In this episode, I'm joined by Dr. Geoffrey Sher of Sher Fertility Solutions. He is an internationally renowned fertility expert. He trained under "The Fathers of IVF, " Drs. Patrick Steptoe and Robert Edwards in the UK. In 1982 he established the 1st private (non-university based) IVF program in the US. Over a career spanning almost 40 years Dr. Sher has been influential in the births of over 17000 IVF babies and has helped fashion the entire field of ART. He is the author of " In Vitro Fertilization, the A.R.T. of Making Babies" and (more recently), "Unexplained IVF failure and Recurrent pregnancy loss: The Immunologic Link"." Today he's joining me to talk about Immunologic Implantation Dysfunction (IID), which may be a factor in unexplained IVF failure and recurrent pregnancy loss (RPL). He will be talking about how the endometrial lining plays an important role in embryo implantation, because as he shares, "you can't put a good seed in bad soil, or a bad seed in good soil." S8 Ep 559What is the ERA Test?

The time to talk about "everything I can possibly do to give myself the best chance of pregnancy success" is BEFORE your embryo transfer. Not after. This is why I discuss tests like the ERA test and Receptivadx test with all my patients before transfer. Every embryo is important to me and I want to make sure I'm doing the very best job for every patient I meet.⠀⁠ ⠀⁠ I also know that there's a virtual alphabet soup of three letter abbreviations that surround the whole fertility journey, so this week on the podcast, I'm breaking down the steps that I take when doing the ERA (Endometrial Receptivity Analysis) test, and how it could help you. The ERA tells us exactly when to start progesterone before a transfer to give the embryo the highest chance of implantation success. Think of it as a personal evite to your Embryo Transfer Party! In a way, the test tells you exactly when to show up for the party. S8 Ep 557Is there any correlation between IBS or Celiac disease, Vitamin D deficiency and hypoglycemia infertility? (Ask The Egg Whisperer)

Kim recently wrote in to ask the Egg Whisperer this question, "Hi Dr. Aimee. I am 34 years old. Recently I was diagnosed as PCOS due to the number of follicles on my ovaries and my acne problem. Before I began my IVF journey, I was diagnosed with IBS (possible Celiac disease or gluten sensitivity that had yet to be confirmed), hypoglycemic, and severe vitamin D deficiency (13L). My question is, is there any correlation between IBS or Celiac disease, Vitamin D deficiency and hypoglycemic (fyi my BMI is 24 and I am not diabetic) and infertility? If so, should I postpone my FET on 04/30/21? What can I do to give myself a better chance of having a successful IVF if I do decide to continue with the FET?" Emily submitted a great question to AskTheEggWhisperer.com: "What can I do to thicken my uterine lining? (I'm 37 and we've been TTC for over 6 months. Obgyn wanted me to try progesterone before going to specialist. My issue is a thin lining of 5mm. What can help my lining?" My answer to her was: Let's start by asking your doctor if Asherman's syndrome has been ruled out. An HSG, saline sonogram and hysteroscopy can all help make this diagnosis and ultimately may help treat it too. You can also add medications and supplements like: estrogen, aspirin, l-arginine and vitamin E that may help. S8 Ep 546We're doing IVF, and I am 41. We've had two unsuccessful cycles. Should my husband do genetic testing? (Ask The Egg Whisperer)

First round, we got 33 eggs retrieved, 19 fertilized, eight PGS tested, and only one normal. This was transferred, but wasn't successful. Same thing with a second cycle. I thought that was a problem with my chromosomes. So I had the karyotype test, which came back normal. My husband has two healthy sons from his previous marriage, so the doctor didn't think it was worth him having the test. Do we just accept that age is the problem here and my eggs aren't good quality? Should we just be happy with our one healthy embryo or should my husband do the karyotype?" S8 Ep 543How Old Is 'Too Old' To Have A Baby?

If you're hoping to get pregnant at some point, a question on your mind may be, "how old is too old to have a baby?" It's a question I get all of the time, and so I want to shed some light on factors to consider when it comes to age and your fertility. One of the things you'll want to look at is your Chronological Age versus your Biological Age. One will tell you the age of your eggs based on how long you've been on the planet (chronological), and one will give a sense of the age of your eggs based on your age, genetics, and environmental factors (biological). Some women's eggs age faster or slower than others, and you can determine that by getting your hormones checked (the "H" of the TUSHY method). Visit TUSHYMethod.com for more information. S8 Ep 541I've freezing my eggs and embryos (with donor sperm) to preserve my fertility as a single woman. I want to become a mother. Should I try a round of IUI first, or move right into an embryo transfer? (Ask The Egg Whisperer)

In this Ask The Egg Whisperer episode, Penny sent in a question. Here's what she asked, "I'm 38 and a half. In the past six months, I've done three egg retrieval to preserve my fertility. In cycle one, I had 15 eggs frozen. Cycle two, four embryos with donors from one normal five AB one, lone mosaic, five DB. Cycle three, seven embryos. Five day five, one day six, one day seven waiting for PGT results. I'm hoping to start a family as a single mom by choice. I want to try to conceive as soon as next month after having an HSG. I ultimately want to have two kids, maybe both with the same donor or possibly with a partner with my frozen eggs, depending on life unfolds. And my fertility doctor recommended trying at least one IUI now that I've eggs and embryos banked, because I could possibly avoid the cost of a transfer and avoid shots. This sounds appealing, but shots sound terrifying. Would you recommend trying IUI with donor sperm first? What are the pros and cons? If yes. S8 Ep 540Why do some doctors recommend taking a one month break between egg collecting cycles? (Ask The Egg Whisperer)

In this Ask The Egg Whisperer episode, Gulbahar sent in a question. Here's what she asked, "My doctor recommended that I take a one month off between egg retrievals. There are only so many times to ask 'why' and not be answered before feeling you can't ask again. What is the science between a month wait? I have low ovarian reserves, so overstimulation is not likely. Why do some women have back to back IVF cycle while others don't?" And the answer is you don't have to take a break. You really don't. I always say when your period starts, you think of me and you come on in by cycle day three, you just keep showing up. I say this because you just never know when you're going to have an awesome cycle. So how would you know that the cycle right after one egg retrieval, is going to have a lot of sparkling eggs unless you actually go and look? So I would advocate for yourself. Ask your doctor if you can come in at the time of your period, just to check, understanding that you might have an ovarian cyst that might make it so that you just need to take a break. I don't necessarily recommend a one month break. I do have patients that just feel like they want to keep coming in cycle after cycle without a break. But if let's say you're 41 years old, you have a follicle count of three the next month after your egg retrieval, you could have a follow up count of four. So I would suggest, showing up, find out what's happening in that cycle and then make a decision as to whether you should take a break or wait for your next cycle. Today on the podcast, I'm sharing a recent Ask the Egg Whisperer episode. I love answering your questions, and Natalie sent in a great one about a bicornuate uterus. Here's what she wrote, "Does having a bicornuate uterus affect fertility? What can be done to make sure the pregnancy sticks and remains healthy so the baby is born full term?" Having a bicornuate uterus is very special. When I see someone who comes to me with a bicornuate uterus, I like to learn more about the patient's anatomy so we can create a plan and build a team to support the patient. First, make sure that you have an accurate diagnosis. You'll also want to get an understanding of your own unique bicornuate uterus' anatomy. Depending on your anatomy and diagnosis, the good news is that it is possible to deliver a healthy full-term baby. It's also true that a bicornuate uterus may cause pregnancy complications. I would recommend talking to your OB GYN to see what they think, and ask what their experiences with patients just like you. Ask for their approach on managing your bicornuate uterus. I got my eggs frozen at 36, after my fiancé passed away. I got 13 eggs. My AMH was 5.7. FSH was 5, and it's now a year later. And I'm still worried about my chances. I really want two kids someday. Should I freeze more eggs when I can? FYI, my mother and grandmother both went through menopause at 42. Thank you for your time and advice." Here's what she had to say, "My question is about IVF with poor response and low AMH. I've had four failed IUIs, and two rounds of IVF. The second round gave us three eggs, one blast that was normal, but my PGS transfer my FET did not work. My AMH is 0.4 I'm 37 years old. My question is seeing such poor response, would you go ahead with an egg retrieval or two? And given my low AMH, I feel like I'm running out of time and desperate, but I don't know if we should do egg retrievals with such low numbers. What can I do?" S8 Ep 533I am 37 and have endometriosis. What should I do to prepare for my FET? (Ask The Egg Whisperer)

Lindsay recently wrote in to Ask The Egg Whisperer. She is 37. Here's what she wrote, "I have a diagnosis of endometriosis. I have endometriomas on my ovaries, but have not been diagnosed surgically. I've been trying to conceive for two years. First on a natural program and now an IVF. I've had three egg retrievals, one canceled cycle because they didn't respond to medications and had only two follow-up calls and they resulted in two PGS euploid embryos total one day six and one day seven. I'm now preparing for my FET December 18. Do you have any suggestions to increase my chances of success. We want two children, and this is our only transfer. We are considering transferring both embryos, but haven't decided yet. These numbers are so precious to us since it took us so long to get here, COVID delays and all, and I would really appreciate your second opinion." Kate recently wrote in to Ask The Egg Whisperer a question. She is 32, and she has a lot of pain during ovulation. She has one failed round of IVF due to being a poor responder. Here's what she had to say, "Roughly every month, I am in horrible pain around ovulation. Sometimes it hurts so bad to even stand up. It's only on my left side. I also get extremely bloated and uncomfortable in the lower abdominal area. Neither my GYN or fertility specialist have an idea of why this is. Do you have any suggestions of what it can be? What tests could I have done? I had an HSG with suspected left fallopian tube blockage, and a sailing sonogram that showed a beautiful uterus. I'm 32. My AMH is 0.7. FSH is 7, and Estradiol is 64. I am starting IVF to get pregnant again more quickly due to my age and the length of time it usually takes me to get pregnant. I have had 2 miscarriages and 1 beautiful daughter. My question is related to timing of cycles and transfer. Is it worth focusing on completing more than 1 cycle (say up to 3) if I only get 1 normal embryo from my first cycle? And then, focus on transfers with what I have? I am worried about transfers taking time due to failed transfers and/or miscarriage and losing more ovarian reserve due to time lost. I started throwing Egg Freezing parties as a fun way for people to learn about fertility. There is humor. There is honest talk. Mostly, I throw them because I want every person to be empowered with knowledge about their fertility. It's really as simple as that. Egg Freezing parties are safe and surgery-free gatherings of women (and sometimes men) to learn more about the process of freezing your eggs (or sperm). Beyond that, we talk in general about fertility and how every woman (and man) can take a proactive role in preserving theirs. I started Egg Freezing parties for a simple reason. Older women were in my office struggling with fertility only to wish they had known more when they were younger. Had someone told them to get their fertility levels checked and to consider the option of preserving their eggs they most certainly would have. I don't want to hear years from now that women wished they'd known more about their fertility. This is what drives me to throw parties to make sure everyone knows they have options! Dr. Eric Forman is the medical and lab director at Columbia University Fertility Center where he oversees the medical practice as well as IVF embryology and andrology labs, while also actively seeing patients. He set the standard for eSETs (elective single embryo transfers) across the globe. And, he revolutionized how we freeze eggs, with his research proving that the rapid vitrification technology does not increase the risk of genetic abnormalities in embryos. PGT-A is a hot topic in fertility medicine and I'm so happy that Dr. Eric Forman joined me to talk to us about a recent Chinese study published in The New England Journal of Medicine. Takeaways: - this study is getting a lot of attention because of the prestigious journal it's published in - the study design isn't how we do things in the real world: we don't just biopsy three embryos - the average age was 29 of the patients and mean egg number 20 and we know from previous studies that PGT-A as a selection tool may help women over age 37 more than younger women - the cumulative live birth rate was higher in the PGT arm - the study provided reassurances about PGT-A: the biopsy is safe, the group who did it had a higher implantation rate, similar cumulative live birth rate with fewer transfers, and fewer miscarriages. There were more congenital abnormalities in the non-biopsy group. A big thank you to Dr. Forman for breaking this all down for us and for sharing how PGT-A can improve the live birth rate on the first transfer and make elective single embryo transfer the standard of care across ages. S8 Ep 526I'm 41 and a carrier for a genetic disorder. Should I do another round of IVF now, or wait until after a healthy pregnancy with our one good embryo? (Ask the Egg Whisperer)

Recently, Elizabeth shared a question on AskTheEggWhisperer.com. She is 41, and she and her husband are carriers of a recessive disorder that results in severe physical and mental disability. Here's what she says, "I'm 41 AMH 0.95. We had a little girl naturally when I was. 40, but she didn't survive. We have a 25% chance of recurrence to screen for the disorder. We did our first round of IVF and we got results back today out of seven eggs, five fertilized, three made it to blast. We have one normal that is not affected. There was another normal blast, but it was affected, and there was one with abnormal chromosomes. We're so happy to have a good one to use. We might want two children though. I have two more rounds covered by insurance, but they can't be used for embryo banking. Should I wait until after a healthy pregnancy with our good one to do IVF again? The first egg retrieval, three fertilized after seven retrieved, one blast tested abnormally. The second retrieval, we added an Omnitrope and dexamethazone during stimulation. ICSI and calcium solution to the eggs. Five retrieved, one fertilize, one blast, and it was normal. I eat very well, I take supplements, I meditate, and I am doing acupuncture. Any ideas on why have such poor fertilization? Do you think I have poor quality eggs, or is that the protocol that's hurting them? Should we be testing my husband's sperm further? Recently, Kristin wrote in to Ask The Egg Whisperer. She's 39, and she's undergoing her first IVF cycle. Here's what she asked, "I am currently preparing my uterine lining for transfer. After two weeks of taking Estrace two pills, three times daily, my uterine lining only measured 4.8 millimeters. My doctor increased my estrogen by prescribing patches. (Two patches that are replaced every other day). After one week of using the patches and pills, my lining increased to 5.5 millimeters. My doctor wants me to continue on this protocol for another week and see what kind of progress we make. What causes it then uterine lining. What thickness do you recommend for transfer? What can I do besides medication to increase my lining thickness?" This is not an uncommon problem, and I share my thoughts on it in this episode.
