I've had so many patients that have had challenges around sex like not having the pleasure that they want during sex or experiencing pain during sex and not being able to communicate well with their partners. For some, this results in not being able to achieve orgasms. There are also challenges many experiences after menopause like vaginal dryness or pain that affects the quality of sex. Statistics show that over 20 percent of people are in sexless relationships, but I think the figure is probably closer to 30 percent. Many people have sex less than once a month and I'm sure you recognize that as well in your practice. I find that during fertility care many people start having issues around intimacy. When a couple is going through fertility treatment sex can start to feel more like a chore and doesn't have the pleasure that it once had. (That's if they ever had a lot of pleasure from sex). I've been able to develop tools that can help patients communicate better with their partners and tune into their bodies. Many are then able to experience pleasure in a more profound way. I'm really proud of couples and individuals that make that kind of progress."

I have patients that come to me from all over the world. There's one question I always ask when reviewing their IVF cycle history. What was your embryo quality? I hear time and time again from patients that they don't know it because their doctor told them embryo quality doesn't matter with IVF. That's simply not true! Just like diamonds in a jewelry store. Quality matters. Just like there is a grading system for diamonds, there is also a grading system for embryos. It's important for you to know that this exists and to ask for your embryo grade. With people waiting until later in life to start growing their families, I see a lot of patients who are over 40. While everyone's body is different, IVF is a great option to consider for many women over the age of 40. I've had many over 40 IVF success stories. I want you to know that pregnancy is definitely possible and that's why I'm going to share my best IVF tips with you this week. 1. Know Your Levels. Track and trend them over time. 2. Getting your levels checked doesn't mean you have a problem. It simply means you want to understand your body. 3. So often, women over 40 don't realize they need help until after they've miscarried. 4. Recognize that you're not alone in wanting to get pregnant naturally. 5. Ovaries are ready for other options, but you may not be. 6. Know what's okay to say and feel. 7. Know that sperm matters as much as eggs. 8. Think about other things to improve your chances of a successful IVF cycle: Egg Whisperer DIET, Considering PGT, and Implantation Testing. 9. Think about working with a surrogate. 10. Assemble your fertility TEAM. 11. Preserve your fertility & consider embryo banking. 12. Know what you may be more likely to experience with IVF over 40. 13. If you're 45 and you've just had a baby. The one thing you want to do is try and live forever, as healthy as you can be. I want to close with one important message. As a fertility doctor, one of the most important parts of my job is helping women close to menopause grow their families. Helping them with IVF cycles until they are successful sometimes means also using an egg donor. Women over 40 who are doing IVF should hear that. Knowing that sometimes your success may involve an egg donor is something that's important to hear at the beginning. One of the most common questions I get asked is "Why didn't my IUI work?" It's a great question because there are many factors that come into play with intrauterine insemination. If you're my patient, we'd start by checking the fallopian tubes, uterus, and sperm quality first. But let's say you've done the #TUSHYcheck, the tubes are clear and everything else looks good. So why doesn't IUI work? The answer I share with my patients is: because you're human. Yes, it's really as simple as that. To put IUI into perspective, 90% of the time it doesn't work. So if your IUI didn't work, I want you to know that there's nothing wrong with you, and you're normal, and human. It's that simple. Young women are donating eggs at the time they are most fertile, often times not realizing the implications of what that means. I've had patients come to me in their 40s without enough healthy eggs of their own to conceive. When they share with me that they themselves donated their young and healthy eggs in their 20s or 30s, but that none are available for them, I feel frustrated and sad for them. Have you ever used Google Maps and been redirected midway via a better route? Think about this show as your personalized navigation tool for fertility and IVF, as Dr. Aimee shares the five things you need to know about IVF. 1. Be Clear on Why You're Considering IVF. Ask yourself these questions. Why are you doing IVF? What are the fertility factors that are playing a role in your case? Is there something else you can do to improve your chances? 2. Realize Your Relationships May Change This is something I ask my patients to think about before, during, and after IVF. 3. Develop a Plan for Unused Embryos When you're going through IVF this is likely the last thing you want to think or talk about. When I meet patients I always ask them two questions. What do you think the problem is on why you're not getting pregnant? How many kids do you want? 4. Know That Pregnancy Isn't Always Easy When you've had a really hard struggle, most of us feel really guilty about complaining about pregnancy. But the reality is that pregnancy isn't always enjoyable for people. I like to think that all of my patients look gorgeous pregnant, but my patients don't always feel the same. For that reason, the depression or anxiety that people may have during treatment may carry over into pregnancy. It's also why I think it's really important to consider who's on your team. Who will be your support? 5. Understand that Your Feelings May Linger The feelings you had when you had a hard time getting pregnant can sometimes continue during pregnancy, after pregnancy, and beyond. Even if you've gone through IVF and it doesn't work the trauma of the treatment can continue. If you were successful with your IVF cycle you may still feel pain in your heart when you hear other people announce their pregnancies. Even if it's a close friend. You can be happy, but you may also carry with you painful memories of how difficult your journey was to get pregnant. This is okay. You can have conflicting emotions and experience them both at once. Talking about your fertility when you're old is not easy, but I want women to stop apologizing for desiring a family. Times have changed. It's okay to want a baby over 40. It's not crazy, nor selfish. Watch my show on this topic here. My approach is for all of my patients to be given their best chance at a healthy pregnancy in the shortest period of time. How do we do this? It starts by not believing everything you read. There's an international headline I saw recently on BBC news that fertility doctors are exploiting older women and "dealing false hope". Do I believe that fertility doctors deliver false hope? No way! I do not. Do I believe that it's imperative to get a diagnosis before treatment, regardless of who you are and your age? Yes! Diagnosis before treatment. Please! I want my patients over 40 to understand their follicle count. Then I want them to decide if moving forward with treatment using their own eggs is the best path forward. There are so many ways to have a baby if your own body is not making it happen there are other creative paths to explore. Egg and embryo donation and adoption are a few examples. Contrary to its name, having PCOS doesn't mean your ovaries have cysts, but rather that your ovaries have a high number of follicles containing eggs that are often unable to mature. Symptoms of the condition include irregular or absent periods, high testosterone, excessive hair growth (hirsutism), and acne. PCOS definitely has it's challenges, but, having it does not mean you'll never have children or that your eggs cannot produce a healthy, beautiful baby.
