS due to the number of follicles on my ovaries and my acne problem. Before I began my IVF journey, I was diagnosed with IBS (possible Celiac disease or gluten sensitivity that had yet to be confirmed), hypoglycemic, and severe vitamin D deficiency (13L). My question is, is there any correlation between IBS or Celiac disease, Vitamin D deficiency and hypoglycemic (fyi my BMI is 24 and I am not diabetic) and infertility? If so, should I postpone my FET on 04/30/21? What can I do to give myself a better chance of having a successful IVF if I do decide to continue with the FET?" Being a fertility patient is like learning a new language. It's almost like you have to adopt a new vocabulary of only three-letter words. In today's Egg Whisperer Show podcast, I'm sharing about PGT. PGT = Pre-Implantation Genetic Testing When I talk about this to patients, one of the first reactions I get is that they think it's associated with a 100% pregnancy rate and a perfect healthy baby. The other myth I hear is that most think they don't need it because they don't have anyone in their family with chromosome issues. Both of these statements are myths. PGT is the closest thing to a crystal ball we have right now but it's still murky. It's the best that science has to offer to give us the highest chance for a healthy pregnancy but it has limitations: it only screens chromosomes, not genetic diseases, birth defects, or genes for autism. My husband has no sperm issues as I am on all the supplements you suggest. I had 2 rounds of IVF last year with no pregnancies. All of my hormones were great and my structures are great. Once with Lupron and once with Cetritide. The RE's were baffled. They diagnosed me me with poor egg quality and a blocked tube. I was not ready to give up. I went to my very open OB-GYN and I did my own research after asking you about possible endometriosis. He agreed it may be the problem even though I don't have any issues. I had the laparoscopy and there was endometriosis in 6 areas! And tube is not blocked. I am sad I had it but glad I listened to you when you said a lot of times it could be the hidden culprit. Not saying it's a guarantee that solves it, but it's a step forward. I'm tearing up as I write. Thank you so much! S6 Ep 365Environmental Toxicant Exposure and Your Reproductive Health with Dr. Kevin Osteen
I'm so excited that Dr. Kevin Osteen is joining me this week for the Egg Whisperer Show. He is a Professor of Obstetrics and Gynecology, Pathology, Microbiology and Immunology at Vanderbilt University School of Medicine and Adjunct Professor of Obstetrics and Gynecology at Meharry Medical College. Since the 1990s, Dr. Osteen's research program has focused on environmental endocrine disruptors and how they impact endometriosis. His laboratory developed "organ-on-a-chip" technology, which is a tiny plastic "chip" about the size of a USB thumb drive that mimics the workings of the microenvironment of a human organ. Dr. Osteen's team created the first endometrium and endometriosis chip models. These "organ-on-a-chip" models provide a way to test and observe what happens in "normal" endometrium, and in those that have endometriosis. This technology changes the way doctors study this disease. Dr. Osteen and I will be talking about the organ-on-a-chip technology, endometriosis, and endocrine disruptors, and reproductive health. Being a fertility patient is like learning a new language. It's almost like you have to adopt a new vocabulary of only three-letter words. In today's Egg Whisperer Show podcast, I'm sharing about PGT. PGT = Pre-Implantation Genetic Testing When I talk about this to patients, one of the first reactions I get is that they think it's associated with a 100% pregnancy rate and a perfect healthy baby. The other myth I hear is that most think they don't need it because they don't have anyone in their family with chromosome issues. Both of these statements are myths. PGT is the closest thing to a crystal ball we have right now but it's still murky. It's the best that science has to offer to give us the highest chance for a healthy pregnancy but it has limitations: it only screens chromosomes, not genetic diseases, birth defects, or genes for autism. S6 Ep 360I was told that my uterus is "too large." Do babies prefer condos to mansions? (Ask The Egg Whisperer)
Recently, Jessica wrote in to Ask The Egg Whisperer. Here's what she says, "Have done 2 transfers back-to-back. Last one was a confirmed fail. I was told at that sonogram that my uterus is too large. I have had PCOS and had two surgeries, the last time was 5 years ago. She said that the average size of a uterus is 7cm x 3cm and mine was 7cm x 8cm and tilted. Do babies prefer condos to mansions?" I love this question and how Jessica has worded it! Here's the thing. If your uterus is enlarged, the one word that I'm thinking of is adenomyosis. So I would talk to your doctor about a pelvic MRI to see what is going on. The other thing that you can do is the ReceptivaDX test to see if there's any type of inflammation in the uterus (but remember: ReceptivaDX is not a test for adenomyosis .It may detect silent endometriosis. But if it comes back negative, it doesn't mean that you do not have adenomyosis.) There's no perfect test for adenomyosis. So, consider: Pelvic MRI, ultrasound findings, clinical signs and symptoms, and laparoscopically looking at the uterus. Those are the ways of figuring it out.
