We decided to do a breakdown of this tragic event and approach it from the perspective of a reliability engineer performing a root cause failure analysis and using other relevant tools to identify the true root causes of this shooting and suggest some corrective actions.  Be prepared: we discuss this awful event rather dispassionately, so please make the effort to understand our approach when we explain it. We hope that one day the proper steps will be undertaken to make sure these events never happen again.
