Dr. Fabian Sandoval is the CEO & Research Director of the Emerson Clinical Research Institute and has over 25 years of bench to bedside research experience. His diversified research career has been in academia, healthcare systems and the public sector. Ep 103Democratizing Clinical Trials with Dana Dornsife of Lazarex Cancer Foundation

Dana Dornsife is a patient-advocate and the founder and CEO of Lazarex Cancer Foundation, a nationwide non-profit organization that seeks to improve the outcome of cancer care - giving hope, dignity, and life to advanced stage cancer patients and the medically underserved by matching patients to clinical trials and providing travel reimbursements to patients for the costs associated with FDA clinical trial participation. Dana founded Lazarex in 2006 after her brother-in-law was diagnosed with pancreatic cancer and she learned first hand of the many barriers patients encounter when trying to access potentially life saving treatment through clinical trials. A passionate patient advocate, she works to disrupt the system to advance much needed change. Under her leadership, the organization was involved in getting the U.S. Food and Drug Administration to publish new guidance language that makes it clear patients can be reimbursed for travel expenses to clinical trial sites. Through Lazarex, Dana also advocates extensively at the state level for laws that reinforce the FDA guidance. She has succeeded in getting new laws passed in 5 states and legislation is pending or being considered in several other states. In 2016, Dana expanded the mission of Lazarex with IMPACT (IMproving Patient Access to Cancer Clinical Trials), a 3-year nationwide program that works to increase cancer clinical trial enrollment, retention, minority participation and equitable access in the hopes of getting cancer drugs to market faster and finishing oncology clinical trials on time and on budget. Most recently, Lazarex is also working to create collaboration around a public health initiative in Philadelphia, PA – Community IMPACT. This bold effort is aimed at creating a replicable model to improve cancer health outcomes and associated co-morbidities, and address cancer health disparities for the residents of medically underserved and socioeconomically challenged minority neighborhoods throughout major metropolitan cities and beyond. Dana is a graduate of Drexel University in Philadelphia. She is a board member of the USC Brain and Creativity Institute at University of Southern California, serves as Chair of the UCSF Cancer Leadership Council and is a member of the MGH Presidents Council. She is Vice Chair of the governing board of the Yosemite Conservancy in addition to being a council member. Dana and her husband, David Dornsife, a USC Trustee, are also long-time supporters of environmentalism and sustainability and are the world’s top investors in clean water projects in Africa through World Vision, an international humanitarian agency supporting water, sanitation and hygiene in 24 African countries. Dr. Stefani Reinold is a board-certified psychiatrist, body image coach, women's mental health expert, and eating disorders specialist. She is the author of the bestselling book, Let Your Heart Out, podcast host of the show, It's Not About the Food, and Creator of the HEART Method: a self-led therapy tool helping with emotional self-awareness and resiliency. She went to medical school at University of Texas at San Antonio and did her residency at George Washington University. Speaking to patients about their weight can be challenging as this often emotionally charged and multifactorial. Our interview is really two separate interviews in one. In the first, we cover his podcast, where he interviews physicians on how they manage to continue finding fulfillment and enjoyment in their lives and practices in a world of crushing debt, wasteful administrative burdens, and so much clicking. He started this as a med student and continues with it in residency, so we also talk about how he has managed to get the approval from his institutions for this. This interview is one of my most important. If you are doing to share any of my episodes, this is one that I would implore you to share with your friends, family and colleagues. This is part 1 of 2 of my interviews with BJ Fogg, PhD, author of the book Tiny Habits: The Small Changes That Change Everything. We all struggle to change our behaviors, to develop good habits and stop bad habits. There is a lot of popular wisdom about this and most, if not all, is just wrong. This is where Dr. Fogg steps in. Dr. Fogg discovered the keys to changing behavior through changing habits. For those of you on medical school faculty, this should be a class. This should actually be taught in high school. Until then, as physicians, this information is critical, not just for lifestyle changes that can help patients eat better, move more, and smoke less, but even applies to checking their blood pressure and taking their medication. Popular wisdom is wrong. Guilt and shame are destructive. People don’t start habits by feeling badly, they start habits by feeling successful. And we are more likely to be successful by starting a habit that is small, that we actually want to do, and the third key to this is a prompt that reminds you it is time to perform the behavior. If you are going to learn piano, you start with chopsticks. If you are going to start to exercise, you do one sit-up. The smallest increment that you can fall back on when you motivation is waning so you don’t fall off the wagon completely and you keep your habit. And you do it at a point in your day that you can associate with the new behavior, even if they are completely unrelated. You’ll have a reminder that is baked into your day. Dr. Fogg founded the Behavior Design Lab at Stanford University. In addition to his research, Dr. Fogg teaches industry innovators how human behavior really works. She knew from the start that she wanted to be a nonclinical physician, but there weren’t many resources out there, so for others going through the same experience, she built her blog. It is not uncommon for us to have difficult conversations, like breaking bad news, discussing an adverse health outcome, but these conversations are about others. We need to have difficult conversations about ourselves. We need to reflect on our own biases and how these can affect the health outcomes of our minority patients and career trajectories of our minority colleagues. In light of current events, I have chosen to rerelease episodes from earlier this year that should be thought provoking and allow us to reflect on the ivory tower, conservative institution that is medicine and how we can help to move the needle to improve health outcomes for our minority patients and the careers of our minority colleagues. One way to improve the former, is to improve the latter. Max Jordan Nguemeni Tiako is a 5th year med student (out of 5) at the Yale School of Medicine and hosts his own podcast, Flip the Script, about healthcare disparities. He starts out discussing how some of what we learn about race being a risk factor for some diseases are a product of social constructs, not genetic predisposition. We then discuss racial disparities in substance-use disorder treatment. We then pivot from health disparities to physician training disparities. We learn about the hardships faced by minority medical students from microaggressions from students and faculty to disparities in grading and how this can affect career trajectory. He ends by discussing some pearls he has learned from his own podcast. Mr. Tiako grew up in Yaounde, Cameroon, and moved to the US to attend Howard University, got a BS in in civil and environmental engineering and then a Masters in Bioengineering from the Georgia Institute of Technology. He is currently doing research connecting his civil/environmental engineering background to identify elements of urban design that can be leveraged to improve health outcomes and move the needle towards health equity. His research fellowship is at the Center for Emergency Care & Policy research at the University of Pennsylvania. Dr. Uché Blackstock is an emergency medicine physician who is passionate about addressing the detrimental effects of structural racism on health outcomes. We discuss the origins of structural racism and how this continues to influence the health outcomes of minorities. She then gives us some tools for reflecting on our own biases and how we can work to address them. In addition to patient care, we talk about improving the diversity of faculty, and the importance of mentorship and sponsorship. We end by discussing something each of us can start doing tomorrow in order to address our own biases. Dr. Blackstock went to Harvard for both undergrad and medical school, did her emergency medicine residency at SUNY Downstate/ King’s County Hospital Center and then a fellowship in ultrasound at St. Luke’s Roosevelt. She recently left her position as associate professor at NYU as well as the faculty director of recruitment, retention and inclusion at the office of diversity affairs at the medical school to focus entirely on her company Advancing Health Equity, which aims to partner with healthcare organizations to address some of the critical factors that contribute to health inequity, through educational trainings and racial equity culture analytics. Passionate about investing wisely and teaching others to do the same, Jeff is a former contributor for The Motley Fool and current contributor for Seeking Alpha. Ep 74Addressing the Vaccine-Hesitant with Dr. Gretchen LaSalle

Gretchen LaSalle, MD FAAFP is a board-certified family physician with a passion for preventative care, from which vaccine advocacy is a natural extension. She is an active writer and public speaker on the topic and, in October 2019, published a book with Wolters Kluwer Press titled Let’s Talk Vaccines: A Clinician’s Guide to Addressing Vaccine Hesitancy and Saving Lives. In a growing climate of vaccine hesitancy, her primary mission is to keep patients from falling victim to vaccine misinformation and to decrease the frustrations, while increasing the successes, that clinicians have in getting patients vaccinated. We discuss how to have the most fruitful discussion with the vaccine hesitant. She has a system for addressing these patients and parents and no surprise, it starts with listening. The hesitation comes with all sorts of concerns, from stories they have heard, to potential side-effects, to simply indecision and she has a well thought out discussion for all of these. We get into the cognitive biases that may be at play and end on her experiences on social media with the vaccine averse, who are the vocal majority, but really the minority vs. someone who is simply hesitant. Dr. Kwadwo Kyeremanteng is the founder of the Resource Optimization Network and a critical care and palliative care physician. He is also the host of the Solving Healthcare Podcast. He is also Canadian, so we discuss the Canadian Healthcare System, which, on its surface, looks like a comprehensive, all encompassing, federally administered single-payor system, like what some of the democrats are discussing in the US. We discuss why this is not the case, and get into some of the details about what isn’t covered, how it is more of a provincial system and what are the differences in provinces. We discuss how the money flows through the system and how private insurance can play a role in some ancillary services. And true to the name of his podcast, at the end, we solve healthcare. Dr. Kyeremanteng was born and raised in Edmonton where he did his medical school at the University of Alberta. He and his wife then moved to Ottawa where he did his Internal Medicine residency training at the University of Ottawa followed by a two-year fellowship program in Palliative Care and Critical Care. He stayed after his training and is now an Assistant Professor in the Division of Palliative Care and Critical Care Medicine. He also has research positions with Ottawa Hospital Research Institute (OHRI) & Institut du-savoir Montfort as a Senior Clinician Investigator. Adam E. Block, PhD, is a recurring guest, having spoken previously about common misconceptions of the Affordable Care Act, and Medicare for All. Today we will be discussing the how erring on the side of an earlier end to shelter in place could have a worse impact on the economy than waiting longer. We also discuss what a return to work could possibly look like and when can we expect our elective patients to start coming back to the offices. In our industry and many others, it is important to be able to pivot and adapt to the new needs created by this virus. From a public health perspective, we also talk about when the risk of poverty as a social determinant of health starts to outweigh the risk of the coronavirus itself and what populations are at highest economic risk. Dr. Block is currently an Assistant Professor of Public Health at the School of Health Sciences and Practice at New York Medical College. He is a health economist with deep experience in the hospital, health plan and government sectors. His research is focuses on how individuals make decisions in health care markets including patient choice of hospitals, physicians and insurance plans. Prior to joining New York Medical College in 2017, has worked for a major hospital system, a large Medicaid managed care plan, spent several years developing the legislation on the Affordable Care Act as an economist at the Congressional Joint Committee on Taxation and later as Division Director of Health Plan Policy in the Center for Consumer Information and Insurance Oversight at CMS. In July, 2018 Dr. Block founded Charm Economics, LLC a translational economics consulting group. Ep 63Support for Physician Burnout Syndrome with Torie Sepah, MD

Torie Sepah, MD, completed her internship in family medicine at Kaiser Permanente Los Angeles Medical Center, then went on to complete a psychiatry residency at LA County + USC Medical Center. She has studied physician burnout syndrome extensively. It is a topic that is very personal to her and we discuss why. She defines the syndrome, what the major causes are, what the consequences are and what has actually been proven to help. Among them is connection. Her Facebook group – Physician to Physician: Healing the Practice of Medicine, has become a signal in the noise surrounding burnout, providing us with a community of physicians who understand the hardships of our field and can help. She has worked extensively in correctional medicine, having served as the chief psychiatrist at the California Institution for Women, being the first female chief psychiatrist at that prison. Since 2018, Dr. Sepah is a community psychiatrist once again, seeing patients with HIV in an integrated clinic and running her own interventional psychiatry clinic, which focuses on deep Transcranial Magnetic Stimulation (dTMS) as well as reproductive psychiatry, neuropsychiatric disorders, and early diagnoses or schizophrenia. Ep 62Discussing Flipping the Script on Health Disparities with Max Jordan Nguemeni Tiako

Max Jordan Nguemeni Tiako is a 4th year med student (out of 5) at the Yale School of Medicine and hosts his own podcast, Flip the Script, about healthcare disparities. He starts out discussing how some of what we learn about race being a risk factor for some diseases are a product of social constructs, not genetic predisposition. We then discuss racial disparities in substance-use disorder treatment. We then pivot from health disparities to physician training disparities. We learn about the hardships faced by minority medical students from microaggressions from students and faculty to disparities in grading and how this can affect career trajectory. He ends by discussing some pearls he has learned from his own podcast. Mr. Tiako grew up in Yaounde, Cameroon, and moved to the US to attend Howard University, got a BS in in civil and environmental engineering and then a Masters in Bioengineering from the Georgia Institute of Technology. He is currently doing research connecting his civil/environmental engineering background to identify elements of urban design that can be leveraged to improve health outcomes and move the needle towards health equity. Ep 61Healing People, Not Patients with Jonathan Weinkle, MD

Jonathan Weinkle, MD, is a general internist and general pediatrician who came to medicine after deciding against careers as a philosopher or a rabbi and is the author of Healing People, Not Patients. He starts off by teaching us how he was able to write a book about his patient interactions without violating HIPAA. His book illustrates the many ways in which it is important to really know our patients as people in order to effectively treat them. The paradox of this is that there never seems to be enough time to do this well, so Dr. Weinkle teaches us how to connect without losing efficiency. While cultural competency is usually discussed, Dr. Weinkle believes that cultural humility really the key – recognizing we don’t know what we don’t know about someone else’s culture. How to keep moving through a visit if you are really listening “with both ears,” but the patient continues to repeat themselves as if you aren’t listening. He also discusses the importance of allowing the patient to set the agenda and then establishing limits to that agenda at the start of the visit. He practices primary care medicine at the Squirrel Hill Health Center, a Federally Qualified Health Center and certified Patient Centered Medical Home, providing comprehensive care to patients of all ages with and without insurance and representing a broad diversity of languages, faiths, cultures, native lands, and socioeconomic backgrounds. In addition, Dr. Weinkle serves as a medical advisor to the Closure project of the Jewish Healthcare Foundation (JHF), a project intended to improve the quality of care and change the individual experience at end-of-life. Under the auspices of the JHF, he is crafting a program to help clinicians master the core competency of respectful communication with patients and families, based largely on the ideas in this book and the research underlying it. Finally, Dr. Weinkle serves as Clinical Assistant Professor in the Departments of Pediatrics and Family Medicine at his alma mater, and as Medical Director of the Physician Assistant Studies Program at Chatham University.
