It seems unpopular these days to tell people the truth about the health risks associated with being overweight. Lying to people and/or avoiding uncomfortable conversations is not going to help anyone. People need to hear the truth. Tell them the truth. Tell them with empathy & compassion, but tell them the truth or their health is in serious jeopardy.
