Dr. Geoffrey Sher is back, and we are talking about Intrauterine Insemination, or IUI. In 1982, Dr. Sher and his colleagues were the first to wash sperm, and then put it through a centrifuge process. After an incubation period, they used a catheter to inject the sperm into the uterine cavity of a patient. He's a true pioneer, and this was the first time IUI had been done!. She is 31, and here's what she had to say, "My husband and I found out we were pregnant and after several ultrasounds and blood work, I was told I had a partial molar pregnancy. My doctor told me that it was simply just really bad luck. I am waiting to get the okay to try again and really want to set myself up for success. I was taking a prenatal before and while we were trying, and now I'm making lifestyle changes. It's very difficult experiencing a molar pregnancy because I don't know anyone else who has had one. The first question I read is from Sylvia, and here's the question she sent in, "We have been trying for nearly two years with no luck. I have been diagnosed with adenomyosis and was wondering if this could be affecting my fertility as there doesn't seem to be much information out there. Most info is coupled with endometriosis but I don't have that. I was just wondering if you had any insight on this?" Here's what I share with Sylvia about adenomyosis: adenomyosis is a fertility threatening condition. It can decrease implantation rates, increase risk of miscarriage, and increase the risk of pregnancy complications. It's important to get an accurate diagnosis and a treatment plan as soon as possible when you have this. There are ways to treat adenomyosis and your fertility specialist or OBGYN can help. Sarah wrote in to Ask The Egg Whisperer, and she sent the following question, "I have done 12 embryo transfers over the past 2 years (failed due to several reasons). During the transfers, my lining always thickened nicely (minimum 9), I have never had a problem with thin lining. All my cycles were medicated and not natural FETs. I have patients that come to me from all over the world. There's one question I always ask when reviewing their IVF cycle history. What was your embryo quality? I hear time and time again from patients that they don't know it because their doctor told them embryo quality doesn't matter with IVF. That's simply not true! Just like diamonds in a jewelry store. Quality matters. Just like there is a grading system for diamonds, there is also a grading system for embryos. It's important for you to know that this exists and to ask for your embryo grade. One of the most common questions I get asked is "Why didn't my IUI work?" It's a great question because there are many factors that come into play with intrauterine insemination. If you're my patient, we'd start by checking the Fallopian tubes, uterus, and sperm quality first.⠀ But let's say you've done the #TUSHYcheck, the tubes are clear and everything else looks good. So why doesn't IUI work? ⠀ The answer I share with my patients is: because you're human. The topic was introduced in a recent Ask The Egg Whisperer session by Nicky who asked, "Are there any supplements or treatments you can recommend for helping Day 3 embryos reach blastocyst?" And to answer this, I want to share about a mnemonic, called AGE. Embryos, or blastocysts have everything to do with your Age Genetics Environment Of course, you can't control how old you are, nor can you control the genetics you were born with, or that your partner was born with. However, if you have the ability to freeze eggs, sperm, or embryos, then you can preserve your fertility by doing so. And, you can control your environment by sleeping well, exercising, most days of the week, having a diet rich in antioxidants and following an anti-inflammatory type diet, avoiding the environmental toxins. On the podcast today, I'm answering a question from Helena from a recent Ask The Egg Whisperer episode. Here's what she asked, "After stopping the birth control pill, is there any reason to wait a few months before trying to get pregnant? Are the first eggs released after stopping the pill potentially weaker, riskier or prone to miscarriage?" There are a lot of myths out there, and this is one of them. A lot of people are told that once you stop birth control pills, you need to wait a certain period of time to get pregnant. This is not true. There's a reason why you have to take a birth control pill every single day. And the reason is if you don't take it every day, you'll ovulate through it. Some studies have shown that you're actually the most fertile right after you stopped the birth control pills. So to answer your question, if you are taking the pill, and stop it - it's safe to try and get pregnant right away. On the podcast today, I'm answering a question from Vicky who wrote in to Ask The Egg Whisperer. She wants to know "How much caffeine is ok for the 90 days pre IVF, during stim and after embryo transfer?" I get a lot of questions about how much caffeine is OK during IVF, and when patients are pregnant. It's my opinion that in the three months before IVF, drinking coffee is not going to hurt your fertility. However, this is a situation where it's best to go by the saying "everything in moderation." When it comes to caffeine during stim, I actually sometimes recommend that my patients consider drinking coffee. Why? The meds can make you feel super sleepy when that estrogen fog rolls in. Those same medications can also make you constipated, and a little bit of caffeine can certainly help with the problems of constipation too. After embryo transfer, same thing: those meds just make you feel so sleepy, and sometimes you need a little bit of a pick-me-up. However, once you're pregnant, coffee can make your heart race. It might make it harder for you to sleep, and cause insomnia, and that can be super irritating. So be careful if you're someone who feels a little anxious. Here's what she want's to know, "I am currently TTC and taking a prenatal vitamin that includes choline (One A Day prenatal advanced). I have listened to your show and you often recommend CoQ10. Would it be beneficial for me to add C0Q10? Is it safe to take with prenatal vitamins?" Heidi recently wrote in to Ask The Egg Whisperer, and here is her question, "I had healthy baby boy end, no issues during pregnancy. He passed away two days after birth (birth asphyxia, umbilical cord around neck, too long labor, infection, placental abruption and C-section was done too late). I got pregnant naturally after TTC 10 months. I have also 19 year old son. I have regular cycle 28days, I'm healthy, no PCOS, miscarriages or other issues. I'm using prenatal vitamins, omega3, coQ10, Maca and Vitex, d-vitamin, iron, folic acid. Physically I've been healing from surgery really good, mentally it's been hard. I want to test my estrogen, progesterone, FSH, LH and iron levels but is it too early to test these because I was just pregnant? How fast hormone levels are getting back to normal after labor? My period came back 4 weeks after labor, they are normal and I ovulated already once." On today's podcast episode, I'm talking about preparing for embryo transfer. I find that it's helpful to think about embryo transfer the same way you might think about having a very special guest over for dinner. What are you going to do if you have a very special person over? You're going to make sure everything is absolutely perfect. When it comes to preparing for an embryo transfer, you want to take the same approach. Your metaphorical home in the house analogy is your uterus. The devastation of doing more tests and finding out that something might be wrong that you have to fix is kind of a pain, but the devastation of going through a transfer that doesn't work is also pretty darn awful. Because I work so hard to help families, if I can do anything to prevent someone from not experiencing the devastation of a negative embryo transfer, I'm going to do everything possible to clean up that house and make sure it's perfect. What I recommend is that people do 3 tests before embryo transfer to make sure your uterus is absolutely perfect prior to your embryo transfer.They are: Hysteroscopy Saline Infusion, Sonogram, and Hysterosalpingogram. Last year in the United States a baby was born every 8 seconds. That's roughly 3.8 Million babies born last year alone! You don't have to be a fertility doctor to know what that means. Millions of people just like you were trying to conceive. Research tells us that most couples conceive within six cycles of timed intercourse, but many others (roughly 10% of couples) are struggling with fertility. Here's the funny thing about what I do. I want to teach you how NOT to be my patient. Of course, there are exceptions to that statement. But in general, I'd much prefer to share with you what I've learned about preparing for pregnancy in hopes that it makes your journey to parenthood easier. She would like advice on what she can do for success in her next round of IVF. Here's what she wrote, "I'm 41 years old. I did four rounds of IVF. The first cycle was when I was 40. Did you know that fertility rates have dropped 50% in the last 5 decades, and more and more people are requiring help to grow their family? Some are calling it "Spermageddon." I'm not one to run with fantastical headlines, but the truth is that we are facing an infertility epidemic. And, this is all heartbreaking for the people dealing with infertility, and for us as a society. Here's what she wrote, "I've been trying for our first baby for the past two years. I had a miscarriage this past summer, and I'm now seeing a fertility doc to see if IVF is the right choice for me. The blood work came out pretty much fine. Tubes are open, and my uterus looks good. So I'm not sure why we're not able to conceive naturally. My husband and I have also gone through genetic testing. I carry a gene for thalassemia. Do you suggest I go for IVF or try other methods first? And what all questions should I be asking my REI before deciding on IVF?" On a recent episode of Ask The Egg Whisperer, Ana Lucía shared a great question. Here's what she wrote, "Thank you Dr Aimee for your show! I just had a miscarriage. I am 42, my progesterone and estradiol levels are very low (8 and 147). I've been taking estradiol 6mg and Crinone daily. Why are they low and how can I improve them? Thank you very much for your guidance." In answering this question, it's important to understand how you're taking progesterone (Crinone) and estradiol. Let's break this down a little: for progesterone, the level in your blood is based on the mode of administration. When I have a patient on Crinone (vaginal form), and their progesterone level is 8 - that's because you're placing it vaginally. It has to go through the walls of the vagina, get absorbed, and then go into your bloodstream. You could add more progesterone to your protocol to raise your progesterone, but I certainly don't want you to worry that your progesterone levels were too low for a healthy implantation to occur if this is a level checked after a frozen embryo transfer. I like the level to be around 10 in a medicated FET cycle if using vaginal progesterone. These numbers sometimes make us worry unnecessarily (especially when we go to Google for answers). You might see things that make you really upset, like information saying that your progesterone should be over 20. And, that's sound advice if you're using an injectable form of progesterone. But, with Crione, which is a vaginal form, you're just fine. Talk to your doctor and share your concerns for additional reassurance.
