In this two-part episode, presenters from the 2021 ASHP Midyear Clinical Meeting present real-world anticoagulation clinical cases and their reasoning and approach to managing their care. In this episode, our patient-specific cases include inpatient management of a pulmonary embolism, management of coagulopathy with liver disease, and management of anticoagulants in acute kidney injury.
