Ep 136Using DNA Methylation to predict biological & functional ages - Episode 136

Join Dr. Gladden on this conversation with Ryan Smith about epigenetic age testing and DNA methylation. Ryan has a biochemistry degree from the University of Transylvania. He went to medical school and started clinical rotations before realizing he didn't want to be a doctor. That's when he pivoted to the world of pharmacy, focusing on peptide synthesis, and opened up Tailor Made Compounding. Ryan has been very successful in his endeavors, with Tailor Made Compounding becoming the 4th fastest growing healthcare company at the time, investing in several other functional medicine spaces and, eventually, starting TruDiagnostic. This is a CLIA certified lab and health data company focused and committed to research with over 30 approved clinical research studies investigating the epigenetic methylation changes of a variety of longevity and health interventions. They have created one of the largest private epigenetic health databases in the world with over 13,000 patients tested and continue to lead the space in epigenetic methylation interpretation and diagnostic prediction. In this episode, him and Dr. Gladden discuss how epigenetic age testing and DNA methylation have evolved, the different kinds of clocks that exist now and what they measure, as well as the advances being made and envisioned in the disease prevention and age reversal spaces. Show notes: 00:55 Dr. Gladden gives a description of this episode and an introduction of Ryan Smith. 02:20 Dr. Gladden explains why he is excited for this show. 03:15 Ryan introduces the audience to TruAge and tells us the big takeaway from this testing. 04:50 What is epigenetic methylation? What are the two main epigenetic marks? Ryan explains our DNA's "on and off switches". 05:57 Dr. Gladden clarifies what is being measured exactly. 06:29 The concept of methylation clock is introduced by Dr. Gladden, and he explains how it keeps track of the aging process but also contributes to it. 07:11 Dr. Gladden asks if Ryan's company is measuring acetylation patterns as well. 07:17 Ryan talks about clinical trials where they are also measuring acetylation patterns and why it's proven not to be ready for prime time large-scale investigation. 08:20 Dr. Gladden asks if they're hitting a plateau in terms of accuracy with the amount of genes being tested. 09:02 Ryan explains why doubling the amount of genes being tested doesn't necessarily translate into a doubling of accuracy. 09:09 How are they getting an insight into the path of physiology? Ryan talks about seeing what is changing and why it is changing, why this is the best way to capture the aging process (which is the biggest risk factor for chronic disease and death) and what questions they are trying to answer. 09:59 Dr. Gladden asks if they are hitting a plateau in cost-effectiveness. 10:49 Dr. Gladden tells the audience why they should care about this and what sets TruDiagnostic apart from other methylation companies. They can predict disease and mortality. 13:44 Ryan talks about the evolution of these algorithms and what people can expect from the reports of their testing. 14:40 Second and third generation clocks are introduced during this intervention before tackling their reports individually. 17:15 Dr. Gladden talks about the benefits gained from these reports/testing. 18:55 What can people do to impact their methylation? Ryan elaborates on the different types of interventions, from lifestyle changes to clinical interventions. 23:19 Dr. Gladden introduces very small embryonic-like stem cells and talks about how there's data being recorded in some trials where these are moving the epigenetic age even 3 or 4 years back in time. 25:30 Ryan talks about what he's seeing with plasma and plasmapheresis interventions. Explains why the results they're seeing are giving them so much hope that these therapies might be one of the ways to reverse the clock. 29:30 Dr. Gladden explains why they can't look at one data point and say that's the answer in this field. 29:58 They introduce the DunedinPace algorithm for rate of aging, which is the first ever third generation algorithm. Ryan explains why it's one of their favorites and how it is making things clearer and giving one of the best metrics they can offer. 35:00 Dr. Gladden talks about what they don't have dialed in yet with these therapies. 35:19 Ryan describes how they're trying to bridge the gap by providing answers to those "how long" questions with their data sets. 36:20 Will the system spring back to its natural age? Or will we be able to reset it? Dr. Gladden poses the question based on the concept of transient epigenetic aging. 36:58 Ryan talks about what he thinks is probably the most exciting area of age reversal right now. 38:49 Dr. Gladden asks Ryan how people can reasonably compare between companies doing this kind of testing and choose their best option. 40:40 Ryan explains the different tissues that can be used for testing and how that's going to affect the algorithm'

Ep 135Sporebiotics: A Better Approach To Gut Health & Wellness? - Episode 135

Join Dr. Gladden and Tina Anderson on this conversation about spore-based probiotics and their ability to rehab your overall health by targeting the bacteria in your gut. Tina Anderson, formerly a lawyer, used to work in the pharmaceutical industry before finding out that it wasn't the best way for her to help people and change their lives. After a harsh revelation during the peak of their careers, Tina and her husband pivoted to the world of natural and progressive health, and disease prevention. As a result of their discovery, they created a unique supplement with superior probiotic strains, giving birth to Just Thrive. These spore-based practitioner grade probiotics are the product that Tina and her husband created to help you take control of your health and change your life. 01:10 Dr. Gladden tells us what's exciting about this episode and the things we will come to understand. 02:10 Dr. Gladden introduces Tina Anderson. 03:20 Tina shares her story: the journey to the world of probiotics. 04:15 We get an insight into the abuses in the pharmaceutical industry. 04:53 Tina tells us the Sales Representative anecdote that really made them pivot from the pharmaceutical industry in the midst of a high point in her and her husband's careers. 06:47 We learn about the day the opportunity to create Just Thrive presented itself for Tina and her husband. 08:16 Tina lets us in on her biggest motivation. 08:45 Tina explains why her husband was seeing a naturopath at that time. Was someone in the family experiencing gut issues? 09:59 We are introduced to spore-based probiotics and what's special about them. 10:48 Tina explains what is the biggest difference between spore-based probiotics and regular probiotics. 10:58 Dr. Gladden explains that dead bacteria can also have an effect on our guts but live bacteria could, theoretically, be better. 11:25 Tina elaborates on what spore probiotics do that other probiotics can't do. 12:08 Tina talks about a gut model study where they confirmed a 30% favorable shift in the microbiome after 2 weeks on it. 13:04 Dr. Gladden explains that there is no "normal" gut biome. 14:08 We find out how exactly are these spores modulating the gut biome. 14:25 Tina makes a very good analogy where our gut is a garden and sporebiotics are the gardener and not just new seeds, as a way to differentiate them and their effects from those of regular probiotics. Are new seeds enough? Or do we need a gardener? 15:15 Tina also clarifies a big distinction in the colonization period of spore-based probiotics vs non-spore probiotics. 15:41 Dr. Gladden explains the concept of spores and asks how they work in this context. 16:57 Tina talks about a study where they measured the spores antibiotic survival capabilities. 17:16 Dr. Gladden elaborates on how antibiotics alter the gut biome. 17:54 Tina explains that spore probiotics can rehab AND maintain our health. 19:05 Dr. Gladden asks if sporebiotics can repair leaky-gut. Tina goes on to describe just how impressive is the level of restoration spores can achieve, proved by a double-blinded study that was performed. 21:31 Tina gives a better understanding of how exponentially detrimental leaky-gut can be to our health with a helpful leaky faucet comparison. 22:03 Dr. Gladden explains that the body is not built to be fragile but it has a threshold, resulting in people living with issues they don't know they have which are driving their inflammation levels up and accelerating aging. 22:30 Tina lists a number of issues and disorders that are stemming from the gut, although we don't associate them with it. 24:05 Tina describes the spore-based probiotics success with bloating issues, diarrhea, and constipation. 24:40 Tina explains that our gut is foundational for our health and how it is the starting point of general wellness. 25:26 Dr.Gladden asks about using spore-based probiotics with antibiotics to decrease the negative effects of the latter, like induced diarrhea. Tina talks about a study that proved the spores effectiveness with this, and survivability to the strongest antibiotics. 26:31 How is it helping people's brain function? 27:00 Tina lists all the brain hormones that are produced in our gut. 27:17 Tina introduces the psychobiotic! 28:02 Tina tells us about other offenders of brain health that the psychobiotic is also helping with. 28:44 Dr. Gladden introduces the topic of probiotic cocktails. 29:17 Tina explains why a cocktail makes much more sense than just taking one probiotic. 30:27 Dr. Gladden inquires about what's on the drawing board for Just Thrive. 31:35 Tina explains that they're only focused on bringing what's truly missing in the market and how they want to make an impact by providing what's needed and what's best. 32:18 Dr. Gladden asks what kind of conditions/disorders they're targeting with the psychobiotic. 32:47 Tina explains how the psychobiotic targets not only certain disorders but specifically works to improve cognitive function too. 33

In this episode, they discuss her personal journey to the creation and development of Pendulum, how it works to treat conditions like metabolic syndrome, inflammation and neurodegeneration, and the ways in which treating and maintaining a healthy microbiome could be a hack to successful aging and disease-free longevity. If you are looking for a natural route to overall health and optimization, this episode is for you. Show notes: Dr. Gladden introduces Colleen, and she introduces herself, gives us her personal journey to the creation and development of Pendulum, her personal connection to it. (1:30) Colleen talks creating products that have the efficacy of a drug but the safety of a probiotic. (2:40) Colleen mentions a study that showed how kids taking antibiotics before six months of age are systematically more prone to diseases like obesity and type two diabetes as they age. Dr. Gladden mentions how antibiotics affect children. (4:00) Colleen tells us how butyrate is a small molecule with a really important role in a variety of different things. Our colon cells, for example, are the only cells in our entire body that use butyrate as their source of energy, as opposed to glucose. (6:10) Dr. Gladden explains how the fiber metabolism generates butyrate that feeds the intestinal lining, including how fascinating it is that this butyrate is responsible for releasing GLP-1. (7:30) Colleen clarifies that if you are missing your microbes, ingesting fiber does not make much difference. (10:00) Dr. Gladden mentions some misconceptions about microbiomes and comparing the person's stool against an "ideal" stool. (11:59) Dr. Gladden talks about keystone strains and explains that what we are eating affects and influences them, highlighting the importance of eating different kinds of fibers. (14:00) Colleen describes a study they did, seeing a way to influence the microbiome and then have this impact on A1C and blood glucose. (17:00) Dr. Gladden talks about how microbes also affect appetites and dictate your cravings. (19:00) Colleen explains that the (Pendulum) formulation is stable but needs to be refrigerated, and lists the five strains: Akkermansia muciniphila, Anaerobutyricum hallii, Bifidobacterium infantis, Clostridium butyricum, and Clostridium beijerinckii. (21:10) Colleen mentions some of the trials they have done and the approach they adopted. (24:10) Dr. Gladden brings up neuroinflammation and poses the question of Pendulum's capacity in decreasing it. (27:04) Colleen elaborates on the known relationship between managing your blood glucose spikes and neural inflammation. (28:05) Dr. Gladden explains what resting metabolic rate is and how it is measured. (32:44) Colleen talks about the importance and role of diversity in your diet to metabolize some foods, maintaining "the fence to your garden" (gut lining of your microbiome) and the regulation of mucin, which is the glue that keeps everything properly in place. (35:34) Colleen talks about what is on the drawing board for her at the moment. (39:15) Dr. Gladden brings up the connection between autism and microbiomes. (41:30) Colleen talks about considering microbiomes as an additional intervention effort. (43:25) Colleen gives out her three takeaways for listeners: the diversification of our microbiome, the importance of regulating how your body metabolizes sugar, and giving yourself a boost with the Pendulum glucose control strains paired with the proper diet. (45:39) Colleen emphasizes the importance of people consulting their physicians when making changes that could affect their health. Hosts: Dr. Mark Young // Dr. Jeffrey Gladden Guest: Dr. William Hsu Highlights from the podcast: (0:55) Introduction (1:51) It's not because of what we know but about what we're asking and how we go about learning. (2:02) Dr. Gladden highlights The common underlying question seems to be, "Can I get back to where I was, health wise?" (4:00) Understanding the game that we're in. (8:25) In the practice of addressing all of these elements people are getting and feeling better overall more so than they have in the past. (9:10) Introducing Dr. William Hsu, Endocrinologist and graduate from Harvard (10:00) Dr. Hsu begins discussing "The Fasting Mimicking Diet" (12:40) Fasting has in large part been overlooked in the medical field. (15:30) Taking a look at Fat, the body's mechanism between feedings. (15:52) Fat is the largest organ in 70% of the American population. (17:05) What is normal? When do we eat (Has an impact on our health) and what do we eat (nutrient composition)? Circadian rhythm. (18:20) The body has an internal mechanism that tells us when to eat and when not to eat. (18:35) Illumination has changed our behaviour. (19:55) Obesity creates resistance to lupton secretary hormone that signals the body when to stop eating. (21:30) The science of Fasting began perhaps 20 or 25 years ago. (22:22) What are the molecular mechanisms that connect nutrition, fasting, and longevity? (22:50) Pulling back from nutrients actually triggers longevity genes and helps promote longer living. (23:15) Fasting actually creates an anti-fragile state. (24:00) Repeated cycle of Fasting and then eating. (24:45) Repeated cycles of fasting and feeding. (25:00) Autophagy — Self consumption. (28:06) What is the optimal hormetic stress? (28:41) Intermittent fasting. (31:21) Fasting for longer than 2 days is referred to as periodic fasting. (32:50) Autophagy does not begin until you have entered into the periodic fasting phase. (33:24) In the 5 day mimicking fast, Dr. Hsu, how did you select the types of foods involved in this fast? (33:55) Dr. Hsu, defines the 3 types of fasting, behavioral, ketogenic and molecular. (34:44) The Fasting Mimicking Diet or fast addresses the molecular or cellular fast. (35:10) Each cell has a receptor or ears and eyes that are referred to as "nutrient sensors." (36:43) Being on and off the gas in order to activate longevity mechanisms. (37:28) Human physiology is all about cycling. (39:07) After that period of fasting, what should you do to regain those nutrients? (40:45) A low calorie diet will cause an individual to lose muscle mass. Fasting changes body, emotion, and behavior. (42:12) x1 a month for the first 3 months and then quarterly afterward (43:22) Would you want to fast when utilizing hormones? (44:40) The importance of cycling with growth hormone and recuperation. (47:45) A lot of people feel better when they are fasting. Mental clarity, more energy and more active. That being said, it will not be the time you get the most gains. (49:17) Fasting changes the hormone bdnf which helps promote mental clarity. (57:45) The game is to modulate hormetic stress (1:00:44) Dr. Hsu, what are the 3 most important takeaways for fasting? Life goals trump health goals, bring someone along on this journey, and start.
