S8 Ep 505I am 44 and have type 2 diabetes. Should I consider IUI or IVF? (Ask The Egg Whisperer)
Recently, Kim sent in a question to Ask The Egg Whisperer. She is 44 and has type 2 diabetes. She wants to know if she should continue trying to get pregnant with her own eggs, or consider other options. Here's what she had to say, "I have had 2 normal pregnancies in the past, with the last one 10 years ago. I have a boy and a girl; my husband and I have tried to conceive for the past 4 years naturally without success. We went to a fertility clinic and have done all test possible everything came back good besides that I was diagnosed with type 2 diabetes with 11.4 A1C and reduced it to 7 in 3 months, my AMH level was 0.25. We have done an IUI cycle last month, and it was unsuccessful, and we are trying IUI for the second time with clomid and obedril shot as the last time. Are we doing the right thing or are we wasting time? I would really like to avoid IVF if possible, plus I would like to use my own eggs if possible. Thank you!" IVF is a process that involves creating an embryo in a lab. To help explain the process of creating those embryos, I'm joined today by embryologist, Salu Ribeiro. We're are talking about what happens in the lab, after eggs have been retrieved, what makes a "golden egg," and how fertilization works. We'll explain some of the terms you may hear when you're doing IVF: ICSI (which is Intracytoplasmic sperm injection) and PICSI (which is physiological ICSI, where sperm are first chosen for ICSI). S7 Ep 496What tests should I consider to determine if I have adenomyosis or endometriosis? (Ask The Egg Whisperer)
Cheryl sent in a great question to Ask The Egg Whisperer. She is 39. Here's what she asked, "Thank you for your show, Dr. Aimee. I had two IVF cycles, but my egg maturation rate is very low (54%, 10%). My AHM level is 0.67. My doctor did not find any signs of endometriosis, but some signs of adenomyosis. I saw there are four potential reasons for low maturation rates: 1) my age; 2) wrong IVF stimulation protocol; 3) endometriosis; 4) reproductive autoimmune failure. Are there any tests that I can do to figure this out?" S7 Ep 495I have chronic endometritis. How can I improve my chances for embryo implantation? (Ask The Egg Whisperer)
Recently, Eliza sent a question in to Ask The Egg Whisperer. Here's what she asked, "I have chronic endometritis, and I am finding very little on what the correct treatment process looks like. I'm already on a 2nd repeat course of antibiotics after 2nd biopsy still came back positive for CD 138 stromal cells. My MD is suggesting to go straight into a transfer after second round of antibiotics without an additional biopsy. I'm afraid I may have an antibiotic resistant form and am wondering if you ever perform endometrial scarring or D&C to treat it? Would you recommend to have a confirmation that the endometritis is gone before implanting? Also I am already on depot lupron 2nd month for endometriosis. Is it a concern that I will not get a period that would shed the bad cells somehow before a transfer? Would love your input on how you deal with resistant endometritis and implantation outcomes." Dr. Lessey has dedicated his research life to studying the lining of the uterus and more specifically, how to help embryos stick better. His research led to the discovery of the role of BCL-6 in the lining of the uterus as a diagnostic test patients can have done by their doctors to guide them toward the best protocol for transfer success. The test isn't for women just going through IVF either. Recently, Jess sent a question in to Ask The Egg Whisperer. Here is what she had to say, "I'm 41, have been trying for 7 years to have a baby. My husband is 39 and has tested fine, no issues. We've went through two IVF rounds in 2018. My husband and I have tried basically every fertility treatment and have not yet had a baby. I got pregnant out of the blue in 2017 but it ended up being a blighted embryo. I have uterine didelphys (double uterus) with a complete septum and two cervixes. Based on all the tests over the years, low ovarian reserve seems to be the main issue. I've never received a formal diagnosis. It's always been "unexplained". I also did an HSG and was told that I have blocked fallopian tubes. I recently experienced my second IVF failure. This second round, I decided to genetically test my embryos (2 euploid and 1 aneuploid/low mosaic). Now that I have 2 failed cycles, I am seeking your advice on what you think I should do differently this time around. I am thinking of requesting the following tests to start with: 1. An ERA and Receptivadx tests. 2. Immunology tests such as thrombophilia and killer cells test. 3. A natural or modified natural cycle. Thanks so much and I await your guidance." My husband had treatment for a brain tumor four years ago, and his sperm did not come back. Luckily before his treatment, he froze 16 vials of unwashed sperm in the UK (England), where we're from. We had it shipped to Wisconsin, where we live now. We've tried IUI and have not been successful. We're now doing IVF with only five vials left. My question is how many vials do you need for one IVF? How will we know if this sperm is sparkling without losing a vial for testing? My AMH is 1.6. My FSH is 7.3. We want two kids. I wish we had better information about IUI success and had not used so many of our frozen vials on IUI. Now we have only five vials left. Nidda recently sent a question in to The Egg Whisperer. She is 32, and has endometriosis and has had recurrent miscarriage. Here's what she asks, "I'm going to read what she shared. Hi, I have endometriosis and recurrent miscarriages. I've had five. My endometriosis was diagnosed in 2017 with an open surgery. I tried naturally for one year. Both tubes were shown open, but no pregnancy. I started IVF in 2018, six embryos, one day three transfer, and the rest day five blasts were frozen. I got pregnant four times. The heartbeat was seen in two of them. And my second IVF in 2021 with one, two day frozen transfer and one, three day frozen. I became pregnant for the fifth time, but miscarried pathology data for our last pregnancy showed normal fetus no data from the placenta normal sperm analysis, DNA frag laparoscopy in 2020 endo removed uterus hysteroscopy done in normal. I've tried prednisone your lipids heparin on my last pregnancy. And PGT is just not an option due to regular, recurrent miscarriage. Blood tests and chromosomal issues in both me and the sperm provider were tested and normal. We have one frozen embryo left. What options do we have?" I know that everyone is different, but I'm trying to get a sense of math and attrition rate. I am 43, almost 44. AMH is 0.4. I have DOR and Hashimoto's, and I'm being treated by an endocrinologist and functional doctor because of my DOR. My RE has recommended that we freeze day two embryos. My short answer: First, an Estradiol level of 3,800 doesn't worry me at all. I would make sure to ask your doctor, "what are my risks of having moderate to severe OHSS and what can I do to not have that?" I would also ask them for an emergency number in case you are feeling crummy and need some help and assistance. S7 Ep 470I have unexplained infertility. How many rounds of IUI should I do before moving to IVF? (Ask The Egg Whisperer)
Recently, Brittany sent a question in to Ask The Egg Whisperer. She has a diagnosis of unexplained infertility, and wants to know how many rounds of IUI she should do before moving to IVF. Here's her question: "Hi Dr. Aimee! I am a 33 year old woman. My husband is the same age and we have been trying for about two years. I want multiple children. I had a case of PID. My IUD got infected when I was 20. The only diagnosis they have for me (I've been to two different clinics) is unexplained infertility. They say my tubes might be scarred from PID, but the saline histogram showed no abnormalities and perfect openings. Maybe endometriosis runs in my family, maybe it's PCOS. There are no definites on anything. So we've just started our first IUI with Femera, so I'm trying to stay positive. How many rounds should I do before moving on to IVF? And what can you suggest or recommend for my situation? I'm supplementing and exercising, walking with lightweights, doing yoga and trying to remain light and positive, please help." I'm glad that Brittany shared all of this background information. While she's been given this unexplained sort of diagnosis, it feels like there likely is an explanation. Whenever I have a young patient like her (and it sounds like everything else is great: No fibroids, no polyps, good sperm), the tubes in my mind are blocked until proven otherwise. She should talk to her doctor, and he may want to ask for a hysterosalpingogram or a laparoscopy, which give more information than a saline histogram. A couple of other things to consider: Since Brittany had an infection with an IUD, I worry that she could also get an infection from an IUI, which stands for intrauterine insemination. She might consider doing another test to look at her fallopian tubes. I would say because of the infection that she had from the IUD, it's possible that the fallopian tubes might be blocked and IVF is the way to go to build her family. I'm 33 years old and me to 3.7 had 25 eggs retrieved, 10 mature fertilized, but none made it to day five. I'm heartbroken and confused." I agree that this is heart breaking, and confusing. I would recommend speaking to your IVF doctor, and ask why they think why things haven't worked until now. Get all of the information you can about your fertility diagnosis. IVF can give you a lot of great information, so ask your doctor very specifically for all your official reports. You want to know about the information from on the day of egg retrieval. Ask your doctor if there were there any physical characteristics that they saw in your eggs that would lead them to believe that this could possibly happen. Ask if there was there something about the eggs that makes them think that this could be an quality issue. And then also ask, if there something about the sperm that makes them think this could be a sperm quality issue, and lastly ask about the genetic screening that you've done and get those results. S7 Ep 465I was about to start IVF, and now my period is 10 days late. Could it be from stress? (Ask the Egg Whisperer)
In this episode, Jess writes in to Ask the Egg Whisperer why her period is late. She's getting ready to do IVF and worries that stress might be impacting her cycle. Here's what she asks, "Hi, Dr. Aimee. I've had my baseline scan. It was fine, but my period is now 10 days late. Why? I've taken a pregnancy test and it is negative. Have I scared my period away worrying about starting IVF? How can I get my period to come? It's always so regular that I'm really frustrated. I want to get started. Please help." My short answer for Jess is that if she were my patient, I would make sure that we check baseline levels of Estradiol, Progesterone, FSH, and sometimes HCG as well. We want to see what's going on with your body, and look to see if there is a cyst. There is no need to sit at home and worry. Be sure and check with your doctor. Recently, Brianna wrote in to Ask The Egg Whisperer a question. On this episode, Dr. Aimee is joined by Dr. Gayane Ambartsumyan of Reproductive Partners Medical Group. So, Brianna is getting answers from two Egg Whisperers! Here's what Brianna asked, "I'm a medical student, third year, in Chicago. I am 26, and my husband is 31. We stopped using contraception June of 2019. We were not truly trying, but hoping at the same time. I started getting worried in June of 2020 and began tracking BBT, cervical mucus, and LH strips. That's a whole lot of tracking day three! My FSH is 10, AFC is 19, AMH is 2.94 and day 21 progesterone is 10.8. We are starting to get really discouraged. The last four months have been really tough with the time and mental energy is taking a toll on already busy lives. My husband is also a doctor. We are so excited to become parents. And aren't sure if we should explore IVF at this time? What is the best route for our hopeful family? Thank you for being so wonderful and helping educate, uplift, and empower so many women." Our short answer is that Brianna could look at ways to reduce stress. She could also consider completing the TUSHY Method set of tests: a Fallopian tube test, a sperm count, and a genetic or carrier screen. Recently, Rose from Iran sent a question in to Ask The Egg Whisperer. Here's what her message said, "I'm 36 years old. I've had three miscarriages all at around eight weeks of pregnancy. I've asked for PGD chromosome X testing or PGS testing, because doctors couldn't give that to me in Iran. Do you think I might be a good candidate for IVF?" My short answer is that yes, Rose might want to consider IVF and additional genetic testing. The genetic integrity will tell us that embryo has the ability to turn into a viable pregnancy.
