We decided to do a breakdown of this tragic event and approach it from the perspective of a reliability engineer performing a root cause failure analysis and using other relevant tools to identify the true root causes of this shooting and suggest some corrective actions.  Be prepared: we discuss this awful event rather dispassionately, so please make the effort to understand our approach when we explain it.
