Ep 197197 Human Trafficking: A Global Concern with Sakher Alqahtani
This week's A Tale of Two Hygienists is a heavier episode dealing with personal loss and worldwide turmoil, but it also offers a glimpse into a very necessary side of dentistry and examines a major issue affecting our world today. Sakher Alqahtani is a forensic and pediatric dentist who works at King Saud University in Saudi Arabia. He is also an honorary professor at the University of Queensland in Brisbane, Australia, where Andrew and Michelle got to speak with him. Sakher developed a new method of age estimation and lectures on that subject and human rights around the world. In this episode, Sakher discusses the important applications of forensic dentistry, breaks down the current refugee crisis and resultant human trafficking, and explains what you can do to get involved. EPISODE HIGHLIGHTS: Interview starts: 6:45 - Why forensic dentistry is relying less on dental records these days and more on social media like Instagram. - Sakher discusses one particularly tragic case where forensic dentistry played a role in identifying victims. - What is "Srebrenica syndrome" and how it applies to Sakher's forensic dentistry work. - Sakher breaks down the global refugee crisis, how age plays a major role in determining whether or not refugees can claim asylum, and where forensic dentistry figures into that. - Exploring the outrageous costs of detaining refugees at the border compared to giving them basic social assistance as their paperwork is processed. - How our modern concept of borders is creating an unnecessary crisis and hurts our growth as a society. - What you can do to get yourself informed about the refugee crisis and how dental professionals can assist patients who may be affected by human trafficking. - How you can get involved in forensic dentistry! QUOTES: "So when we do the identification it's for the living, because they cannot live a normal life otherwise." "The younger the person is, the more accurate age estimation is." "You have to protect that individual and people around them by applying the best method and by giving the best answer for that question." "You as taxpayers pay twenty times more for a person to be detained at the borders than if you let them in and give them money from social services... while their papers are processed." "This construct of borders and these harsh laws regarding people's movement is a new concept." "Diversity always enriches a society." You'll still learn something important. Andrew and Michelle had the honor of attending RDH Under One Roof, which took place July 31 through August 2nd this year in Grapevine, Texas. At RDH Under One Roof, members of the dental hygiene profession gather en masse to learn from top experts and keynote speakers, as well as check out the latest in dental hygiene tech. While they were there, our two hosts recorded an informal yet very informative roundtable episode featuring nearly 40 GUESTS. EPISODE HIGHLIGHTS: Interview starts: 3:52 - Don't be afraid to take on any challenges that come your way while in school. At the very least they provide ample learning opportunities. - Make sure to find the time to relax and find a social life for yourself outside of class. - While you're learning how to take care of other people's mouths, keep an eye on your own body and stay healthy. - Never burn bridges; you can never predict if you'll end up working someone you were rude or inconsiderate to. - Thanks to biofilm research, it's never been a more exciting time to enter the dental hygiene field. - Network whenever you can; the connections you make in school can shape your career for years to come. - Proper ergonomics when holding instruments will absolutely pay off in the long run. - Last but certainly not least: never give up. QUOTES: "Be open to any challenges or opportunities that lie your way." "Just follow your passions and you will do it. "Your patient is not a chart. Your patient is not a number. Your patient is a human being." "Really learn to advocate for your patients." "Your education does not end. It just began with hygiene school and the day that you graduated." "If you want something bad enough, go get it." "You can't clearly study if your mind is foggy. You'll get more clear when you do something nice for yourself." It's come to this: the final installment in A Tale of Two Hygienists' infection control series. To close things off, we've brought on a former law enforcement officer who can speak with authority about both the safety and legal ramifications of an unsanitary dental office. Duane Tinker is the CEO of Dental Compliance Specialists, which works with dental practices across the United States to ensure staff are well-trained in infection control and are working with the best and safest equipment. Known as "The Tooth Cop," courtesy of his previous line of work in Texas law enforcement, he's also the host of the Talking with the Tooth Cop podcast. And how Duane thinks the dental industry has improved overtime. QUOTES: "Before you can be a doer you have to be a dreamer." "There's so much marketing going on out there, you've gotta rise above the noise." "It's not any one person's fault but the reality is if there's a failure, it's everybody's fault." Congenital Muscular Dystrophy (CMD) is an inherited disease that atrophies skeletal muscles and results in poor development of motor skills and other modes of movement. While dental hygienists obviously don't treat CMD, they will nevertheless likely encounter patients who live with the disorder and consequently have special requirements in the dental chair. Matty Manley and Megan Meyer are the hosts of the Two Rare Mama Bears podcast, where they discuss CMD in all its forms, explore the difficulties faced by those who live with it, and speak to a variety of experts and policy-makers on the subject. Matty and Megan are themselves both mothers to children affected with congenital muscular dystrophy and also lead the CMD advocacy team. In this episode, Megan and Matty discuss their own highs and lows raising children who live with CMD, help us understand just how convoluted the diagnosis process can be, and explain how our offices and techniques should aim to accommodate for patients with CMD. QUOTES: "'Failure is not an option' came from us saying 'Okay, we have no idea what we're doing, but we have to make it work'." "We had one of the shortest diagnostic journeys—and it took us two years." "Dental care might be further down on their priority list because they're literally trying to stay alive." The Best Episode Ever with Leah and Kelsey from Two Therapists and a Thesis Podcast!
EA refreshing departure from our normal episodes, we meet up with Leah and Kelsey while in Brisbane for the ISDH. We begin with some Aussie slang and then briefly talk some dental before getting back off-topic, which I guess would technically be on topic as the fact that there was no topic is, in fact, the topic. We veer into other nonsense before returning back to dental again. It was quite the roller coaster of emotions with some dental discussion thrown in. **EXPLICIT** I put that warning up because there are a few mild swear words that sometimes turn into a string of swearing. Our sensitive audience should be made fully aware of it! I will NEVER apologize for this episode! Yes its a bigger/longer/and at times foul-mouthed episode, but there is soooo much to take away from here. Ep 189189 Martinis and Menopause with Kelli Swanson Jaecks
Menopause, that inevitable middle-aged milestone, can actually negatively impact oral health, which is pertinent not only to our patients but to mental dental professionals as well. To this end, we've invited on Kelli Jaecks, a dental hygienist based in Oregon as well as a professional speaker and author, who specifically focuses on women's health issues. Kelli was somewhat of a professional late-bloomer, not starting college until she was 30, and she's proof that it's never too late to find your calling and pursue it. Apart from her education as a hygienist, Kelli also earned her master's in communication and adult education and briefly served as president of the American Dental Hygienists Association (ADHA). Vaping is everywhere these days, to the point where it's difficult to go for a walk without spotting someone puffing from a vape pen of some kind. She specializes in implant dentistry and has familiarized herself with the dental risks of smoking and vaping. Intro to Orthodontics with Dr. Bryan Green
Andrew and Michelle are launching a new mini-series on specialty, this week is part 2 on orthodontics. At first glance, this topic doesn't seem to have a lot of apparent crossover with dental hygiene, but listeners will be surprised to learn how really does relate to the field. This is a must listen for dental teams and front office staff too! Bryan Green, DMD, is an orthodontist based out of Boiling Springs, SC. Bryan has been practicing orthodontics for the last decade, and prior to that worked in periodontology for three years. He believes that patient compliance is essential for braces to be effective and that hygienists are the first line of defense in treating orthodontic issues. In this episode, Bryan explains how his periodontology background has been a boon to his current orthodontic work, discusses why it's better to delay orthodontic treatment rather than start too early, and tells us if Invisalign is all it's cracked up to be. EPISODE HIGHLIGHTS: Interview starts: 4:15 - The added benefit Bryan believes a combined periodontology-orthodontic background brings to the table. - What kind of different residency programs are there for orthodontics and what can you learn from each? - The surprising longevity of orthodontics and why it's so important in dentistry. - Why wouldn't one pursue orthodontics? - The processes that cause tooth asymmetry and create a need for orthodontics. - How early should children be brought in to address orthodontic concerns? - The assessments Bryan makes during a patient's typical first orthodontic appointment. - The confusing that arises from dentists' and orthodontists' differing tooth numbering systems. - Why all the orthodontic treatment in the world won't matter if patients aren't willing to comply with your instructions. - The reason teenagers prove to be more difficult patients than children. - Weighing the merits and drawbacks of brackets vs. trays/aligners. - Do as-seen-on-TV aligners like Invisalign actually work? QUOTES: "I never understood why all dentists are not considered DDS's or DMDs... but at the end of the day it doesn't matter; you're all dentists." "Nobody ever died because of crooked teeth." "Hygienists are like our first line of defense." "I've never been sorry for waiting with treatment, but I've been sorry that I started too early." "If you want to be successful in orthodontics, you've got to have compliance." "The [kids] that are breaking brackets, they typically have poor hygiene." "Communication is the key." Ep 178178 Dental-Related Inpatient Hospitalization Trends with Dr. Josefine Wolfe
For this bonus episode—powered by Crest+Oral B—we're happy to invite back on our show Josefine Ortiz Wolfe, who's continuing our latest series on public health initiatives in the dental profession. Josefine Ortiz Wolfe, Ph.D., RDH, is the director of oral health at the Texas Health Institute. She also writes and educates online and in person as The Informed Hygienist. Recently, she and her team at THI completed a long-term study examining the treatment of non-traumatic dental conditions throughout the state of Texas over a ten-year period. In this special episode, Josefine discusses in detail the study she conducted with her colleagues, emphasizes the importance of systemic change throughout our field, and explains why you should always go to a dentist's office before a hospital for any dental issue. EPISODE HIGHLIGHTS: Interview starts: 1:51 - Josefine dives into the longitudinal study she conducted at the Texas Health Institute. - What is a non-traumatic dental condition? - Why it can be very dangerous for a patient to admit themselves to the wrong kind of medical facility for their condition. - How Josefine and her team were able to collate the information for their study. - The system issue Josefine discovered after they finished reviewing the data. - The shocking rise in cost of admission over the course of a decade. - How many people have actually died due to dental issues in this ten year period? - The importance of "system change" as opposed to simply improving treatment of individual patients. - Why hospitals aren't really set up to treat dental issues. - Is the dental profession beginning to respond to the conclusions of Josefine's study? - What Josefine hopes her report will achieve in the long term, and what she and her team have planned in the future. - Why she sees this report as an opportunity to tell a story about Texas, and why students listening should consider going into public health. QUOTES: "We're actually talking about those patients that actually showed up and their dental condition was so severe that the medical professional felt it necessary to immediately admit them into the hospital." "13% of Texans over the age of 65 have no teeth at all, so we know that directly affects their ability to manage other chronic diseases." "The problem is not just localized to one particular population. This is truly a systemic issue." "The majority of the conditions that folks were going into an in-patient setting for could have been treated at a dental office for a lot less than $37,000." "We really need to be getting these stories out into the hands of the public." "Behind all of these numbers, there are stories." "We have the opportunity here to explain how a healthy mouth will impact and improve every aspect of our lives and save lives." "I always say statistics are stories with the tears wiped away. Let's bring those tears back." Ep 177177 Student Focused Presents: Public Health
Our monthly student roundtable discussion episodes are dedicated to help you navigate the at-times turbulent waters from graduation to full employment in dental hygiene. For this installment, we're shining a light on hygienists who've devoted a significant part of their careers working in the public health and nonprofit sectors. Kyle Isaacs, who previously appeared in episode 87 of A Tale of Two Hygienists, has three decades' experience as a dental hygienist. She's spent the last five years working in the public health field as part of Miles 2 Smiles, which provides dental hygiene to patients who normally have difficulty accessing it. Amy Hazelwood has worked in dental hygiene for over 40 years and started the nonprofit Dr. Angie's Dental Health Exchange, which offers care to uninsured, unemployed and low-income patients. Jennifer Hasch graduated from her hygienist program six years ago and has worked both publicly and privately. Currently, she's the dental services manager for the Shawnee Christian Healthcare Centre in Louisville, KY, where she provides care to communities negatively impacted by segregation. In this episode, our enthusiastic roundtable guests discuss what drew them to professions in the public health sector, explain why public and nonprofit services are so vital, and make a case for why you should consider a career outside of private or corporate practice. EPISODE HIGHLIGHTS: Interview starts: 3:08 - What experiences and values motivated Kyle, Amy and Jennifer to work in public health. - How Amy's nonprofit uses community service to build credit for its dental care. - Are there any barriers preventing graduates from obtaining a position in public health services? - The hurdles Amy had to clear in order her own nonprofit, and the challenges graduates interested in nonprofit work should consider. - Jennifer defines what an F.Q.H.C. is, explains what services it provides and lays out the guidelines it must abide by. - Kyle relates her new role working in an integrative office and what she hopes to achieve there. - Which populations do each of their clinics specifically cater to? - The range of benefits graduates can expect from private, corporate and nonprofit practices. - Diving into the connections and experiences that volunteer work can provide. - Why you should give public health work genuine consideration as a career path. QUOTES: "When I graduated from hygiene school I went right away to that same place where I had gotten care as a child and volunteered." "I went into public health because of the need that I saw." "I was fortunate enough to go to the dentist every six months as a kid, so I didn't really connect that people could suffer from a lack of access to care." "It's kind of one-stop shop for health." "[F.Q.H.C.] gives folks a stake in their own health care, which I think is important." "It's gonna take just as much time to correct these issues as it did to create them but we're definitely committed for the long haul." "I feel like I'm very well paid for the work that I do and it's definitely a rewarding position." "Do not underestimate the potential of your impacts in this career." QUOTES: "A lot of hygienists can be more than tooth-scrapers." "Our traditional way of looking at caries is through visual inspection and tactile sense." "If it sticks, we're treating it. It's as simple as that." "If there's something I feel is not necessary to treat, I make sure I let the patient know the why of not wanting to treat it." QUOTES: "I can't express enough that we have to get patients to own their problems." "Whatever is something that is doable for you—every single patient, every single appointment—I say just do it." "If you are using arginine as part of your protocols and you're having amazing luck with, I say do it!" Ep 173173 Local and Topical Anesthetics in Dentistry with Tom Viola R.Ph
For this bonus episode—powered by Crest+Oral B—we're happy to invite back Tom Viola, R.Ph who appeared in another bonus episode earlier this January. Tom Viola is a speaker, writer, consultant, and expert in the field of dental pharmacology. When it comes to the anesthetics that keep dental patients from feeling pain during even the most invasive procedures, Tom is THE go-to guy. From Benzocaine to Xylocaine to good old fashioned (if out of fashion) Novocaine, he has the word on dosage, acidity, and side-effects. In this special episode, Tom discusses the wide variety of anesthetics used in dentistry, the simple tool you can use to prevent complications, and why everything changes the moment your patient is a child. EPISODE HIGHLIGHTS: Interview starts: 1:50 - Tom explains why Novocaine fell out of use as a standard anesthetic in dentistry. - A breakdown of some of the most commonly used topical anesthetics. - How do you choose which anesthetic you use with each patient? - The hazards of mixing anesthetics with acids. - Which is the best topical compound? - Figuring out which type of local anesthetic you should use for a given situation. - Sulphur, sulfa and sulfite: don't confuse the three, especially when it comes to allergic reactions. - Why needle trauma can play as much of a role in paresthesia as anesthetic does. - Basically everything we know about anesthetics changes when administering to children. - What can go wrong when administering anesthesia and how can we prepare for it? QUOTES: "We're applying a highly acidic gel to the patient's already inflamed and potentially infected mucosa. What kind of sadist would do that? Well it would be the entire profession of dentistry." "We can do a lot to allay that discomfort by just using a different topical if they're truly feeling that benzocaine burn." "I believe patients get more epinephrine sitting in a traffic jam, waiting to get to the office than they get in the office." "If there was an anesthetic that dared to be different it would be Carbocaine." Ep 172172 Caries Detection and Etiology with Dr. Pam Maragliano-Muniz!
Interview starts minute 7:55 Cariology Series Part ONE Be sure to buckle up because the next few weeks of A Tale of Two Hygienists Podcast are going to be a rollercoaster of knowledge. This is the first episode in our new cariology series. Dental caries are among the most pervasive oral ailments and their treatment is core to dentistry and dental hygiene, so we've invited a few seasoned professionals who have insight into how they come to be and how we can best treat them. Our first guest in this series is Pam Maragliano-Muniz. Pam is the Editor-in-Chief of Inside Dental Hygiene, the owner of Salem Dental Arts, an experienced prosthodontist and a hygienist even before that. We also previously had on for Episode 92! Her years in the dental field have honed her insight into the natural and unnatural ways teeth break down. In this episode, Pam discusses the primary risk factors leading to dental caries, explains how hygienists themselves can cause lesions by accident, and breaks down saliva's important contribution to keeping gums, teeth and tongues in good shape. EPISODE HIGHLIGHTS: Interview starts: - How exactly dental caries occur? What are the three major risk factors? - Why orthodontics and prosthodontics play a role in the proliferation of tooth decay. - The reason why patients with even partial dentures should make more frequent hygiene appointments. - Why hygienists should stock up on articulating paper for their offices. - How improperly contoured teeth can cause lesions in the long term. - Saliva's essential role in maintaining oral health and what happens when its effects are neutered. - Weed may be legal where you live, but it still impacts your oral health. - Exploring different categories of higher-risk patients. - Which tools are effective at clearly communicating tooth plaque to layperson patients. QUOTES: "If I don't really understand what's going on here, how am I seriously going to be able to help this patient prevent recurrence of disease?" "We really need to think about the environment in which these teeth live." "Saliva really does it all!" "The role of the hygienist when it comes to preventing tooth decay is so important."
