S7 Ep 486I have had multiple miscarriages. What supplements and tests should I consider? (Ask The Egg Whisperer)

In this episode, Dr. Aimee is joined by Dr. John Norian. Recently, Rebecca from New Zealand sent a question to Ask The Egg Whisperer. Here's her question, "I'm 31 and have two children. My FSH is seven. We started trying to conceive in late in 2020, and got pregnant right away. Sadly, it ended in a six week spontaneous miscarriage. I got pregnant again after the miscarriage and this pregnancy resulted in also a miscarriage at 12 weeks of pregnancy. I'm RH negative, and had the RhoGAM after both losses. I'm booked for fertility appointment about a month. I've made lots of lifestyle changes, avoiding BP, and now eating a plant-based diet. What would you recommend as next steps for me as far as supplements and are there any other tests that you would recommend?" Tune in to hear the answer to this question, and others. S7 Ep 485How many vials of sperm do you need for IVF cycle? (Ask The Egg Whisperer)

In this episode, Dr. Aimee is joined by Dr. John Norian. Recently, Emma wrote in to Ask The Egg Whisperer a question from Wisconsin. Here's what she had to say, "I'm 33. My husband had treatment for a brain tumor four years ago, and his sperm did not come back. Luckily before his treatment, he froze 16 vials of unwashed sperm in the UK (England), where we're from. We had it shipped to Wisconsin, where we live now. We've tried IUI and have not been successful. We're now doing IVF with only five vials left. My question is how many vials do you need for one IVF? How will we know if this sperm is sparkling without losing a vial for testing? My AMH is 1.6. My FSH is 7.3. We want two kids. I wish we had better information about IUI success and had not used so many of our frozen vials on IUI. Now we have only five vials left. How do we make the best chance of achieving our family dreams?" Tune in to hear the answer to this question, and others. S7 Ep 484Everything You Need to Know about PCOS with guest Dr. Lucky Sekhon

PCOS is one of the most common and most misunderstood conditions that we treat as reproductive endocrinologists, and this week I'm delighted to be joined on the podcast by Dr. Lucky Sekhon of RMA of New York to talk all about it. PCOS stands for polycystic ovary syndrome, and that name can lead to some confusion. A lot of people think that they probably have cysts in their ovaries. The truth is that all women have multiple cysts (which are really small follicles that house eggs) in their ovaries. Those with PCOS have follicles that look like what we call a "string of pearls" on the periphery of their ovaries, and often have irregular (or no) cycles. Nidda recently sent a question in to The Egg Whisperer. She is 32, and has endometriosis and has had recurrent miscarriage. Here's what she asks, "I'm going to read what she shared. Hi, I have endometriosis and recurrent miscarriages. I've had five. My endometriosis was diagnosed in 2017 with an open surgery. I tried naturally for one year. Both tubes were shown open, but no pregnancy. I started IVF in 2018, six embryos, one day three transfer, and the rest day five blasts were frozen. I got pregnant four times. The heartbeat was seen in two of them. And my second IVF in 2021 with one, two day frozen transfer and one, three day frozen. I became pregnant for the fifth time, but miscarried pathology data for our last pregnancy showed normal fetus no data from the placenta normal sperm analysis, DNA frag laparoscopy in 2020 endo removed uterus hysteroscopy done in normal. I've tried prednisone your lipids heparin on my last pregnancy. And PGT is just not an option due to regular, recurrent miscarriage. S7 Ep 4745 Novel PCOS Treatments

PCOS stands for polycystic ovary syndrome. Many people interpret it to mean you have big cysts on your ovaries, but that's not true at all. When I hear PCOS I think of it as a call to, &ldquoPlease Confirm this Ovary Syndrome". So much is misunderstood about PCOS! Women are often told, "oh you don't need to worry about it until you're ready to have babies." That could not be further from the truth. S7 Ep 472Statistically, how many day two or day three embryos would a 43 year old need to bank to have one live birth? (Ask The Egg Whisperer)

Recently, a 43 year old listener sent in a question about how many embryos (statistically) she would need to freeze to have a significant chance at a live birth. Here's her question, "Statistically, how many embryos (day two or day three) would a 43 year old woman have to freeze to have one live birth? I know that everyone is different, but I'm trying to get a sense of math and attrition rate. I am 43, almost 44. AMH is 0.4. I have DOR and Hashimoto's, and I'm being treated by an endocrinologist and functional doctor because of my DOR. My RE has recommended that we freeze day two embryos. S7 Ep 471I'm worried about OHSS (Ovarian Hyperstimulation Syndrome). Do you think an Estradiol level of 3,800 right before retrieval is too high? (Ask The Egg Whisperer)

Joyce sent in a great question to Ask The Egg Whisperer. Here's what she asked, "Do you think an Estradiol level of 3,800 right before retrieval is too high? Will it affect egg quality?" My short answer: First, an Estradiol level of 3,800 doesn't worry me at all. I would make sure to ask your doctor, "what are my risks of having moderate to severe OHSS and what can I do to not have that?" I would also ask them for an emergency number in case you are feeling crummy and need some help and assistance. S7 Ep 470I have unexplained infertility. How many rounds of IUI should I do before moving to IVF? (Ask The Egg Whisperer)

Recently, Brittany sent a question in to Ask The Egg Whisperer. She has a diagnosis of unexplained infertility, and wants to know how many rounds of IUI she should do before moving to IVF. Here's her question: "Hi Dr. Aimee! I am a 33 year old woman. My husband is the same age and we have been trying for about two years. I want multiple children. I had a case of PID. My IUD got infected when I was 20. The only diagnosis they have for me (I've been to two different clinics) is unexplained infertility. They say my tubes might be scarred from PID, but the saline histogram showed no abnormalities and perfect openings. Maybe endometriosis runs in my family, maybe it's PCOS. There are no definites on anything. So we've just started our first IUI with Femera, so I'm trying to stay positive. How many rounds should I do before moving on to IVF? And what can you suggest or recommend for my situation? I'm supplementing and exercising, walking with lightweights, doing yoga and trying to remain light and positive, please help." I'm glad that Brittany shared all of this background information. While she's been given this unexplained sort of diagnosis, it feels like there likely is an explanation. Whenever I have a young patient like her (and it sounds like everything else is great: No fibroids, no polyps, good sperm), the tubes in my mind are blocked until proven otherwise. She should talk to her doctor, and he may want to ask for a hysterosalpingogram or a laparoscopy, which give more information than a saline histogram. A couple of other things to consider: Since Brittany had an infection with an IUD, I worry that she could also get an infection from an IUI, which stands for intrauterine insemination. She might consider doing another test to look at her fallopian tubes. I would say because of the infection that she had from the IUD, it's possible that the fallopian tubes might be blocked and IVF is the way to go to build her family. S7 Ep 465What are some of the reasons to get no embryos after a round of IVF? (Ask the Egg Whisperer)

In this episode, a listener asks about the reasons she did not get any embryos in her latest round of IVF. She is 37, and had 25 eggs retrieved. Here's what she has to say, "What are some of the reasons to get no embryos after a round of IVF? I'm 33 years old and me to 3.7 had 25 eggs retrieved, 10 mature fertilized, but none made it to day five. I'm heartbroken and confused." I agree that this is heart breaking, and confusing. I would recommend speaking to your IVF doctor, and ask why they think why things haven't worked until now. Get all of the information you can about your fertility diagnosis. IVF can give you a lot of great information, so ask your doctor very specifically for all your official reports. You want to know about the information from on the day of egg retrieval. Ask your doctor if there were there any physical characteristics that they saw in your eggs that would lead them to believe that this could possibly happen. Ask if there was there something about the eggs that makes them think that this could be an quality issue. And then also ask, if there something about the sperm that makes them think this could be a sperm quality issue, and lastly ask about the genetic screening that you've done and get those results. S7 Ep 465I was about to start IVF, and now my period is 10 days late. Could it be from stress? (Ask the Egg Whisperer)

In this episode, Jess writes in to Ask the Egg Whisperer why her period is late. She's getting ready to do IVF and worries that stress might be impacting her cycle. Here's what she asks, "Hi, Dr. Aimee. I've had my baseline scan. It was fine, but my period is now 10 days late. Why? I've taken a pregnancy test and it is negative. Have I scared my period away worrying about starting IVF? How can I get my period to come? It's always so regular that I'm really frustrated. I want to get started. Please help." My short answer for Jess is that if she were my patient, I would make sure that we check baseline levels of Estradiol, Progesterone, FSH, and sometimes HCG as well. We want to see what's going on with your body, and look to see if there is a cyst. There is no need to sit at home and worry. Be sure and check with your doctor. S7 Ep 461I'm 26, and we've been trying for over two years. What should we do next? (Ask the Egg Whisperer)

Recently, Brianna wrote in to Ask The Egg Whisperer a question. On this episode, Dr. Aimee is joined by Dr. Gayane Ambartsumyan of Reproductive Partners Medical Group. So, Brianna is getting answers from two Egg Whisperers! Here's what Brianna asked, "I'm a medical student, third year, in Chicago. I am 26, and my husband is 31. We stopped using contraception June of 2019. We were not truly trying, but hoping at the same time. I started getting worried in June of 2020 and began tracking BBT, cervical mucus, and LH strips. That's a whole lot of tracking day three! My FSH is 10, AFC is 19, AMH is 2.94 and day 21 progesterone is 10.8. We are starting to get really discouraged. The last four months have been really tough with the time and mental energy is taking a toll on already busy lives. My husband is also a doctor. We are so excited to become parents. And aren't sure if we should explore IVF at this time? What is the best route for our hopeful family? Thank you for being so wonderful and helping educate, uplift, and empower so many women." Our short answer is that Brianna could look at ways to reduce stress. She could also consider completing the TUSHY Method set of tests: a Fallopian tube test, a sperm count, and a genetic or carrier screen. If you'd like to hear more about what both doctors have to say about this question, tune in to this episode. S7 Ep 460I'm 36. I've had three miscarriages all around eight weeks of pregnancy. Should I consider IVF? (Ask the Egg Whisperer)

Recently, Rose from Iran sent a question in to Ask The Egg Whisperer. Here's what her message said, "I'm 36 years old. I've had three miscarriages all at around eight weeks of pregnancy. I've asked for PGD chromosome X testing or PGS testing, because doctors couldn't give that to me in Iran. Do you think I might be a good candidate for IVF?" My short answer is that yes, Rose might want to consider IVF and additional genetic testing. The genetic integrity will tell us that embryo has the ability to turn into a viable pregnancy. And then, the quality of the embryo tells us the implantation potential of that embryo. You can tune in to the episode for my full explanation, and my answers to other listener questions. Today on the podcast, I'm delighted to be speaking to Dr. Lucky Sekhon, or as many of her patients call her, Dr. Lucky. S7 Ep 456Is it possible that the test incorrectly recorded my AMH and FSH levels? (Ask the Egg Whisperer)

Priscilla recently wrote in to Ask The Egg Whisperer a question. She had her AMH and FSH levels tested, and she is wondering if they are incorrect. Here's what she had to say, "Back in August, I did a test and my AMH came back at 0.9. My FSH was 15. I'm 34 years old. I have super regular cycles. Is there a chance the test is wrong? And do I have a chance of conceiving naturally?" My short answer is this: There is a chance that the there's some lab variation. I'm not going to necessarily say that it's "wrong," but to know if it was truly accurate for you, I would recommend that you repeat it. I encourage people to track and trend their levels because one test gives you a snapshot of what is happening at just one time. So don't just rely on one snapshot. I think that's the mistake people make by not understanding their full diagnosis. Without checking your labs more than once, just one FSA, AMH, and Estradiol does not tell you the whole story. Sometimes, people rely one set of results, when the reality is if you checked your hormone levels one more time, you might have picked something up that you would not have picked up if you just relied on that first level. Be sure and check out my TUSHY Method for more info on how to get a diagnosis. To hear the rest of my answer, and the answer to other listener questions, tune in to today's episode. S7 Ep 455Why do follicle counts drop from one month to the next? (Ask The Egg Whisperer)

Recently, a listener wrote in and asked why follicle counts can vary from month to month, and specifically why they drop some months. And, it's a great question! For now, we have to check estradiol, AMH, and FSH and have patients come in for a follicle count so we can determine if it's a good month to move forward with a cycle. S7 Ep 451Is working night shifts bad when trying to get pregnant via embryo transfer? (Ask The Egg Whisperer)

Recently, Suzanne wrote in to ask if working a night shift may negatively impact her chances of becoming pregnant via embryo transfer. Here's what she has to say, "I'm wondering if you think that it's very bad to work night shifts when trying to get pregnant with an embryo transfer. I also work some days and evenings, but mostly nights. And I've heard different things about it. If it's good or not, what do you think about it? Can it really disturb implantation and cause miscarriage?" And my answer for Suzanne is that it depends on many things, including how long you've been trying and your age. Recently, Yamilka sent a question in to "Ask The Egg Whisperer," and here's what she said: "I am from Florida, 36 years old. I'd like to start the fertility process on my own. However, I'm overweight have type two diabetes and sleep apnea. I was diagnosed with PCOS. For the last couple of years, my doctors weren't clear if I have it or not. I have regular cycles that lasts around 32 days and I want to do IUI or IVF soon. What are the possibilities of me receiving treatment successfully?" My short answer is that Yamilka's chances of having a healthy pregnancy at age 36 are really, really good. While she does have PCOS, I would want to see what we can do to help you be even healthier. S7 Ep 447What should my progesterone level be before and after an frozen embryo transfer? (Ask The Egg Whisperer)

Recently, a listener sent a question in to Ask the Egg Whisperer. Here's what she asked, "Hi, Dr. Aimee. I recently had an FET(Frozen Embryo Transfer) that ended in a miscarriage at six and a half weeks with a high grade, PGS tested embryo. I did the era test. We did the transfer on the right day. I was wondering what should my progesterone level be before and after the frozen embryo transfer?" My short answer is this: before the frozen embryo transfer, on the lining check day, you want it to be less than one. I like it to be around 0.5. On the transfer date, I don't check progesterone level, but if there's a concern that someone's having a reaction to the progesterone, I check it. I like it to be at least 20, if you're doing intramuscular progesterone. The next week, with a pregnancy test, I always check progesterone with my HCG levels. I like the progesterone level to be over 20, ideally around 30. Tune in to hear the full answer to this question and many others. S7 Ep 446Can the ERA test be done to figure out the best implantation window during a natural FET cycle? (Ask The Egg Whisperer)

Recently, a listener sent this question in to Ask The Egg Whisperer: "Hi Dr. Aimee! Thank you for all the work you're doing and for giving us the opportunity to ask questions. I listened to your last show on the ERA test. And I would like to know whether an ERA (Endometrial Receptivity Analysis) test can be done to figure out the best implantation window in the case of a natural FET (Frozen Embryo Transfer) cycle?" I always love answering these questions. Let's look at the basics of IVF before we talk about the ERA test. I like to think of it in three stages: 1. Egg and Sperm come together to make an embryo 2. Embryo transfer preparation 3. Embryo transfer The answer is: Yes, I do the ERA test in natural cycle transfers as well, because I want to get it right. I want to know exactly when a patient should do their trigger shot. I want to know exactly what time I should be doing that transfer, so that I never look back and say, "Oh, I wonder if the reason why this test is negative is because I didn't get the timing right."
