On October 21, 2021, cinematographer Halyna Hutchins was killed on the set of the Western film Rust when a gun held by actor Alec Baldwin discharged during a rehearsal. The bullet struck Halyna in the chest and also injured director Joel Souza. She was rushed to the hospital but died from her injuries. She was just 42 years old.
The tragedy shocked the entertainment industry and raised urgent questions: How did a live round end up in a prop gun? Who was responsible for checking the weapon? And why were there so many safety violations on set?
