The Transition journey for children and young people from child to adult health services is a hot topic in healthcare. Coinciding with the upcoming launch of the National Capability Competency Framework for Transition, in this latest episode, we talk to Louise Porter, National Lead Nurse from the Burdett National Transition Nursing Network, and Rachel Naunton, Lead Practice Facilitator of the Me first project, about the importance of communication training in developing and improving Transition services. They discuss the launch of an exciting new training module they have co-produced with young people, parents and a variety of professionals, that aims to support health care staff to have effective conversations about transition.
