When seeing patients, I always recommend people to gather their Fertility TEAM, and the T is for therapist. I'm so excited to have Dr. Loree Johnson share her valuable info with all of you today. In this conversation, we're going to be talking about some of the mental health impacts of being a fertility patient. I'll be asking Dr. Johnson about the difference between Infertility Stress and Infertility Trauma, we're going to talk about grief as it relates to fertility (and how it's different than other grief), and she'll share how to navigate the stress of being pregnant after having loss. Dr. Loree Johnson is a Licensed Marriage and Family Therapist (LMFT) with more than 25 years of experience as a clinician, educator, and clinical supervisor. After her first-hand experience with fertility challenges taught her the importance of self-care for managing the physical and emotional stress that come with the fertility process, coupled with the lack of representation she saw of therapists of color in the fertility space, Dr. Loree found her purpose supporting individuals and couples on their path to parenthood. S12 Ep 92Top Reasons You're Having a Hard Time Getting Pregnant
There are many things that can impact your ability to become pregnant. In today's episode, I'm reviewing the top 10 things to consider if you are TTC. 1. Environmental Toxins. Avoid microwaving food and plastic containers or bottles. Always store food and glass or stainless steel containers. Million Marker offers a test to find out if you have in-body toxicity. 2. Chemicals in food and everyday products. Verify that the products you are using are chemical free. You can do that on https://www.ewg.org/ 3. Being sedentary. The CDC and American College of OBGYN, both recommend exercise in amounts of 150 minutes per week. Avoid over exercising, as low BMI can also cause fertility issues. 4. Food allergies or intolerances. Work with a nutritionist to find out if you have a food allergy or other food intolerance. You may also want to consider working with a specialist if you have PCOS or using Pollie.co to help you manage PCOS if you have it. 5. Blocked Fallopian Tubes. Check with your doctor to find out if this is what may be causing you to have a hard time TTC. 6. THC and Cannabinoids can affect the motility of the fallopian tube and the receptors in the lining of the uterus. This makes embryo implantation harder and it might not happen as quickly as you would want. 7. Nicotine. Nicotine can affect the DNA and the egg in the sperm cells, and it can also affect a woman's fertility such that she could go potentially into menopause earlier. 8. Blaming birth control pills. Many people think that birth control pills "cause" infertility, but the truth is that because they regulate your period they may be masking the indicators of infertility. Make sure you get your hormone levels checked with your doctor every year to confirm your levels. 9. Ignoring biology. Every woman is born with a finite number of eggs, and if you want to wait to have kids until later in life, be sure and freeze your eggs when you are in your 20s. 10. Not getting enough sleep. S12 Ep 91How to Talk to Your Doctor About Fertility if You Are Over 40
Talking about your fertility when you're over 40 is not easy, but I want women to stop apologizing for desiring a family. Times have changed. It's okay to want a baby over 40. It's not crazy, nor selfish. My approach is for all of my patients to be given their best chance at a healthy pregnancy in the shortest period of time. How do we do this? It starts by not believing everything you read. There's an international headline I saw recently on BBC news that fertility doctors are exploiting older women and "dealing false hope". Do I believe that fertility doctors deliver false hope? No way! I do not. Do I believe that it's imperative to get a diagnosis before treatment, regardless of who you are and your age? Yes! Diagnosis before treatment. Please! I want my patients over 40 to understand their follicle count. Then I want them to decide if moving forward with treatment using their own eggs is the best path forward. There are so many ways to have a baby if your own body is not making it happen there are other creative paths to explore. Egg and embryo donation and adoption are a few examples. S12 Ep 88Using the SpermQT Test to Complete Your Path to Fertility with guest Dr. Kristin Brogaard
Today, Dr. Kristin Brogaard is joining me today to talk about the Path Fertility SpermQT test. I'm so excited to have her share about the test, because as you'll hear her say "lots of infertility goes undiagnosed because men go undiagnosed, and when men go undiagnosed, women go through procedures that will not work." (Who else wants to see this on a bumper sticker?) While sperm were long considered to be just carriers of DNA, it turns out they are a lot more complex than that! Understanding the sperm side of the equation can help anyone trying to conceive better understand what's going on, and help them figure out the best plan to get pregnant. A few exciting highlights from our discussion: Male and sperm epigenetics play a big role in getting pregnant, and having a successful live birth. The SpermQT test can tell someone if their sperm is excellent, average, or poor quality. - Knowing this information can help you and your doctor determine the most efficient and appropriate next steps in your treatment or protocols. Understanding the quality of the sperm can lead to better outcomes. If it is "excellent," there is an increased likelihood of IUI working, even in cases where doctors thought sperm might be the issue with getting pregnant. If sperm quality is "poor," you may want to try making lifestyle changes to improve sperm quality, and retest. - Poor sperm quality often correlates with other longer term health issues for men, as sperm acts as a "canary in the coal mine." (an early warning system) Dr. Kristin Brogaard has a PhD from Northwestern University where she developed a novel epigenetic technology that allowed for a more accurate understanding of gene expression in health and disease cells. She is now the chief scientific officer of Path Fertility, whose goal is to raise the standard in male reproductive health, starting with male infertility. S12 Ep 87Does Sperm Really Matter?
Does sperm really matter? If you've been tuning in to all of the posts this week for "Super BALLS Week" you'll know that of course it matters! It truly does take both sperm and an egg to make a baby. Today I'll review the sperm question a bit, and explain why it matters. AND, I'll give you a lot more info about how to improve sperm quality. Here's the thing: I know that guys want to hear what they want to hear. They have selective listening. I'm not kidding when I tell you that I had someone email me recently and say "My husband's sperm tested out GREAT, and when we went over the results with the doctor we heard two different things. He thinks he heard our doctor say that it means he can just keep drinking, smoking, and enjoying marijuana and doesn't need to make any lifestyle changes. I heard something very different, which was that he should still cut back until we are pregnant. Can you help settle this for us?" And while it's an unpopular answer (at least with the guys), my response is: Men should cut back or abstain from drinking, and smoking (marijuana too) if you're TTC. Here are a few of the things I suggest to anyone bringing the sperm side of the equation to the baby making situation: - consider freezing your own sperm for future use when you are young. - get in the best shape of your life if you're trying to conceive. Sperm matters, and men's health is directly related to their sperm quality. - take some prenatal vitamins. Yes, really. Many of the same supplements that I recommend for egg quality also are great for sperm quality. - cut back on smoking, drinking, and drug use. Marijuana negatively impacts sperm. - don't cook your balls: avoid hot tubs, using your laptop on your lap, and other situations that may create high temps for the area. - find ways to cut back on stress, if you live a stressful life. I'm delighted to have Dr. Ramy Ghayda join me today to talk about sperm, and the importance of sperm DNA Fragmentation testing and how to improve sperm health. When trying to conceive it takes both an egg and sperm cell. And, as you'll hear Dr. Ghayda explain, we now have access to more tests for male factor infertility, and they are more affordable and accessible than ever. He and I both believe it's very important for people, and fertility patients (everyone, really!) to know their options and to be educated about what they can do to improve sperm quality, to test sperm quality. S12 Ep 77A Genetic Counselor Explains Genetics & Hereditary Disease for Fertility Patients with Dena Goldberg
Your Genetics are a key part of my TUSHY Method (Y = Your genetics) and I'm so honored to have Dena Goldberg (also known far and wide as Dena DNA) joining me today! Some of the questions I know you have on your mind about genetics are: Nothing came back on my 23 & Me test. I'm good to go, right? Answer: Not so fast. 23&Me relies on "SNP testing," and only looks at "very specific bookmarks on very random genes." You'll want a more thorough test for preconception screening. I don't have any history of disease in my family. Do I need to do genetic testing? Answer: It's a good idea. There are syndromes such as Lynch Syndrome that are asymptomatic in 90% of people who are carriers. You'll want to know if you have those genetic markers before passing them on to your embryo and child. Can you make sure my embryo doesn't have autism? Answer: Autism is actually a description of a constellation of symptoms. So it's not an underlying condition in and of itself. There are hundreds to thousands of genetic causes to autism. We can catch some of these cases, but not all. Autism is multifactorial, meaning that many of these cases are probably caused by a combination of genetics and environment or in other words, nature and nurture together. When you're doing IVF, it's important to be prepared. I want you to have the best outcome possible, and for me that includes making sure you have everything you need on hand to support your physical, mental, and emotional health as you go through the process. Today on the podcast, I'm sharing my top 10 things you need when you're embarking on IVF: 1. Get your electrolyte rich drinks ahead of time 2. Stock up on a protein powder+shake that you love 3. Consider scheduling a food delivery service 4. Prepare for a poop emergency: stool softeners are your friend 5. Purchase panty liners because you can spot and that's considered normal after an egg retrieval and waterproof underwear helps with the discharge too 6. Egg Whisperer fertility pants 7. Mostly, I throw them because I want every person to be empowered with knowledge about their fertility. It's really as simple as that. Egg Freezing parties are safe and surgery-free gatherings of women (and sometimes men) to learn more about the process of freezing your eggs (or sperm). Beyond that, we talk in general about fertility and how every woman (and man) can take a proactive role in preserving theirs. I started Egg Freezing parties for a simple reason. Older women were in my office struggling with fertility only to wish they had known more when they were younger. Had someone told them to get their fertility levels checked and to consider the option of preserving their eggs they most certainly would have. Egg freezing is IVF without the F. You go through a standard cycle of In Vitro Fertilization in which you take hormone shots that stimulate the growth of eggs. A doctor like me then extracts your eggs through a simple surgical procedure, and I work with an embryologist to preserve your eggs in liquid nitrogen until you're ready to use them. So the big question is what happens now that you're ready to use your eggs? In other words, what are the steps towards fertilization (or as I like to say, what the F?) Step 1. This week, I'm thrilled to (re)introduce The TUSHY Method - 5 essential tests to have done when you're seeking a diagnosis. What does TUSHY stand for? T is for Tubes–as in your fallopian tubes. First, you need to determine whether the fallopian tubes are open. U is for Uterus. I recommend doing an ultrasound of your uterus to figure out if there are any anatomic issues affecting your ability to get pregnant. S is for Sperm, because every pregnancy is a two-part gig! Learning about your partner's or donor's sperm health is essential. H is for Hormones. I recommend testing for three different critical hormone levels to understand your fertility in a more personal way. Y is for Your genetic profile. Dr. Geoffrey Sher is joining me again, and this time we are going to talk about implantation failure during IVF. Dr. Sher is co-founder of Sher Fertility Solutions (SFS) and an internationally renowned expert in the field of assisted reproductive technology, and influential in the births of more than 17,000 IVF babies. One day I hope that I'll be able to say the same thing about myself. Over the last 30 years, he has helped fashion the entire field of ART after training under the fathers of IVF, Dr. Patrick Steptoe and Robert Edwards. He established the first private IVF program in the United States in 1982, then later expanded his practice to include several centers throughout California before founding the first Sher Fertility Solutions in Las Vegas. He's also the author of several books, In Vitro Fertilization: The ART of Making Babies, which is now in its third edition. In this episode, we're talking about implantation failure during IVF. Many times I wonder why embryos do not stick and grow. Dr. Sher is discussing the role of the endometrium, the difference between implantation and miscarriage, and the three factors he considers when advising on treatment for implantation failure. S12 Ep 52Implantation and The Two Week Wait
Discover all the answers to some of the most common questions you may have about your menstrual cycle, hormones in your menstrual cycle, ovulation, implantation, and the 2ww (two week wait). If you are trying to get pregnant, you may have heard the term—two week wait, the often nervous time of waiting between ovulation and taking a pregnancy test. For many, this is a dreaded time because it's hard to know exactly what is happening. But by learning more about your cycle, you can have more knowledge about what might happen. The implantation window is during the two week wait. Since progesterone is the hormone responsible for supporting implantation, it must be present during the entire implantation window. You can track your progesterone during this window by tracking PdG, progesterone in urine. Proov has unique testing capabilities to test PdG during the implantation window to confirm successful ovulation. Studies show PdG levels are incredibly important for implantation and pregnancy success, for example, women with high PdG during the implantation window were 75% more likely to get pregnant. In contrast, women with low PdG had a miscarriage rate of 90%. Fertility patients know that treatment feels like being on a roller coaster. It's a time that can be full of optimism and disappointment, and because it can feel isolating, I include "Therapist" in my foundations of building your own Fertility TEAM (Therapist, Eating healthy and Exercise, Acupuncture, Mindfulness and Meditation are the basics). I love that Dr. Glezer explains how to approach the whole picture of mental health: getting support, taking good care of yourself, getting enough sleep, finding someone to talk to, eating well, doing some physical activity, and finding an integrative psychologist to work with if that feels right.
