Ep 60#60: Compassion & Courage in Covid
Drs. Kirk and Kimberly Milhoan are an amazing physician couple with a unique combination of expertise in Pediatric Cardiology and Pediatric Anesthesia. They have experienced what happens when speaking out against the status quo on Covid therapeutics , and what that cost is. Kirk tells of how he became subject to investigation by the Hawaii Board of Medicine for a conversation he had regarding Covid therapies. Throughout the pandemic they began to question the evolving push of vaccines over outpatient therapies. Kirk also has concerns with what is happening to children’s cardiac health as a result of using an emergency use injection therapy in our young, with no long term data. They have been actively involved in Global Covid Summit and proponents of the sanctity of the physician-patient relationship, recognizing natural immunity from Covid and avoidance of doing more harm than good with Covid mRNA technologies. Kirk Milhoan has a BA in Biology and Chemistry and a PhD in Cardiovascular Physiology and Pharmacology, focusing on cellular mechanisms of cardiac inflammation, specifically the role of platelet activating factor in activating the myocardial inflammation process. He received his medical degree, Magna Cum Laude, from the Jefferson Medical College of the Thomas Jefferson University. He completed his pediatric residency at David Grant Medical Center at Travis Air Force Base and pediatric cardiology fellowship at San Diego Children’s Hospital/UCSD. He is a Diplomate of the American Board of Pediatrics in Pediatrics and Pediatric Cardiology, and a Fellow of the American College of Cardiology and American Academy of Pediatrics. He served thirteen years in the United States Air Force as a pediatric cardiologist and flight surgeon. Kim is a graduate of Stanford University, with a B.A. in Human Biology, with a concentration in Public Health and Health Administration. She worked as a health care management consultant for American Practice Management before obtaining her medical degree. She completed her anesthesiology residency, as chief resident, at the University of Health Science Center in San Antonio. She is a Diplomate of the American Board of Anesthesiology in Anesthesiology and Pediatric Anesthesiology, and a Fellow of the American Society of Anesthesiology, with a specialty in Pediatric Anesthesia. She and Kirk co-founded For Hearts and Souls, a non-profit specializing in the international care of children with congenital heart disease. Kirk serves as medical director and diagnostic cardiologist for the organization, and they have voluntarily served the needs of tens of thousands of children with heart defects throughout the world. The organization has performed over 700 invasive pediatric cardiac procedures internationally. Kirk is also the Senior Pastor of Calvary Chapel South Maui. He and his wife have treated many patients with COVID-19 through their federally recognized free mobile medical clinic in Maui. We are long overdue for another Doctor As Patient episode. I decided it would be perfect for me to share my own personal story and journey with migraine. Having been a lifelong migraine patient, it has impacted my life since I was 11 years old. I have tried to never let it define me, but living with a chronic illness certainly controls many aspects of your life. I have tried almost every therapeutic option/treatment and now am blessed to have found a game changing new medication that has given me my life back! I have a slight obsession with a trainer I follow on Facebook and Instagram. I’ve been following Cori Lefkowith of REDEFINING STRENGTH for years, enjoying her motivational posts and loving her targeted exercises for my troublesome injuries. She has great rehab advice that I have tried and tested on my numerous Ortho injuries. She has helped me when many PT’s couldn’t even crack the mystery. She posted something a few weeks ago that blew my mind: “Most people fail because of how they try to achieve their goals.” She went on to write that the date at which most people completely quit on their New Year’s Resolutions...is…..?? It’s not the Summer...Or a specific holiday or event… It’s not the middle of February which was my guess. It’s JANUARY 19th! (This date is based on a study done of over 800 million people’s activities in 2019) Which led me to thinking and asking WHY do people quit so early? What are the stumbling blocks that cause us to give up? How can physicians and other healthcare experts help motivate their patients or themselves even….to realize their fitness and weight loss goals? Last week the Supreme Court blocked the OSHA vaccine mandate for large businesses. While this is a huge ruling, how exactly did this overturn and how was it decided? During this pandemic we have seen decisions being made based on doctrine. Do administrative agencies have the authority to implement medical mandates based on their interpretation of science? To help us unpack what happened and where we might be headed with this ruling is our guest, attorney Ryan Heath. He does an amazing job explaining how OSHA had over reached their authority and power as interpreted by the Supreme Court. Remember that currently healthcare facilities that operate under CMS ( Center for Medicare Services) still need to comply with vaccine mandates and this is a completely separate path that will play out differently. You’ve heard all the jokes- what’s your poo telling you?, eat sh&t and die, you’re full of sh&t. Today we are going in deep and discussing how “eating” poo can actually save people! ( no they don’t really have to eat it, but it makes a great title!) Fecal microbiota transplant ( FMT) aka a stool transplant, is the process of transferring stool from a healthy patient into another individual. Currently in the USA, the only FDA approved use of this treatment is for C. difficile infections. It has been used experimentally to treat other GI diseases such as IBS, colitis, and even some neurological conditions like MS, Parkinsons and even autism. What is even more amazing, is that the 10,000 plus species of microorganisms in the GI tract make up a unique “microbiome” in each individual. There is huge diversity in the gut microbiome depending on diet, genetics, environment and overall health of the individual. Research is exploring the potential curative role of gut bacteria in various diseases. What CAN a physician do other than prescribe your medications, perform exams on every orifice you have and cut and paste you back together with knives, nukes and robotics? Guess what? With the continuous corporatization of American healthcare, physicians have been losing their voice, power and have not figured out a successful way to keep their power in advocating for the patients best interest. As I have often told patients, I feel like I have as much power as the Walmart checker does in their corporation. Our guest today is Joe Crane. A longtime member of the labor movement and union organizer of nearly two decades, he helps doctors reclaim their profession as the Doctors Council's National Organizing Director. Joe has been working with and organizing doctors for over four years. Most notably, he successfully negotiated several physician collective bargaining agreements and took a group of physicians on the picket line in Washington state's first ever physician strike. The Doctors Council is the country's oldest and largest union of attending physicians. With members spanning four states and both public and private sector workplaces, the Doctors Council is proud to be a united voice for doctors, their patients, and the communities they serve. This episode is in honor of World Mental Health Day (and because my bestie k.d asked a long time ago for this) It’s really hard to do justice to a topic in 40 minutes that is so vast and layered. The mental health crisis in the US has fallen on deaf ears for years. Now with Covid bringing emotional stress, healthcare burnout, social isolation, disruption of societal norms- well it’s no wonder the whole world hasn’t literally gone mad. Ep 39#39: Follow the Science to Hell
What does it really mean when we say follow the science? Believe it or not, this week is a Doctales with Cocktails episode- albeit a more sobering discussion over a fine, local whiskey. What a perfect episode to ponder over a tumbler of. Blue Line Unapologetic American Whiskey! This week we discuss some historical things done in the name of science. Specifically we dive into Eugenics, the science of “the day” for over 50 years. It became accepted by the scientific community and world elites, intellectuals and political leaders including Alexander Graham Bell, Winston Churchill, John Maynard Keynes, and Woodrow Wilson, to more notably Margaret Sanger and Adolf Hitler. The concept born in the late 1800’s believed that selective breeding of humans would breed away undesirable traits that contributed to a weaker, unhealthy society: feeblemindedness, criminality, birth defects and mental health issues for example. If you were in the ‘ELITE’ crowd, you were a Eugenicist. Fast forward to current societal and scientific struggles. What concepts or “group think” exist? Is following “the science” today’s pandemic the same thing? What about the labels associated with being anti/ pro vax, anti/pro climate change? We contend that while there are often many scientific truths and facts in these principles, we can get caught up in the debate without removing our own bias. What happens next is “the science” becomes political, and for many their politics is a natural extension into becoming their religion. Is Covid all about science or “scientism”? We believe there should be healthy debate which means allowing scientists to challenge one another to reach the truth. Shutting down opposing views and censorship only pushes the agenda of unilateral thinking. Ultimately those who only listen to one side are believers in dogma, not science. At the time of this interview, rates of myocarditis in healthy young adults are now being correlated with the vaccine.The CDC confirmed at least 226 confirmed cases of myocarditis, have been reported in people under 30 who got the Pfizer or Moderna vaccines. He received his bachelor’s degree in epidemiology from Toronto, Ontario, a master’s degree from Oxford University, and a PhD from McMaster University’s Department of Health Research Methods, Evidence, and Impact. He has also studied at Johns’ Hopkins University. He has studied bioterrorism, advised the Canadian Government and also worked with WHO in 2008 until recently. From 2017 until December 2019, Alexander was employed by the Washington, D.C.-based Infectious Diseases Society of America (IDSA), where he specialized in systematic reviews. At IDSA, Alexander worked on several clinical practice guidelines. He served in an advisory role with Health and Human Services (HHS) under President Trump in 2020. This week we are talking about “drugs”, specifically the world of PBMs with Dr. Lee Strandberg. Pharmacy Benefit Managers(PBM) are the middlemen between your pharmacist and your prescription and your insurance company. Yep.. more middlemen. If you have never heard of the term PBM, well you are not alone. Most people have no clue that they exist, how they work and how the cost or medications is controlled, negotiated or paid for. They are the one paying the pharmacist for your medication! While there are approximately 60 PBS, the 3 largest ones currently control 80% of the market. We discuss both arms of the pharmacy world- how the pharmacy acquires the medications and purchases them. Hang on as we unravel the mysterious chain of events on how the pharmacy gets paid by the PBM and how they negotiate the prices with your insurance company. We do get a little deep in the weeds on cost control and price negotiations, so pay attention. We only WISH that getting a Zpack could be as simple as buying a head of lettuce from your local farmers market. Dr. Lee Strandberg is our guest this week who intimately knows the ins and outs of the complex layers of the pharmacy world. He is a Professor Emeritus, Pharmacy Economics and Public Health Oregon State University and was Director, Managed Care pharmacy for SHS Plans from 2005-2011. He was appointed by Oregon Governor Kitzhaber to the OR Health Policy Board, Medicare-Medicaid Integration of Care and Services work Group which wrote part of the CCO and reporting structure. He was a committee member on the OR Evidence Based Review of Prescription Drugs Preferred Drug List. During the past 40 years he has presented papers, seminars and consulted on the impact of pharmaceutical use on health care costs and quality. Ep 19#19: Sinking the Covid Needle or Not? (Part 2 with Dr. Peter McCullough)
Hang on to your seats, this is a long one! This week is a continuation from our podcast last week with Dr. Peter McCullough regarding Covid-19! Last week we discussed evidence based treatments for Covid-19 , best practices to avoid hospitalizations and death plus what resources we have. This week is all about the vaccines, safety, adverse events and RISK!. Plus is there a bigger theme in all of this? It feels like we are fighting a war not against Covid, but against each other. With opposing sides in the public and medicine, to vaccinate or not to vaccinate; who , what, when and why IS the question! We used to think asymptomatic spread was frequent and now we are learning otherwise. New evidence from published data available from tracking over 10 million people in China shows there is a less than 1% spread from asymptomatic people. The evidence seems to show we need to “Stop” testing asymptomatic well people! What we are doing in public health now seems wasteful, expensive. Why are we now still having to wear masks if we are vaccinated if it’s fully safe and effective as promoted? Next we look at extreme differences between the states handling the pandemic response from Michigan to Texas. Taking it further, look up north to the border to home and native land in Canada. The border is closed, Covid hotels are mandatory for noncitizens entering for ANY reason, with no regard to their health, immunity or current vaccination status. Draconian or Totalitarianism? You won’t believe it, but it’s TRUE! Johns Hopkins published a booklet in 2017 referencing THE SPARS PROJECT. It was a “response” document and plan to deal with the next pandemic , predicting even the exact genus and species that would hit the world in the near future. You can read how they had already mapped out how to handle, shut down and analyze a global crisis. Next we get into how the response to Covid-19 feels to be weaponized. Schools, businesses, churches, and countries shut down and often with massive fines for not adhering to authority. Is this really The GREAT RESET? We move on to discuss how the vaccine initially was promoted as a “volunteer” basis to protect those at risk. Now we are seeing the implementation of financial coercement, Universities mandating it, talk of vaccine passports, business threatening to fire employees who are unvaccinated. What!! While Dr. McCullough is pro- vaccine, he definitely believes that Covid-19 vaccination may not be appropriate for everybody. Covid survivors, potential childbearing women and pregnant women were ALL excluded from the trials, why would we expect them to blindly get the vaccine without any data? This is a risk too big to ask for many people and one that is naively endorsed by many. The CDC is very clear on their statement regarding who should get the vaccine : ask your doctor if the vaccine is right for you. They DON'T GET TO DECIDE! It is a discussion and decision between the physician and patient. No other vaccine( flu, pneumococcal, shingles, etc) has been pushed this hard to the masses in the history of vaccines. To date there are 3514 deaths associated with the vaccine: anaphylaxis , reactogenic deaths (from blood clots, high fevers, etc) and then thrombotic events that occur farther out in time. The numbers for vaccine related adverse events is over 40,000 per 100 million. Something to consider: Is Johnson and Johnson the safest vaccine because it is only ONE vaccine? Finally: The proposition of vaccinating healthy low risk people to protect the population at large is fraught with moral implications. There WILL be college kids who die due to the rare event from vaccine induced causes .. from being coerced into the vaccine because of pressure from the masses. How do we assess risk, benefit and sort through the data………….. What an honor to have this intelligent discussion and input from a world renowned and highly published physician. So many times at work I have been shocked when patients find out I had the flu, had surgery or experienced a “real medical” problem. No we aren’t superheroes, and we DO share your pain. This week we have the privilege to hear from Dr. Manisha Rayavarapu. She is a Family Medicine physician, diagnosed with Lupus during her 2nd year of residency training. She describes how her faith along with her husband, have been the most important support on her journey. She shares her struggles with navigating the health system ,uncompassionate colleagues and administration, to family who may not understand how her illness affects her life. She has gone on to write a book on her experiences, struggles, fears and strength on coping with her ‘new normal’. Her memoir, “Butterflies, Boards and Blessings” is her way of helping educate and raise awareness about lupus.
