"I've built a good practice of people who value my work, who understand it, understand what we're doing, and they're invested." "If you're not doing the things that most people don't think is overkill anyway, then you might be held liable for that." "We gotta see the silver lining here and really push our infection control to be improved." "Approach it from a place of being helpful rather than being sales-y." It's A Tale of Two Hygienists' monthly student roundtable episode, which means Andrew and Michelle have invited a trio of guests to educate student listeners on a chosen topic. This week, they are discussing human papillomavirus infection (HPV infection) and associated oropharyngeal cancers. Spring Hatfield is a dental hygienist who educates patients on the oral-systemic disease connection. Kathryn Gilliam is a practicing clinician, author, speaker, as well as a clinical coach and consultant; she is the founder of PerioLinks, which offers specialized coaching programs to oral healthcare professionals. JoAnn Gurenlian is the graduate program director at Idaho State University and a 40-year veteran of dental hygiene. In this episode, our guests break down what goes into a comprehensive HPV examination, discuss its sexual and non-sexual modes of transmission, and impress the importance of taking patients' sexual histories. EPISODE HIGHLIGHTS: Interview starts: 4:10 - Kathryn discusses how frequently the average person comes into contact with HPV throughout their lives and explains why tactility plays such an important role in examining patients for related cancers. - Why the current trend in birth control among sexually active high school students does little to control the spread of HPV. - Spring looks at how too few primary care physicians and physicians' assistants are screening their patients for HPV-related cancers or even discussing the subject with them. - Examining the non-sexual modes of transmission for HPV and why immune system-strengthening is so vital in preventing transmission. - JoAnn impresses the necessity of continuing education programs that would help professionals perform more effective oral examinations and comfortably converse with their patients about HPV and sexual history. - Kathryn examines the unfortunate pattern of passing the buck throughout the medical profession when it comes to discussing sexual history. QUOTES: "It's so critical to perform the extraoral head and neck exam because these cancers in the oropharynx are beyond our visual field." "I don't think most hygienists/dentists have this discussion with their patients about the association of HPV with oropharyngeal cancer." "If you haven't ever asked questions that seem this personal, it's helpful if you practice." "With a little bit of experience and a little bit of focus on the patient, we can become very accomplished in this role of taking a sexual history." "We have to elevate our identities for ourselves so we can elevate our identities for our patients." "We have a huge responsibility for the lives of our patients." "We are the preventative oral health specialists, so go prevent something." Ep 214214 Sonya Dunbar, "The Geriatric Toothfairy"

A Tale of Two Hygienists is launching a series on how professionals and patients integrate dental treatment with other forms of medical care. For this week, Andrew and Michelle have invited a guest who specializes in oral health care for the senior population. Sonya Dunbar has been a dental hygienist for over 30 years. Known as "the Geriatric Toothfairy," she is the founder and CEO of Mobile Dental Xpress, which brings oral health care to elderly patients housed in long-term care facilities. Sonya is a national speaker on the importance of oral care for elderly patients, trains nursing home staff, and also provides seniors with "S.O.S. packs" that contain 3-6 months of oral health care products. In this episode, Sonya discusses the reasons why younger hygienists are hesitant to work with seniors, explains why they should move past these apprehensions, and makes a case for why you should sponsor one of her S.O.S. Packs. EPISODE HIGHLIGHTS: Interview starts: 10:16 - Sonya explains why working with seniors has been a lifelong passion for her. - Why so many younger hygienists and dental professionals are averse to treating the elderly. - What services does Mobile Dental Xpress provide its patients and how do they charge for them? - How Sonya trains her team, holds them accountable, and meets any challenges that come their way. - Some of the more frequent ailments amongst the senior population. - Why professionals in private practices should make a point of scheduling geriatric patients for either the first or last session of the day. - The reason Sonya and her team typically don't treat patients in short-term care. - What can be found in Sonya's S.O.S. packages and how you can sponsor one. - How to tell if an elderly patient at your private practice is no longer able to take care of their oral health as well as they used to. QUOTES: "If you put me between a rock and a hard place, the hard place is gonna lose because I'm coming out fighting!" "I'm on your team and we're here to save these residents, and this is what we're gonna do." "We need to bring awareness to our seniors and long-term care facilities because it's like they're a forgotten population and it's making my heart break." "The mouth is the window of your health." "It's not easy; it has to be something you want to do." LINKS: The Geriatric Toothfairy - https://www.geriatrictoothfairy.org/ S.O.S. Packs - https://www.geriatrictoothfairy.org/sos-packs/ S.O.S. Ep 213213 Student Focused Presents - Sealant Literature Review

It's A Tale of Two Hygienists' monthly student roundtable episode, which means Andrew and Michelle have invited a trio of guests to educate student listeners on a chosen topic. This episode's subject is all about sealants: what are the most effective sealing materials? What can they prevent? And when can they prove to be a hazard? Jason Luchtefeld (previously heard on episodes 64 and 145 of A Tale of Two Hygienists) is a dentist based in Robinson, Illinois who has been practicing for 20 years. Julie Frantsve-Hawley (previously on A Tale of Two Hygienists #68) works with the nonprofit group DentaQuest's Partnership for Oral Health Advancement. Spring Hatfield currently works in a small Alabama practice, having previously been a receptionist and assistant before becoming a dental hygienist. In this episode, our guests summarize and analyze the readings on sealants they brought to this roundtable, debate the finer points of what counts as permanent or non-permanent restorations, and discuss the dreaded "restorative death spiral." EPISODE HIGHLIGHTS: Interview starts: 3:03 - Jason summarizes the 2017 literature review "The Use of Pit and Fissure Sealants," which discusses the wide variety of materials utilized in sealants since the procedure began in the 1920s. - Julie reviews "Barriers to Sealant Guideline Implementation within a Multi-Site Managed Dental Care Practice"—which she actually co-authored—and discusses how pit and fissure sealants can play a key role in "secondary prevention" when applied to non-cavitated carious lesions. - Spring reviews a randomized clinical trial from last year that tested whether resin-based sealants could prevent non-cavitated carious lesions from progressing. - What a study on color taught Julie about the power of terminology and puns. - Why Jason doesn't think there's a significant difference between a permanent restoration and preventative, non-permanent restoration. - Spring explains what the "restorative death spiral" is. - Take care to ensure that your patients can see to their basic dental needs at home, even if it means just giving them free toothbrushes. QUOTES: "Isolation is critical for the success of sealants." "If you have dentists that are claiming sub-optimal skills for placing a sealant, then they shouldn't be placing fillings or crowns." "If we don't have these terms and have general consensus on what they mean, that instance in itself is having a barrier!" "Always be inquisitive and be open to change your mind when faced with evidence." "Just because you learned it in school doesn't mean you do it your whole career!" Ep 208208 Student Focused Presents: Treating Patients with HIV

For the latest student roundtable, A Tale of Two Hygienists has invited on three guests who either work with patients affected by HIV/AIDS or have studied the virus to debunk myths and misconceptions surrounding this epidemic. Daniel Lopez has been a hygienist since 2012; since graduating, he's been working with Access Community Health Center in Manhattan, which focuses on LGBTQ patients and the treatment of HIV. Amber Riley is based in San Diego and has been a dental hygienist for over two decades and works in a suburban practice; a small portion of her patients live with HIV and she's seen firsthand how the prognosis of those living with the virus has changed over the years. JoAnn Gurenlian has over 40 years of dental hygiene experience under her belt; she is the graduate program director at Idaho State University, and back in the 1980s she was part of a task force that investigated HIV/AIDS during the initial outbreak. In this roundtable, our guests discuss how we can prevent exposure to and the spread of HIV, explain the importance of "patient navigators" and how oral health care professionals should adjust their treatment for patients with HIV/AIDS. EPISODE HIGHLIGHTS: Interview starts: 3:05 - JoAnn breaks down the difference between quantitative and qualitative studies and the benefits of each. - What treatments and methods can be used to prevent exposure to and proliferation of HIV? - JoAnn discusses the positive correlation between patients' sexual orientations and their health care providers' own orientations. - The ins and outs of HIV PrEP (Pre-Exposure Prophylaxis): prescribing, side-effects, and what advancements are being made. - Who are "patient navigators" and why are they so essential for helping people get the treatment they need? - How Maslow's hierarchy of needs can determine whether or not people seek out dental treatment and care. - Dispelling the idea that HIV contraction is the result of promiscuity. - What does—and doesn't—change for our guests' usual standard operating procedures when they learn a patient has HIV/AIDS. QUOTES: "We're pretty much the future model of what health care is trying to shift towards." "For anyone who feels that their risk of exposure can be high, this is a preventative procedure." "We need to find a way to talk about risk in a positive way." "Sexual health is absolutely demonstrated in the oral cavity." "Every generation is less toxic than the last." "If someone is virally suppressed, there are no documented cases of them transmitting the virus." "You have to learn to be discerning about what is good research." Ep 207207 Airway Health with Julia Worrall!

Sometimes the real challenge in treating an airway disorder is getting the patient to actually take our advice—which can mean encouraging the other care providers a patient has to speak to them about the problem. This week's guest believes in not only treating airway disorders within our profession, but reaching out to other medical fields to ensure these problems are comprehensively addressed. Julia Worrall, "The Sleep RN," is a registered nurse and the program director of the FAH Airway Advocate Program, which advocates for greater awareness and treatment of airway issues. A member of the National Commission on Correctional Health Care, she works tirelessly to address the epidemic of sleep disorders affecting Americans today. In this episode, Julia explains how hygienists can sound the alarm on airway issues, discusses how dental professionals can link up with people in other fields to tackle these problems, and suggests ways we can more effectively reach out to patients about airway concerns. EPISODE HIGHLIGHTS: Interview starts: 16:30 - Why Julia really appreciates the learning curve aspect of her work, even if it means having to discard what she thought to be true. - Overcoming our natural impostor syndrome as the dental community takes on more and more medical issues. - How dental professionals can become "first responders" when it comes to airway issues. - Why how we communicate our airway concerns to patients can affect if they actually take our advice. - On building bridges with nurses and other members of different medical professions. - How our knowledge of airway issues has changed over the course of 2019—and what Julia says 2020 has in store for us. - Getting over your fear of admitting "I don't know." - What's the most interesting thing Julia has learned at this conference? QUOTES: "We're also professionals and that means we don't stay stuck in what we believe to be true when there's new evidence." "You have to balance your time with what you're teaching and what you're learning." "Own your space, own your knowledge." "Each patient that you have deserves 100%." "I wanna help you guys get to where you're seen, you're heard, you're validated." Ep 206206 Oral Bacteria and Alzheimers with Anne O. Rice

While you might not see the connection at first, there is a connection between poor oral health, poor sleep, and neurodegenerative diseases such as Alzheimer's. To interview this week's guest about the subject, Michelle is joined by the similarly named Machell Hudson, who co-hosts the Reflection Health Podcast on channel 1 of A Tale of Two Hygienists' own Dental Podcast Network! Anne O. Rice is a registered dental hygienist and the Sleep Care Coordinator for Sleep Group Solutions, which specializes in dental treatment of snoring and sleep apnea. In this episode, Anne discusses the relationship between bacterial infection and Alzheimer's/dementia, explains when you need to say "no" to a patient, and reveals why you need to become very good friends with berry hibiscus tea. EPISODE HIGHLIGHTS: Interview starts: 3:17 - Michelle and Machell reflect on when they first met and how they realized they were kindred spirits. - How bacteria, yeast and the herpes virus are "flamethrowers" that kickstart Alzheimer's and associated conditions. - Why dental professionals need to impress the long-term effects of bacterial infection to patients, rather than just the aesthetic effects. - What should hygienists be looking for when examining patients for bacterial infection? - Anne's argument for why dental professionals need to be extremely supportive of patients living with Alzheimer's and similar diseases. - Breaking down a study that revealed a significant link between blood pressure and brain health. - The relationship between sleep apnea, poor sleep, and neurodegenerative diseases. - The science behind why exercise is so crucial for brain health. - Taking the proper precautions when working with patients dealing with bacterial infection. - How can we make the most of long wait times in a way that benefits patient education? "I'm not asking anyone to do anything more than what they're doing, just tweak a little bit and be a little more educated." "Vessel health is so crucial to brain health." "Mid-life high blood pressure astronomically increases your risk for Alzheimer's disease." "Sleep is one of the biggest things. We just really need to take it seriously." "Let me be crystal clear: if somebody has a cold sore, please, don't treat them. For their own sake!" "Let's protect ourselves a little bit more." Ep 205205 Vitamins, Gut Health, and Sleep with Dr. Stasha Gominak

Michelle is joined by co-host Eva Masters Ramsey for the latest A Tale of Two Hygienists interview to discuss anything and everything pertaining to the unexpected relationship between sleep, vitamin intake and oral health. Stasha Gominak is a neurologist-turned-sleep coach who learned of the link between chronically poor sleep and chronic illness. As part of RightSleep, she investigates how a little bit of vitamin deficiency can go a long way to causing long-term impacts on the body, including airway paralysis during REM sleep—which leads to a whole host of oral issues! In this episode, Stasha discusses how sleep studies led her to focus on the relationship between vitamin intake and bacteria, reveals some of the most important discoveries she's made in the course of her research, and explains how lifestyle changes in the last 200 years have massively impacted health across multiple cultures. EPISODE HIGHLIGHTS: Interview starts: 19:21 - How did a neurologist like Stasha end up focusing on vitamins and working in the dental field? - The unusual and inexplicable commonality Stasha found among a shocking number of sleep study participants. - Why Stasha believes there's a link between REM sleep and many physical ailments. - How Stasha stumbled across the overlooked relationship between Vitamin D and paralysis during sleep. - Stasha's advice for effectively supplementing your vitamin intake. - Why it's vital to figure out the correct dosage of Vitamin D for you and your family members. - Stasha relates some of her adventures (and misadventures) in administering the right dosage. - The reason so necessary to keep your "bugs"—i.e. gut bacteria—healthy. - Stasha breaks down the upcoming workbook she's preparing for expectant mothers. QUOTES: "I think as long as we cure people, we can feel confident that we know what we're doing." "Every single one of these people is getting paralyzed too much or not getting paralyzed enough!" "Nobody makes money off of Vitamin D—in fact, it's free! If you walk outside without sunscreen you make it yourself!" "For every single hormone, too much and too little both screw us up." "I recommend that anyone who's listening to this does not play around with Vitamin D unless you're serious about it." "The same thing that makes you awake and alert in the day makes you sleep at night." "The ultimate outcome is: I don't ever have to have a cavity filled again." "We should really come into the discussion with a viewpoint of collaboration and compassion." Ep 203203 Budgets, Business, and Benchmarks

Continuing its deep dive into the business side of dentistry, this week A Tale of Two Hygienists is examining the nitty gritty of office budgets and what makes for a healthy working relationship between a dentist and their hygienists. Dianne Glasscoe Watterson, RDH, has been working in the dental industry since 1972. She actually started her career not as a hygienist but as a dental office manager. Now, she takes a three-pronged business: writing, speaking and consulting. Her latest book is Real World Dental Hygiene: Where the Rubber Cup Meets the Road In this episode, Dianne discusses why she made the shift from office management to hygiene, examines which expenses take the biggest chunk out of a dental office's budget, and explores some of the legal issues surrounding downtime. EPISODE HIGHLIGHTS: Interview starts: - How a very unexpected absence started Dianne on the path from working the front desk of her office to becoming a hygienist. - The visit from a practice management consultant that inspired Dianne to go back to school and continue her education. - What Dianne hopes readers will get out of her latest book. - Why a dental office's primary dentist has an impact on how much hygienists do or do not gravitate toward the business side of their field. - The reason dental supplies eat so much into an office's overhead. - Taking a close look at which benchmarks a successful dental practice should meet. - Why everyone suffers if dentists keep their hygiene fees low. - Dianne's strategy for minimizing downtime, and why she'd like to see a punch-clock case go to court. - Discussing hygienists owning percentages of their dental office—plus the one office that's actually wholly owned by hygienists. - Dianne's parting advice to listeners emphasizing the sheer amount of support a well-trained hygienist can bring to an office. QUOTES: "That day I was baptized by fire into chairside assisting." "The numbers tell a story." "If a practice is financially healthy, then the doctor can better afford to take care of the people who work there." "75% of all restorative needs are discovered in hygiene visits." "Doctor should not minimize the importance of hygiene procedures and keep those fees low when they're raising up everything else. It's just not fair and it's not right." "The more money that comes in, the better the doctor can take care of the people who work there." "Many, many hygienists do not know what their salary-to-production ratio is." Josey is the Vice President of Education at Dental Intelligence, Inc. and until very recently the Chief Operating Officer at CarolinasDentist. The subject at hand is teledentistry, where technology allows dental professionals to examine and diagnose individuals who may not be able to make it to a clinic. And a big thanks to MouthWatch for helping to put this discussion together! Carol Roszel, RDH, is the oral health director for Little City Foundation, an organization that works with intellectually and developmentally disabled individuals. Staci Stout is a hygienist and the owner of Dentacare and the corresponding Dentacare Foundation, which provide oral health care to schools, nursing homes and corporations. Elaine Mamola is the founder of Smiles By Delivery, which utilizes teledentistry as a means of providing homebound seniors and some veterans with proper care. Brittany Kinol is a dentist and the founder of Miracle Dental Associates, which helps underserved children in the Pittsburgh area. In this episode, our guests explain how teledentistry makes their services much more accessible than they otherwise might have been, discuss some of the techniques and processes used to bring dentistry into the home, and explore how hygienists are absolutely essential to remote work. EPISODE HIGHLIGHTS: Interview starts: 6:51 - Our guests explain how they provide underserved populations with dental care and how teledentistry plays a major role in that. - How hygienists are key players in making teledentistry happen. - Some of the more unusual substitutes for denture relines Elaine has seen in her time. - How technology expedites—and occasionally complicates—teledentistry work. - Staci breaks down the difference between synchronous and asynchronous teledentistry. - Carol explains how much information is needed to make a remote identification or diagnosis. - Why Carol believes teledentistry is such a major advancement to the field. - What are some of the legislative complications of teledentistry? - Brittany shares her love for dental hygienists and why she sees them as a progressive force. - Our guests advise listeners on how they can get started with teledentistry. QUOTES: "You're utilizing this dentist a lot better because he can be providing exams in multiple locations." "Teledentistry really kind of closes those gaps and it makes the care much more efficient." "Not only has [teledentistry] enhanced our treatment, but it has increased the number of people being seen." "Teledentistry is truly one of the biggest advancements in dentistry in decades." Ep 198198 Finding Clarity with Dental Endoscopes

Periodontal surgery can be essential in treating oral disease that goes below the gumline, but the very thought of going through such a procedure puts off patients who would greatly benefit from it. Luckily, Michelle and Lacy were able to chat with two hygienists who have firsthand experience with a tool that can make at least some of those surgeries unnecessary. Nicole Fortune is a dental hygienist and marketer for Champlain Periodontal in South Burlington, VT who assists with many surgical and non-surgical oral procedures. Frances Tryon is a trainer at OraVu, which manufactures its own dental endoscope along with other tools that can be used to deeply clean the root of oral disease without necessitating surgery. In this episode, Nicole and Frances lay out the benefits that dental endoscopes provide, take a VERY close look at periodontal surgery, and explain what it takes to train people on endoscope operation. EPISODE HIGHLIGHTS: Interview starts: 8:12 - What exactly is a dental endoscope, what can it do that basic oral examination can't, and how has it evolved over the years? - Nicole and Frances relate their own experiencing using endoscopes and why they believe they're beneficial. - Getting into the nitty-gritty of traditional periodontal surgery and understand why professionals view endoscopic surgery as the preferred alternative. - Exactly how portable is endoscopic technology, and Frances prefers to train people one-on-one in its usage. - How long does it take to train somebody on a dental endoscope? - The tricky task of navigating dental implants while using an endoscope. QUOTES: "It's really not new technology—it's just better technology now." "To me, fractures in the tooth is like the black hole of dentistry." "Patients want more options—minimally invasive and predictable." "Our job is to find the tool that will do the least amount of harm but gets us the maximum amount of benefit to the patient."
