S8 Ep 536Does having a bicornuate uterus affect fertility? (Ask The Egg Whisperer)
Today on the podcast, I'm sharing a recent Ask the Egg Whisperer episode. I love answering your questions, and Natalie sent in a great one about a bicornuate uterus. Here's what she wrote, "Does having a bicornuate uterus affect fertility? What can be done to make sure the pregnancy sticks and remains healthy so the baby is born full term?" Having a bicornuate uterus is very special. When I see someone who comes to me with a bicornuate uterus, I like to learn more about the patient's anatomy so we can create a plan and build a team to support the patient. First, make sure that you have an accurate diagnosis. You'll also want to get an understanding of your own unique bicornuate uterus' anatomy. Depending on your anatomy and diagnosis, the good news is that it is possible to deliver a healthy full-term baby. It's also true that a bicornuate uterus may cause pregnancy complications. I would recommend talking to your OB GYN to see what they think, and ask what their experiences with patients just like you. Ask for their approach on managing your bicornuate uterus. S8 Ep 535My mother and grandmother went through menopause at 42. I'm 37, should I freeze my eggs? (Ask The Egg Whisperer)
Megan wrote in to ask The Egg Whisperer about freezing her eggs. She's 37, and tragically lost her fiancé last year. Here's what she had to say, "Hi, Dr. Aimee. I got my eggs frozen at 36, after my fiancé passed away. I got 13 eggs. My AMH was 5.7. FSH was 5, and it's now a year later. And I'm still worried about my chances. I really want two kids someday. Should I freeze more eggs when I can? FYI, my mother and grandmother both went through menopause at 42. Thank you for your time and advice." S8 Ep 354I have a low AMH, and had a poor response to IVF. What can I do? (Ask The Egg Whisperer)
Recently, Kate sent a question to Ask The Egg Whisperer. She is 37, and has low AMH, and was recently deemed a poor responder during an IVF cycle. Here's what she had to say, "My question is about IVF with poor response and low AMH. I've had four failed IUIs, and two rounds of IVF. The second round gave us three eggs, one blast that was normal, but my PGS transfer my FET did not work. My AMH is 0.4 I'm 37 years old. My question is seeing such poor response, would you go ahead with an egg retrieval or two? And given my low AMH, I feel like I'm running out of time and desperate, but I don't know if we should do egg retrievals with such low numbers. What can I do?" S8 Ep 533I am 37 and have endometriosis. What should I do to prepare for my FET? (Ask The Egg Whisperer)
Lindsay recently wrote in to Ask The Egg Whisperer. She is 37. Here's what she wrote, "I have a diagnosis of endometriosis. I have endometriomas on my ovaries, but have not been diagnosed surgically. I've been trying to conceive for two years. First on a natural program and now an IVF. I've had three egg retrievals, one canceled cycle because they didn't respond to medications and had only two follow-up calls and they resulted in two PGS euploid embryos total one day six and one day seven. I'm now preparing for my FET December 18. Do you have any suggestions to increase my chances of success. We want two children, and this is our only transfer. We are considering transferring both embryos, but haven't decided yet. These numbers are so precious to us since it took us so long to get here, COVID delays and all, and I would really appreciate your second opinion." S8 Ep 532My partner had a rapid decrease in sperm. What tests or supplements should we consider? (Ask The Egg Whisperer)
Recently, Corey sent a question to Ask The Egg Whisperer. Here's what she had to say, "My partner's most recent semen analysis was not great at all. As a quick background, January, 2020. Twenty-five million, with fairly low motility. July 2020, 28 million with an even lower motility rate. In the August and October analysis, they couldn't find anything. He took pregnyl shots for three months before our last test, also taking a few supplements daily. Do you have any recommendations on supplements that might increase our chances or testing that we should be doing to see why this was such a huge drop in sperm?" S8 Ep 531I have horrible pain at ovulation, and a canceled IVF. What tests should I have done? (Ask The Egg Whisperer)
Kate recently wrote in to Ask The Egg Whisperer a question. She is 32, and she has a lot of pain during ovulation. She has one failed round of IVF due to being a poor responder. Here's what she had to say, "Roughly every month, I am in horrible pain around ovulation. Sometimes it hurts so bad to even stand up. It's only on my left side. I also get extremely bloated and uncomfortable in the lower abdominal area. Neither my GYN or fertility specialist have an idea of why this is. Do you have any suggestions of what it can be? What tests could I have done? I had an HSG with suspected left fallopian tube blockage, and a sailing sonogram that showed a beautiful uterus. I'm 32. My AMH is 0.7. FSH is 7, and Estradiol is 64. I have one canceled IVF due to poor response." Tune in to hear the answer to Kate's question. Do you have questions about IVF? Join Dr. Aimee for The IVF Class at The Egg Whisperer School. S8 Ep 530I'm 40, and starting IVF. How do I make the most of it? (Ask the Egg Whisperer)
Recently, Jamie wrote in to Ask The Egg Whisperer a question. Here's what she had to say, "Hi Dr. Aimee! I just turned 40 in October and had a natural pregnancy that ended in miscarriage in November. I am starting IVF to get pregnant again more quickly due to my age and the length of time it usually takes me to get pregnant. I have had 2 miscarriages and 1 beautiful daughter. My question is related to timing of cycles and transfer. Is it worth focusing on completing more than 1 cycle (say up to 3) if I only get 1 normal embryo from my first cycle? And then, focus on transfers with what I have? I am worried about transfers taking time due to failed transfers and/or miscarriage and losing more ovarian reserve due to time lost. I know that age 40 is the beginning of a steep decline in fertility and every month counts." Tune in to hear the answer to Jamie's question. Do you have questions about IVF? S8 Ep 529Why I Throw Egg Freezing Parties
I started throwing Egg Freezing parties as a fun way for people to learn about fertility. There is humor. There is honest talk. Mostly, I throw them because I want every person to be empowered with knowledge about their fertility. It's really as simple as that. Egg Freezing parties are safe and surgery-free gatherings of women (and sometimes men) to learn more about the process of freezing your eggs (or sperm). Beyond that, we talk in general about fertility and how every woman (and man) can take a proactive role in preserving theirs. I started Egg Freezing parties for a simple reason. Older women were in my office struggling with fertility only to wish they had known more when they were younger. Had someone told them to get their fertility levels checked and to consider the option of preserving their eggs they most certainly would have. I don't want to hear years from now that women wished they'd known more about their fertility. This is what drives me to throw parties to make sure everyone knows they have options! I'm an egg rescuer, egg whisperer and fertility myth buster! Do you have questions about Egg Freezing? Click here to join me for my Egg Freezing Class. S8 Ep 528PGT-A with guest Dr. Eric Forman
Dr. Eric Forman is the medical and lab director at Columbia University Fertility Center where he oversees the medical practice as well as IVF embryology and andrology labs, while also actively seeing patients. He set the standard for eSETs (elective single embryo transfers) across the globe. And, he revolutionized how we freeze eggs, with his research proving that the rapid vitrification technology does not increase the risk of genetic abnormalities in embryos. Dr. Forman is joining me today on the Egg Whisperer Show podcast to discuss a hot topic in fertility medicine: PGT-A. We are going to discuss preimplantation genetic testing (PGT), the science behind this testing, and a recent paper, published in the New England Journal of Medicine (Live Birth with or Without Preimplantation Genetic Testing for Aneuploidy). S8 Ep 527Fertility Expert Series: Eric Forman on PGT-A
PGT-A is a hot topic in fertility medicine and I'm so happy that Dr. Eric Forman joined me to talk to us about a recent Chinese study published in The New England Journal of Medicine. Takeaways: - this study is getting a lot of attention because of the prestigious journal it's published in - the study design isn't how we do things in the real world: we don't just biopsy three embryos - the average age was 29 of the patients and mean egg number 20 and we know from previous studies that PGT-A as a selection tool may help women over age 37 more than younger women - the cumulative live birth rate was higher in the PGT arm - the study provided reassurances about PGT-A: the biopsy is safe, the group who did it had a higher implantation rate, similar cumulative live birth rate with fewer transfers, and fewer miscarriages. There were more congenital abnormalities in the non-biopsy group. A big thank you to Dr. Forman for breaking this all down for us and for sharing how PGT-A can improve the live birth rate on the first transfer and make elective single embryo transfer the standard of care across ages. S8 Ep 526I'm 41 and a carrier for a genetic disorder. Should I do another round of IVF now, or wait until after a healthy pregnancy with our one good embryo? (Ask the Egg Whisperer)
Recently, Elizabeth shared a question on AskTheEggWhisperer.com. She is 41, and she and her husband are carriers of a recessive disorder that results in severe physical and mental disability. Here's what she says, "I'm 41 AMH 0.95. We had a little girl naturally when I was. 40, but she didn't survive. We have a 25% chance of recurrence to screen for the disorder. We did our first round of IVF and we got results back today out of seven eggs, five fertilized, three made it to blast. We have one normal that is not affected. There was another normal blast, but it was affected, and there was one with abnormal chromosomes. We're so happy to have a good one to use. We might want two children though. I have two more rounds covered by insurance, but they can't be used for embryo banking. Should I wait until after a healthy pregnancy with our good one to do IVF again? Or should we do another round now?" You can tune in to hear my answer, and listen to the answers to other listener questions. S8 Ep 525I have hypothalamic amenorrhea, and I've had two egg retrievals, with poor fertilization. What do you recommend? (Ask the Egg Whisperer)
Recently, Tracy shared a question on AskTheEggWhisperer.com. She is 29, and her husband is 39. Here's what she says, "I have hypothalamic amenorrhea. I've had two egg retrievals. The first egg retrieval, three fertilized after seven retrieved, one blast tested abnormally. The second retrieval, we added an Omnitrope and dexamethazone during stimulation. ICSI and calcium solution to the eggs. Five retrieved, one fertilize, one blast, and it was normal. I eat very well, I take supplements, I meditate, and I am doing acupuncture. Any ideas on why have such poor fertilization? Do you think I have poor quality eggs, or is that the protocol that's hurting them? Should we be testing my husband's sperm further? What would you do for my next cycle? Thank you for being such a guiding light." You can tune in to hear my answer, and listen to the answers to other listener questions. S8 Ep 522The Truth About Fertility Drugs
Most fertility treatment hinges on the strategic use of drugs to help the body be as fertile as possible. My goal is to give each of my patients a personalized fertility plan with custom calendars of what to do, which drug to take, and when. Today on the podcast, I'm walking through fertility calendars (keeping things confidential, of course) and talking about the drugs I commonly use to give my patients the best chance at fertility success. S8 Ep 521I have silent endometriosis. What do I need to do to have a successful IVF cycle? (Ask The Egg Whisperer)
Recently, Rita sent a question in to Ask the Egg Whisperer. She is 30 and lives in Boston. Here's what she has to say, "I recently found out I have silent endometriosis, and had surgery for it. I'm now experiencing worse and more aggressive endometriosis. I've switched fertility doctors to one who specializes in excision surgery and fertility, and his recommendations was to do one cycle of fast-track IVF and bank as many embryos as possible. What is fast track IVF, opposed to regular IVF? He did not put me on an "endo friendly" protocol. Do you get fewer eggs with an endo friendly protocol? My dream is to be a mom of five. My AMH is 4, FSA is 7. I've been taking CoQ10, 0 fish oil, vitamin D, NAC, and I just started melatonin. Is there anything else you can recommend?" S8 Ep 520I have been deemed a poor responder. Should I change doctors, or advocate for a different protocol? (Ask The Egg Whisperer)
Recently, Kelly sent a question in to Ask the Egg Whisperer. She is 31 and lives in California. Here's what she has to say, "I'm in the middle of an IVF cycle and I was deemed a poor responder. My AMH is 3.1. My follicle count was 27 at baseline. I only had nine follicles that were responding on day eight. I was told is due to my BMI, but I feel like the protocol just wasn't aggressive enough. I'm thinking of canceling and advocating for a new protocol. What would you suggest for me? I want three to four children and our diagnosis is male factor infertility. My husband is 38, and has 2% morphology. I have one blocked tube and was told it's scar tissue, but it's not in hydrosalpinx. I love the show and I have learned so much." S8 Ep 516We have 3 PGT normal embryos. Should we do a medicated FET, or a more natural FET? (Ask The Egg Whisperer)
Recently, Piper sent in a question to Ask The Egg Whisperer. Here's what she asked, "My wife and I have learned so much. I've had two failed frozen embryo transfers, reciprocal IVF with my wife's embryos. They were poor quality day three embryos. And my wife's latest retrieval yielded three PGT normal embryos. Would you recommend a medicated FET or more of a natural FET with no birth control pills, ovulation trigger shot, and progesterone supplementation only if necessary? Why? What levels do you like a progesterone estrogen to be on the day of the FET? We are so excited and we feel like we actually have a shot with, with these gorgeous new embryos. Here's what she asked, "In terms of adding HGH to a protocol, does it add any benefit to do it during stims, or is it better to prime with it before your IVF cycle starts? My doctor's having me take Aygestin five days after ovulation for 10 days, then we'll schedule my baseline. Can I try naturally this month? I hate wasting a month and possibly a good egg." There are lots of ways of doing HGH priming, and I am sharing what I do with my patients as an answer to Jessica's question. You can tune in to hear the answer, along with other listener questions. S8 Ep 511I am 38, and on my fifth IVF cycle. I have 2 PGS normal embryos. What should I consider before transfer? (Ask The Egg Whisperer)
Recently, Amy sent a question in to Ask The Egg Whisperer. Here is her question, "We just finished our fifth IVF cycle and we finally have two PGS normal embryos. I am in shock. We're preparing for a December transfer. I had two transfers in the past. The first one resulted in my son in 2017. And unfortunately the second one resulted in a miscarriage at eight weeks. My new fertility doctor wants to stick with the same protocol, since both of my two transfers were technically successful. I just turned 38 and I do have mild endometriosis. I had a surgery in 2013 to confirm that, so this makes me nervous. Should I continue with the same protocol? Is there anything else I should do before transfer?" S8 Ep 510What causes it thin uterine lining, and what thickness do you recommend for embryo transfer? (Ask The Egg Whisperer)
Recently, Kristin wrote in to Ask The Egg Whisperer. She's 39, and she's undergoing her first IVF cycle. Here's what she asked, "I am currently preparing my uterine lining for transfer. After two weeks of taking Estrace two pills, three times daily, my uterine lining only measured 4.8 millimeters. My doctor increased my estrogen by prescribing patches. (Two patches that are replaced every other day). After one week of using the patches and pills, my lining increased to 5.5 millimeters. My doctor wants me to continue on this protocol for another week and see what kind of progress we make. What causes it then uterine lining. What thickness do you recommend for transfer? What can I do besides medication to increase my lining thickness?" This is not an uncommon problem, and I share my thoughts on it in this episode. The last year has been especially challenging for those in fertility treatment. This episode is all about how to care for yourself, and includes loving tips on how family and friends can support someone in fertility treatment, too. Self-care is anything but selfish. Recently, Kim sent in a question to Ask The Egg Whisperer. She is 44 and has type 2 diabetes. She wants to know if she should continue trying to get pregnant with her own eggs, or consider other options. Here's what she had to say, "I have had 2 normal pregnancies in the past, with the last one 10 years ago. I have a boy and a girl; my husband and I have tried to conceive for the past 4 years naturally without success. We went to a fertility clinic and have done all test possible everything came back good besides that I was diagnosed with type 2 diabetes with 11.4 A1C and reduced it to 7 in 3 months, my AMH level was 0.25. We have done an IUI cycle last month, and it was unsuccessful, and we are trying IUI for the second time with clomid and obedril shot as the last time. Are we doing the right thing or are we wasting time? I would really like to avoid IVF if possible, plus I would like to use my own eggs if possible. Thank you!" S8 Ep 504Nuts and Bolts of IVF
IVF is a process that involves creating an embryo in a lab. To help explain the process of creating those embryos, I'm joined today by embryologist, Salu Ribeiro. We're are talking about what happens in the lab, after eggs have been retrieved, what makes a "golden egg," and how fertilization works. We'll explain some of the terms you may hear when you're doing IVF: ICSI (which is Intracytoplasmic sperm injection) and PICSI (which is physiological ICSI, where sperm are first chosen for ICSI). And, we talk about how embryos grow, and what an embryologist looks for in eggs, sperm, and embryos. I love talking to Salu about embryos, and this conversation is a lot of fun! S7 Ep 496What tests should I consider to determine if I have adenomyosis or endometriosis? (Ask The Egg Whisperer)
Cheryl sent in a great question to Ask The Egg Whisperer. She is 39. Here's what she asked, "Thank you for your show, Dr. Aimee. I had two IVF cycles, but my egg maturation rate is very low (54%, 10%). My AHM level is 0.67. My doctor did not find any signs of endometriosis, but some signs of adenomyosis. I saw there are four potential reasons for low maturation rates: 1) my age; 2) wrong IVF stimulation protocol; 3) endometriosis; 4) reproductive autoimmune failure. Are there any tests that I can do to figure this out?" S7 Ep 495I have chronic endometritis. How can I improve my chances for embryo implantation? (Ask The Egg Whisperer)
Recently, Eliza sent a question in to Ask The Egg Whisperer. Here's what she asked, "I have chronic endometritis, and I am finding very little on what the correct treatment process looks like. I'm already on a 2nd repeat course of antibiotics after 2nd biopsy still came back positive for CD 138 stromal cells. My MD is suggesting to go straight into a transfer after second round of antibiotics without an additional biopsy. I'm afraid I may have an antibiotic resistant form and am wondering if you ever perform endometrial scarring or D&C to treat it? Would you recommend to have a confirmation that the endometritis is gone before implanting? Also I am already on depot lupron 2nd month for endometriosis. Is it a concern that I will not get a period that would shed the bad cells somehow before a transfer? Would love your input on how you deal with resistant endometritis and implantation outcomes." S7 Ep 492ReceptivaDX Test and How it Can Help You with guest Dr. Bruce Lessey
I'm so excited to have Dr. Bruce Lessey of ReceptivaDX join me on the podcast. Dr. Lessey has dedicated his research life to studying the lining of the uterus and more specifically, how to help embryos stick better. His research led to the discovery of the role of BCL-6 in the lining of the uterus as a diagnostic test patients can have done by their doctors to guide them toward the best protocol for transfer success. The test isn't for women just going through IVF either. This test can also be done to help women get an endometriosis diagnosis without going through a laparoscopy surgery. ⠀ ⠀ In addition to being a Fertility Doctor at The Fertility Center of the Carolinas, in Greenville SC, Dr. Lessey is also the scientific advisor for Cicero DX. His work on identifying BLC6 became the key component of the Receptivadx test. ⠀ ⠀ I'm delighted to have him on the show! Many IVF patients have benefited greatly from the Receptivadx test, and I'm looking forward to asking him a lot of questions about his work. S7 Ep 491I have a double uterus, and two cervixes, and have been trying to get pregnant for seven years. Do you have any resources or guidance for me? (Ask The Egg Whisperer)
Recently, Jess sent a question in to Ask The Egg Whisperer. Here is what she had to say, "I'm 41, have been trying for 7 years to have a baby. My husband is 39 and has tested fine, no issues. We've went through two IVF rounds in 2018. My husband and I have tried basically every fertility treatment and have not yet had a baby. I got pregnant out of the blue in 2017 but it ended up being a blighted embryo. I have uterine didelphys (double uterus) with a complete septum and two cervixes. Based on all the tests over the years, low ovarian reserve seems to be the main issue. I've never received a formal diagnosis. It's always been "unexplained". S7 Ep 490I'm 42, and have had 2 failed IVF cycles. What tests should I consider, and what should I do differently for this next cycle? (Ask The Egg Whisperer)
Recently, Isiuwa sent a question in to Ask The Egg Whisperer. Here is what she had to say, "I am a 42 year old woman who started IVF in 2019 that ended in a failure. I also did an HSG and was told that I have blocked fallopian tubes. I recently experienced my second IVF failure. This second round, I decided to genetically test my embryos (2 euploid and 1 aneuploid/low mosaic). Now that I have 2 failed cycles, I am seeking your advice on what you think I should do differently this time around. I am thinking of requesting the following tests to start with: 1. An ERA and Receptivadx tests. 2. Immunology tests such as thrombophilia and killer cells test. 3. A natural or modified natural cycle. Thanks so much and I await your guidance." S7 Ep 488Ask The Egg Whisperers w/guest Dr. Merhi (Should I continue high doses of medication and do mini-IVF?)
Dr. Zaher Merhi joins Dr. Aimee for this episode of Ask The Egg Whisperers. The first question we take is from Janice, who shares this, "My husband and I have been trying to conceive for the past two years. I am 36 and my husband is 43. In January 2020, we decided to get a fertility workup. My uterus was fine, tubes clear and hormone levels normal. Should I continue with high doses of medication or perhaps think of doing mini-IVF? Can high medicated doses affect egg quality? Is there anything you can recommend so that I can increase my chances of getting a normal embryo w/ my next round?"
