S1 Ep 60#60 - Post-partum Challenges with Dr. Charles

Featured in this week’s episode of You Are Not Broken host Kelly Casperson, MD interviews OBGYN Dr. Meedlen Charles. Dr. Charles will discuss the effects of Postpartum Depression and how women can be embarrassed talking about the issues they have after giving birth. Dr. Meedlen talks about the importance knowing that women with previous conditions of anxiety and depression are more likely to be affected by Postpartum. However, most women will have effects after birth, and this is a normal part of giving birth that many do not want to discuss. In some cultures, it is frowned upon to talk about what women experience after pregnancy and birth. Most women will experience many difficult side effects after birth including lack of desire, body issues, anxiety, depression, vaginal dryness, and more. Some women may feel as if these effects are uncommon, but the reality is that many women experience side effects after giving birth. Women may have a difficult time having sex with their partner or breast feeding for a long period of time. When returning to the work force, women might endure a hard time “wearing multiple hats” and managing their role as a new mother. These feelings are normal and common in most cases. We talk about pain and dryness with sex. Hormones can be a reason for this, so can birth trauma. Tips and tricks and our favorites! Typically, there are appointments a few short weeks after giving birth to observe if a woman is experiencing postpartum. Unfortunately, many believe that postpartum will stop after 30 days or 3 months, but the reality is that it can last up to a year after giving birth. The highest risk for perinatal suicide occurs between nine months to a year after giving birth. Learn when Dr. Charles brings in physical therapy to a woman’s healing plan post-partum. We talk about low libido post-partum. So common and there are so many reasons why. The best cause of action is to talk honestly to your doctor because while it may feel embarrassing, they are there to help you. They can recommend lotions or oils to make your body feel more comfortable during sex, therapists to help you deal with your anxiety or depression, and more. S1 Ep 42Get Out the Vote!! - Sex Education on the Ballot and a Sex Therapist Weighs in.

Today I am here with sex educator and sex therapist Tracy Dalhstead-Rienstra. She wanted me to talk about this important piece of legislation coming out on the November ballot for people who live in Washington state. As an expert in the field of sexuality, she talks about the current standard of education that she sees in adults in her practice and what adults who didn’t get a good sex education look like in their struggles in intimate relationships. BACKGROUND Senate Bill 5395 (SB 5395), passed by the Legislature in 2020, would require Washington’s public schools to provide comprehensive sexual health education instruction that is medically and scientifically accurate, evidence-informed, age-appropriate, aligned with health and learning standards, and inclusive of all students regardless of their protected class status. A referendum called Referendum 90 will be on the November ballot asking voters to approve or reject Senate Bill 5395. A 2017 Centers for Disease Control and Prevention (CDC) survey indicates that nearly 40 percent of all high school students report they have had sex, and 9.7 percent of high school students have had sex with four or more partners during their lifetime. Among students who had sex in the three months prior to the survey, 54 percent reported condom use and 30 percent reported using birth control pills, an intrauterine device (IUD), implant, shot or ring during their last sexual encounter. The birth rate for women aged 15-19 years was 18.8 per 1,000 women in 2017, a drop of 7 percent from 2016. According to CDC, reasons for the decline are not entirely clear, but evidence points to a higher number of teens abstaining from sexual activity and an increased use of birth control in teens who are sexually active. Though the teen birth rate has declined to its lowest levels since data collection began, the United States still has the highest teen birth rate in the industrialized world. Sex Education and States All states are somehow involved in sex education for public schoolchildren. As of October 1, 2020: § Thirty states and the District of Columbia require public schools teach sex education, 28 of which mandate both sex education and HIV education. § Thirty-nine states and the District of Columbia require students receive instruction about HIV. § Twenty-two states require that if provided, sex and/or HIV education must be medically, factually or technically accurate. State definitions of “medically accurate" vary, from requiring that the department of health review curriculum for accuracy, to mandating that curriculum be based on information from “published authorities upon which medical professionals rely.” Research on sexual abuse shows comprehensive sexual health education is an important and effective prevention strategy. When students learn about and develop skills related to affirmative consent, they are more able to set personal boundaries and respect the boundaries of others. (Schneider, M., & Hirsch, J. S. S1 Ep 25#25 - Meet a Pelvic Pain Expert Physician - Dr. Gerig!

Dr. Gerig is owner and founder of www.pelvicsolutionscenter.com. She sees men and women who have pain in the pelvic region, which may include the lower abdomen and back, gluteal area, genitals and rectum. She is an expert in interstitial cystitis (IC). Learn that not everyone that comes into her clinic with interstitial cystitis actually has IC. The classic IC symptom – pain perception in the bladder when it fills up and it is relieved when you urinate. There usually is associated urinary frequency. Pelvic floor muscle tension (high tone pelvic floor) is more common than IC and is often confused with IC. IC, pudendal neuralgia and pelvic floor muscle tension are the three main things Dr. Gerig treats. What a PT and a pelvic floor expert finds on physical exam cannot often be seen on a CT scan. She integrates the role of physical therapy in her practice. She thinks 95% of her patients need PT. PT is the primary treatment except for ulcerative IC. Ulcerative IC is a condition where there are ulcers in the bladder (Hunner’s ulcers). Physical therapy is vital in educating women about their bodies and bodily function. Clean eating is helpful. Your primary fluid intake should be water. Integrate an anti-inflammatory diet. Integrate movement every day into your life. Slowly increase your activity as you are able. Dr. Gerig thinks movement and stress management are very important when you have muscle tension. Tips for stress management!!! Healing from pelvic pain takes time. There is no magic wand! It takes time and work. Medications for IC and pelvic pain. Elmiron side effects. Supplements used – glucosamine and quercetin, aloe vera. Other medications used: histamine blockers, neuromodulators, vaginal muscle relaxants. Dr. Gerig’s goal is to get her patients OFF medication eventually. Switching topics, Pudendal neuralgia. This is a pelvic nerve called the pudendal nerve, it feeds the rectum, urethra, genital. Symptoms are burning, throbbing pain, worse with sitting. People often have urinary frequency and a “foreign body” sensation in the pelvis. It can cause pain with urinating, pain with sex, pain with bowel movements. Post childbirth child pain. Prolonged pressure of the baby’s head on the pudendal nerve can cause urinary issues. Women can have pain with sex, pain in tailbone. Find a PT who is well trained in treating pelvic pain.
