When Sex Hurts – Dr. Krapf Dr. Jill Krapf is a gynecologist specializing in vulvovaginal disorders in Tampa, FL Dr. Krapf is active in research and has published chapters and peer-reviewed articles on vulvodynia, vulvar dermatoses, and hypoactive sexual desire disorder. Check out my other episodes #131 – National Lichen Sclerosis Day and #172 – Vulvar Itch Why write a 2nd edition of the book “When Sex Hurts”? How common is pain with sex for women? Often times with pain with sex there is a physical cause, but you can’t always see it. Pelvic pain and pain with sex is very isolating and people feel very alone. People don’t know how to talk about it. What happens after the pain is fixed. What does “all in your head” mean and why people don’t like that. “Those who perceive their partners as kind and understanding report more intense pain than women whose partners have responses that promote adaptive coping.” – what! What is adaptive coping? How to communicate your pain to your partner and your doctor. What if you are a partner of someone with dyspareunia - - “best partner response is having a partner who engages in what researches call facilitative responses – affection and encouragement to cope with the pain in an adaptive way, such as by focusing on pleasure, deep breathing or positive thoughts while experiencing the pain.” We talk about stress, sex and the gut – things like irritable bowel syndrome related to pain with sex? The role of birth control pills and pelvic pain and health. In her book, “When Sex Hurts” they talk about Yaz (drospirenone) and pelvic pain – a prospective study of women’s genitals and sexual function prior to starting Yasmin – and then after three months- decreased clitoris, decreased blood flow, decreased labia minora, decreased libido, fewer orgasms and more dyspareunia. The role of testosterone in pelvic pain – one study showing women with higher androgen levels have less work related neck and shoulder pain then women with low androgen and differing levels of Testosterone may explain some of the gender related differences in pain perception The difficulty in giving testosterone to women and the problem with pellets. S1 Ep 213213. You Are More Than Just Your Breast Cancer - Hormones and Breast Cancer
You Are More Than Just Your Breast Cancer - Hormones and Breast Cancer Dr. Corinne Menn is a board certified OBGYN and North American Menopause Society Certified Menopause Practitioner. She has a special interest in areas often neglected in women's health: Perimenopause and menopause, the unique needs of female cancer survivors and those at high risk for breast cancer. Dr. Menn tells us her story of breast cancer dx at age 28, going through menopause three times and is now a pro-hormone educator. Dr. Menn discusses taking systemic hormones and her journey and how it has changed her life. Why are doctors and patients so afraid of hormones? Doctors are so risk adverse – but let’s talk about the risks of not treating menopause. The myth that estrogen causes cancer. How Tamoxifen works. Breast cancer nomenclature – estrogen positive – this doesn’t mean estrogen caused the cancer – we clarify. The nuance of being overweight and risk of breast cancer. The number one killer of women with history of breast cancer is heart disease. Safety of systemic hormone replacement therapy in breast cancer survivors Article in Breast Cancer Research and Treatment · January 2022 – Dr Bluming - "Twenty-five studies of HRT after a breast cancer diagnosis, published between 1980 and 2013, are discussed, as are the 20 reviews of those studies published between 1994 and 2021. Only 1 of the 25 studies, the HABITS trial, demonstrated an increased risk of recurrence, which was limited to local or contralateral, and not distant, recurrence. None of the studies, including HABITS, reported increased breast cancer mortality associated with HRT. Even in the HABITS trial, the absolute increase in the number of women who had a recurrence (localized only) associated with HRT administration was 22. It is on the basis of these 22 patients that HRT, with its demonstrated benefits for so many aspects of women's health, is being denied to millions of breast cancer survivors around the world." The problem with the HABITS trial. Why are we scaring women so much about breast cancer? Who is the right breast cancer survivor to start hormones? A good candidate? S1 Ep 112212. Sex Med and Urology - Top Takeaways at the 2023 AUA conference
I went to the 2023 American Urologic Association's National Conference. Here are some top takeaways and thoughts. So much on this week’s podcast! Low hormones in men and women — can increase risk for cardiovascular disease Women have a decreased risk of heart attack and stroke when started on hormones in the safety window of menopause (first 10 years) The Traverse study paper coming out in two months - can’t wait! Testosterone is not a gendered hormone All bones need testosterone Women in early menopause should be on hormones at least until natural age of menopause to decrease risk of cardiovascular disease, osteoporosis (NAMS guidelines and this indication has FDA approval). Equality in healthcare Three new oral testosterone pills on the market for men, not for women We need to focus on brains and bones health in women just as much as we do for men Treat women the same in healthcare Did you know the UK is requiring doctors to talk to their patients at age 50 about menopause? Amazing! If hormones are so dangerous, why don’t we stop estrogen in trans women after the age of 50 - a thought exercise on how we think about giving hormones to all bodies - Is staying on hormones during menopause gender affirming care? Oral DHEA supplements — what’s the deal? Bulkamid — urethral filler for stress incontinence ECoin training Prolapse repair in premenopausal women Should we offer it or no? Male infertility is associated with shorter life expectancy Marijuana affect sperm Using testosterone for maintenance of vaginal health Michael Pearlman balance scale of desire More about Addyi (filbanserin) Importance of elastin and collagen Very small percentage of women have cervical orgasms - but it matters. 211. Feeling Broken In The Bedroom? What You Need To Know About Your Sexual Health
Feeling Broken In The Bedroom? What You Need To Know About Your Sexual Health - With Dr. Kelly Casperson This is an interview I did with Lesley McShane for her podcast “Redesigning Midlife” and it was just such a good interview I asked her if I could share it again. 1. Basics: a. Menopause means it’s been one year and one day since your last period. b. The average age of menopause is around 51. and c. for ten years prior to menopause, women go through peri-menopause, where hormones are doing a reverse puberty. 2. A lot of women stop or have pain during penetrative intercourse during this time due to a decrease in natural lubricant and collagen, but this can be easily treated. 3. Even if you don't have menopause symptoms, if you are over the age of approximately 51, you have gone through it. 4. The most sexually satisfied people aren’t young people, it is people in their 50s, 60s, 70s. 5. Great sex is not a passive thing. It’s an active process, just like someone that is physically fit. It has to be prioritized. 6. Use of lube increases your success of orgasm by 70-80%. Silicone based lube is best because you don’t absorb it like you do the water based. 7. There is no “how long” should I use Vaginal Estrogen cream. The use of it is like using sunscreen or wearing a seat belt. 8. You don’t need a “sex drive”. We do things because they are awesome. If you aren’t having awesome sex, don’t wonder why you don’t desire it. 9. We feel entitled to a spontaneous sex drive that has been sold to us by media. And when we don’t achieve that, we end up feeling broken. 10. Putting something in the vagina only leads to female orgasm 30% of the time so special focus need to be paid to the female body. Your largest sex organ is your brain. 11. Don’t sit passively by waiting for spontaneous desire to happen. Get dressed up and go to the party even if you don’t think you want to. Chances are you’ll have a good time once you get there. 12. Control of stress and cortisol levels in our bodies will help us desire more intimacy. 13. If there is a desire mis-match in your relationship, normalize it by talking about it. 14. Communication with our partner about menopausal symptoms and what we are going through is important to our relationship. 15. Get educated on hormone replacement therapy as an option. When women start thinking about how they want to live as they get older and get educated, then they see how beneficial HRT could be for them. 16. Women between the age of 50-60 that are on hormone therapy live 3-4 years longer than women in the same age group not on hormone therapy. 17. For our young adult children that are starting their own sexual lives, the biggest thing they need to know from the start is about consent. 18. They need to be aware of is orgasmic equality. 19.We have to educate our girls that pleasure is for everybody. I love how you call private parts “personal parts” – why is this important? So many middle aged women have body shame – when does this start? How she found out her book was banned. S1 Ep 183183. Religion and Sex – Can They Be Bedfellows?
Religion and Sex – Can They Be Bedfellows? Today we discuss the Muslim culture, sex, and vaginismus with Dr. Sadaf Lodhi, OBGyn and sex coach for Muslim women. She is host of the Muslim Sex Podcast – we discuss her origin story Religion can have a profound effect on one’s sex life/sexuality. Islam is a sex positive religion. When the Muslim countries were colonized they tended to become more puritanical. Islam discusses sex within the context of marriage. Many times women experience shame and people don’t talk about women’s pleasure. Common things Dr. Lodhi sees women coming to her with: Seeing sex as a duty Low libido Beliefs like sex is dirty or sex will be painful One of her most important things she likes to do is empower women through education. Women get pressure from their partner and an expectation that it is a duty. And they are failing them and God. Vaginismus – the involuntary contraction of the muscles surrounding the lower 1/3rd of the vagina. 1/3rd of Muslim culture may struggle with vaginismus. The role of mindfulness in helping vaginismus and low desire. “People make putting something in the vagina a big freaking deal” – Dr. C. The role of PT in treating vaginismus. There are no FDA approved medications for vaginismus. Dr. Lodhi shares a huge win with us. It is not about the number of times of sex a week. The role of the scholars and the males in helping women feel sex positive. S1 Ep 180180. Becoming Empowered and Getting Vulnerable with Dr. Casperson
EListen to this very deep and vulnerable conversation I had with my friend Dr. Kathy Zhang on her podcast, The Purpose Filter. There are so many aspects to living a life of fulfillment, but what if there was one area of your life you could improve that would spill over into all others? Sounds like a bit of a life hack. Well, it is, and we want to share it with you on today’s episode. Is your sex life fulfilling, or are you just going through the motions? The empowerment you get from knowing what you want in the bedroom and being able to speak up and ask for it, or the ability to say no, when no means no, is a crucial element to all aspects of your life. If you can stand up for yourself in these very vulnerable and intimate situations, the empowerment that happens outside of the bedroom will come naturally. This all starts with you and your willingness to make a decision to uplevel your life and stop living on autopilot. “Should we care about the people who are sleeping with the people we are giving Viagra to?” “My husband died so now I need to take care of myself”. “It is easy to be average” “She just wants the damn thing to end – which is another reason that women fake orgasm” On faking orgasms - “He thinks it is the end of the party and you were never at the party.” “You can’t just give somebody desire. It is an inside job” “I think working on your sex life is the final frontier of personal growth.” “Everything you do to have a better sex life spills over to other aspects of your life” “What if my whole purpose in life is to tell people to use vaginal estrogen cream?” Tune in to hear how you can create more joy and abundance in your life! Topics discussed in this episode: • Dr. Casperson’s experience in a largely male dominated field • What is it like becoming the first doctor in the family • Bad advice Dr. Casperson received when she was becoming a doctor • Giving yourself permission to step into your power • Why your voice is important and unique • What was Dr. Casperson’s turning point to change the way she lived her life • Applying dying lessons now while we are living • Why women feel their pleasure and themselves are not a priority • How to move past shame and guilt surrounding sex and beyond • Going from being pulled along for the ride to getting into the driver seat • How young people should re-frame purpose • Sharing the gift of awareness and presence Connect with Dr. Kathy Zhang on Intagram https://www.instagram.com/purposefilter/ And on her website: https://luyikathyzhang.com/ Did you get the You Are Not Broken Book Yet? https://amzn.to/3p18DfK Join my membership to get these episodes ASAP when they are created and without advertisement and even listen live to the interviews and episodes. www.kellycaspersonmd.com/membership --- Send in a voice message: https://podcasters.spotify.com/pod/show/kj-casperson/message Learn more about your ad choices. Meet Glen Lovelace! I love his generosity toward helping men in relationships and his masculine energy on this podcast! He shares his story from desperate (even suicidal) plans to living a life of helping others live their best relationships. When women learn about their sexuality and get empowered, often they feel stuck because of a spouse who they fear is not open to change. I wanted to bring on someone who literally deals with this as his job. Glen helps men who's wives have been transformed in one way or another, usually by coaching education. Glenn Lovelace is the wingman for men whose wives have transformed from coaching. He is a certified coach through the life coach school. Glenn has coached over 3,000 clients 1 on 1 since 2008. His transformation started with bodybuilding after being in a very low place in his life. He decided he needed to prioritize himself to be the best spouse and father he could be. "It’s okay to be uncomfortable." How to “gently” change your partner (and remember you can’t change anyone who doesn’t want to be changed). 1) Show them how you have changed 2) Bring in a third party influencer 3) Learn how to speak the same language 4) Always remember you two are on the same team Roomate Syndrome – just existing together and losing touch why you love and are attracted to each other. If one person does their work, the marriage can get better. Your spouse is just there to love. Can couples start speaking the same language? Glen says yes! Intimacy not in the bedroom is important! Wanting your spouse to change for you to be happy is a toxic plan. Happy Wife Happy Life - it’s a lie and Glen explains how it wasn’t good for him. Each person is in charge of their own pleasure. Don’t assume what is needed in a situation - - Ask How to be a man worth desiring? Start dating and desiring your wife. Approach her how she wants to be approached. Date every week. S1 Ep 177177. Me and My Body Are A Team - Teaching Reconnection with Katrina Ubell, MD
A mindwork episode! Dr. Ubell is one of the first physician life coaches! She gained and lost the same 40 lbs at least 10 times since puberty and now helps women become a team with their body and stop obsessing over food using the power of mindwork. She reached out to me to be on the podcast and at first I didn’t know how to connect the dots besides life coaching. I have a podcast episode called “sex while fat” – we are okay at any size. I was worried she just wanted to talk about losing weight. So I said Okay if we can talk about how mind work in her arena can also relate to a positive body image and sex life. Like my older episodes talking about money and sex – these topics are more related than we think because of our thoughts about them. “I am interested in people stopping hating themselves” – Dr. Ubell What is diet culture? The concept of mutual respect between you and your body. Can weight loss goals exist with body positivity? Shame is useless. “I haven’t had success so there must be something wrong with me” Dr. Ubell Why do we get numb or disconnected from our bodies? Over eating as a trauma response – sexual assault or childhood trauma – I got big to not feel or too attractive. Changing one’s identity requires changing our thoughts. Menopause and weight gain/loss – watch what you believe about this. Acceptance of our bodies as we get older. Entitlement thoughts! I feel so seen! I am entitled to my latte! How to stop invalidating your emotions Relationship with yourself and thoughts about yourself are important for both weight loss and sexuality. S1 Ep 174174. Vibrators, Princess Marie Bonaparte, and Motorcycle Coils
EFirst I read from the recently published article, "The Anatomy of Female Sexual Arousal: Princess Marie Bonaparte, the Halban-Narjani Procedure, and the Art of Constantin Brancusi." Then I talk about seven different types of vibrators. Then I finish with a tragic case of motorcycle spring meets penis. Enjoy! 1. The OG - Hitachi Magic Wand. external, powerful and marketed as personal massagers 2. 𝗖𝗹𝗮𝘀𝘀𝗶𝗰 𝗽𝗲𝗻𝗲𝘁𝗿𝗮𝘁𝗶𝘃𝗲 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - put curve on them, call them G’Spot’ and raise the price 3. 𝗖𝗹𝗶𝘁𝗼𝗿𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - these focus on external clitoral stimulation. Air pulse – Wevibe Melt – Best in class IMO. 4. 𝗗𝘂𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 𝗮𝗸𝗮 "𝗥𝗮𝗯𝗯𝗶𝘁𝘀" - Work in and out at the same time. 5. 𝗕𝘂𝗹𝗹𝗲𝘁 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - usually a "starter" vibrator for most folks. 6. 𝗖𝗼𝘂𝗽𝗹𝗲𝘀 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - many many different types here. Eve by Dame is one that is "hands free", many strap onto a penis. 7. 𝗔𝗻𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - kinda self explanatory. Donnenfeld SR, Stanfield AV, Hammett J. Did you get the You Are Not Broken Book Yet? https://amzn.to/3p18DfK Join my membership to get these episodes ASAP when they are created and without advertisement and even listen live to the interviews and episodes. www.kellycaspersonmd.com/membership Vaginal Lasers How a male can introduce my book to his female partner Unopposed Estrogen can cause harm Q&A from listeners: Genetic testing in 2017 and was positive for Lynch Syndrome. I was just diagnosed with uterine cancer yesterday. 43 years old and total hysterectomy in my very near future. Terrified about trying to get my hormone levels correct post surgery!! Any suggestions or recommendations? I’m a 38 y/old breast cancer survivor. I’m on estradiol, got all the fancy lubes, and now I’m curious about testosterone… People only seem to talk about estrogen for women, but my libido is nonexistent and penetrative sex is excruciating. Curious if testosterone also takes a hit while on hormone blocking meds post-cancer. I discuss this stuff with the oncologist but it feels trivial compared to the work they did to cure my cancer. However, my PCP won’t touch the hormone topic with me and defers to the oncologist because of my cancer history… thoughts/suggestions? I’ve gathered so much info from you about the estradiol cream and so much more than I got my gyn to prescribe the cream for me. I’d like to note that I am 59 and I have never had any problems with painful sex or any vaginal issues in the past. I applied the first dose both inside and a bit on the outside areas, it was fine with no noticeable changes. But 4 days later I applied the second dose, this time just inside and within a day or so I felt changes. My clitoral area was sensitive with discomfort and I experienced dryness in the outer labia like never before - not really painful but noticeable discomfort. I have not taken a third dose but I really want to be able to use this so I don’t shrivel up and go away LOL. I also was getting a few hot flashes and they really decreased after the first dose but are back now, although not as much. Is it possible that the discomfort will diminish after a while if I keep taking it? Would it be effective if I only applied it once weekly? I’m 58 and 2 years in menopause. I use an estrogen patch, progesterone, and a testosterone cream - thanks to you - but it’s not clear to me if my transdermal estrogen will protect the vulva from atrophy. Should I also be using an estrogen cream on my vulva in addition to my HRT regimen? I asked my doctor and she said creams for the vulva are only from lichen sclerosus, but that doesn’t sound right.
